In response to a congressional request, this book evaluates the extent to which state efforts have been effective in reducing the perinatal transmission of HIV. The committee recommends that testing HIV be a routine part of prenatal care, and that health care providers notify women that HIV testing is part of the usual array of prenatal tests and that they have an opportunity to refuse the HIV test. This approach could help both reduce the number of pediatric AIDS cases and improve treatment for mothers with AIDS. Book excerpt: Mother To Child Transmission (MTCT) of HIV is a major challenge in Sub-Saharan Africa due to a variety of socio-economic and political factors. In South Africa, for example, there was element of denialism by government on the fact that treatment could assist in reducing the likelihood of transmitting the virus to the baby. It was only in 2001 after the Treatment Action Campaign took the South African government to court that they were ordered to develop a programme to prevent the transmission of HIV from mother to child. The South African government established a PMTCT programme that aligned itself with the United Nations' Millennium Goal of ensuring that all HIV positive pregnant women receive treatment in an effort to eliminate babies born with HIV. This study, therefore, explored the experiences of HIV positive mothers who enrolled on PMTCT programme at Tonga hospital. The researcher explored the participants' understanding of the programme prior to enrolment, the extent to which they experienced the programme, the nature of support and services they received while on the programme as well as challenges they faced. All this was done with the intention and commitment to strengthening the intervention strategies for the HIV pregnant women, thereby ensuring that they receive top quality services from a group of multi skilled professionals. To achieve this, the study applied a collective case study within a qualitative approach. The population for the study was HIV positive mothers who enrolled on the PMTCT programme between June 2011 and July 2012. The sample consisted of 12 HIV positive mothers who took part in the PMTCT programme at Tonga hospital. For data collection purposes, the researcher applied semi-structured interview. Informed by the findings, the study concluded that there is lack of knowledge and understanding of the programme in the community. It further established that the clinic is situated under the ward where HIV positive patients are treated and that psycho social services are not offered to the HIV positive pregnant women. It also concluded that the women's husbands or partners are not encouraged to be part of the programme. Consistent with the above, the study recommended that the PMTCT programme should be provided in a holistic and well integrated manner, where all health care disciplines contribute as required to make the participants' experiences on the programme more comfortable both socially and emotionally. There should be a way to get the male counterparts of the participants more involved in the programme so as to afford them the opportunity to get first-hand information about pregnancy and what is expected of them as partners. Each health worker's role should be clearly defined and a referral system be developed so that all services are easily accessible.

Additionally, acute HIV infection among pregnant and breastfeeding women poses a double burden in that acute maternal HIV accounts for nearly one-third of all mother-to-child transmissions of HIV (MTCT). To reach global targets for elimination of MTCT and HIV prevention for mothers, it is critical to integrate effective primary HIV prevention strategies into maternal and child health (MCH) services. Tenofovir disoproxil fumarate (TDF)-based pre-exposure prophylaxis (PrEP) prevents HIV infection in adherent women. The World Health Organization (WHO) recommends PrEP for all individuals, including pregnant and breastfeeding women, at substantial HIV risk (defined as HIV residence in regions where HIV incidence is >3%). Programmatic delivery of PrEP for pregnant women is currently being considered in high-prevalence regions, though implementation approaches that efficiently optimize the benefit of PrEP during pregnancy have not been defined. Additionally, although WHO guidelines support PrEP use in pregnancy, national committees have differed in their conclusions. For example, PrEP use during pregnancy is supported by Kenyan antiretroviral guidelines but the lack of complete safety data led PrEP to be contraindicated for pregnant women in the current South African PrEP guidelines. As countries expand programmatic delivery of PrEP to pregnant women, it is important to understand motivations and beliefs for using PrEP during pregnancy to address concerns unique to this population. The studies within this dissertation address the implementation science gaps described above for the delivery of PrEP for HIV prevention to pregnant and postpartum in sub-Saharan Africa. To inform efficient PrEP delivery models, we present an empiric risk score for identifying pregnant and postpartum Kenyan women at highest risk for HIV acquisition who would mostly benefit from PrEP while reducing unnecessary exposure among low-risk women. Using data that could be easily collected in standard MCH clinic settings without additional laboratory diagnostics, our risk score identified 56% of pregnant women who acquired HIV among just 16% of women. Using register data from 62 antenatal MCH facilities throughout Kenya, we further estimated the absolute number and proportion of HIV-uninfected pregnant women in Kenya who could be offered PrEP under different public health approaches, including offering PrEP universally or based on either regional HIV prevalence and/or individual-level HIV risk factors. We found that offering PrEP only to pregnant women in the region with highest HIV prevalence (Nyanza) would reduce PrEP use among low-risk women by 74%, but exclude 63% of women with high risk for HIV based on individual-level characteristics nationally. To complement service delivery data, we also assessed experiences of using PrEP during pregnancy among HIV-uninfected Kenyan women in HIV-serodiscordant couples who became pregnant while using PrEP. The personal experiences of women with direct exposure to PrEP during pregnancy offers valuable insights for informing development of effective PrEP messaging strategies and programs. Finally, we evaluate whether adverse perinatal outcomes were more frequent in a cohort of Kenyan and Ugandan HIV-infected women who used TDF-containing antiretroviral therapy (ART) during pregnancy compared to HIV-infected women who used ART during pregnancy that did not contain TDF. Our findings support the growing evidence that prolonged prenatal TDF use is not associated with adverse perinatal outcomes and contribute to the few prospective studies evaluating the safety of TDF use during pregnancy from African cohorts. The studies within this dissertation aim to address these implementation science gaps and inform optimal and effective delivery of PrEP for HIV prevention to pregnant and postpartum in sub-Saharan Africa.

However, even though the programme is virtually effective in developed countries, many developing countries are reporting child HIV infections due to the MTCT. The programme has existed in Kenya for more than a decade, yet in 2011, 12,894children were HIV infected due to MTCT Objective: To evaluate the PMTCT programme, especially the HIV testing from the antenatal period to the postnatal period among expectant parents attending Nyeri Provincial General Hospital in Central Province, Kenya. Design: Retrospective analysis of the hospital registers. Methods: Three hospital registers were analysed for the period from July 2009 to September 2012. The registers were for antenatal, intrapartum and postnatal care respectively. Each register documented the utilisation of PMTCT services by the expectant parents. Descriptive and inferential statistics were produced to analyse data from the registers. Results: The PMTCT services utilisation was sub-optimal. Of the 504 expectant mothers who attended the antenatal clinic, 59.9% came once, 80.4% had their first visit in the third trimester (between weeks 28 and 40) and only 6.9% were accompanied by their partners. All the women were HIV tested in their first visit but only 12.1% were rescreened after three months, and only 3.8% had been tested prior to the current pregnancy (p=0.000). No expectant mother was tested for HIV intrapartum or postpartum. The children of the 504 mothers who were HIV tested were those whose parent/s were known to be HIV positive or who had presented to a child welfare clinic with recurring symptoms suggestive of a failing immune system. Conclusion: Public health programs need to strengthen the PMTCT and HIV prevention programmes to ensure that HIV testing preconception and in pregnancy is fully implemented and strengthened, alongside continued education of the public through community programmes and the media. To avert further horizontal and vertical transmission of HIV, there is a need to address urgently the identified missed opportunities in the PMTCT program. These programmatic challenges require health system redesign and strengthening, resource allocation, addressing research gaps and reassessing the current PMTCT policies.
