Indeed many animal tests developed since the 1960s have been optimised for the benzodiazepines and some programmes have even screened candidates as potential anxiolytics on their benzodiazepine-like side-effects rather than their anxiolytic activity. With the realisation of the drawbacks of the benzodiazepines, namely their potential for tolerance and dependency, there has been a renewed interest in alternative anxiolytics both from existing drugs such as the tricyclic and monoamine oxidase antidepressants and from newer agents such as buspirone. In addition anxiety is no longer considered to be a unique entity but rather an umbrella term for a series of specific anxiety disorders such as panic disorder without or with agoraphobia, generalised anxiety disorder (GAD), specific phobias, social phobias and post-traumatic stress disorder (PTSD). These new clinical categories have opened another dimension in the therapy of anxiety requiring the optimisation of treatments for different syndromes. This book is a critical review of today's anxiolytics and those that may become the anxiolytics of tomorrow. What is clear is that currently there are few clinically satisfactory alternatives to the benzodiazepines for the treatment of acute anxiety. For chronic anxiety, it is generally agreed that benzodi azepines are not the treatment of first choice. The tricyclic and monoamine oxidase antidepressants, the serotonin reuptake inhibitors and buspirone offer better solutions for chronic anxiety but they are still far from being ideal.

Book excerpt: This book covers the entire field of research in the area of minor tranquillizers and its application to current clinical practice in the treatment of anxiety and insomnia. These drugs are principally the benzodiazepines and related drugs with a similar mechanism of action, such as zolpidem and zopiclone. The molecular mechanism of action of benzodiazepines is described, focussing on the interaction of these drugs with the different isoform of the GABAA^O >receptor, and the consequences of this for brain function. Recent advances in this knowledge have provided a framework for defining the physiochemical nature of the interaction between such drugs and their receptor protein, and thus pave the way for the design of new anxiolytic and hypnotic drugs. The animal models available for evaluating the potential of such new therapeutic agents in the treatment of anxiety and insomnia are discussed. Furthermore, understanding of the physiological regulation of the GABAA receptor may provide insights into the aetiopathology of these diseases. The clinical use of benzodiazepines and related drugs in the treatment of anxiety, insomnia, epilepsy and as anaesthetics are explored. The advantages and limitations of such treatments are discussed, and the impact of drugs evaluated. The Health Insurance Study (HIS) used data on the prevalence and adverse consequences of use to investigate the effect of different levels of health insurance on health and the quality of medical care. Consumption of these drugs was chosen because of their widespread use and the possibility of affecting possible adverse consequences by good medical care. This volume reviews pertinent drug use literature, outlines HIS methods for measuring the prevalence and intensity of use, discusses results from the HIS enrollment population, and lists criteria for acceptable quality of care for persons presenting with complaints associated with the prescription of these drugs. Illustrative findings include: (1) up to 3 and 18 percent of HIS participants were hypnotic or anxiolytic drug users, respectively; (2) women used more drugs than men; and (3) impact from their use was lowest among persons who took them least frequently.
