Available in PDF, EPUB and Kindle. Book excerpt: In the past ten years, there have been significant advances in the diagnosis and therapy of arterial vascular disease. While long-term morbidity can only be improved by changing the patient's life-style and removing any risk factors that may be present(hypertension, hyperlipidemia, obesity, diabetesmellitus, nicotine abuse), symptomatic relief may be obtained by various procedures. Besides conservative treatment of arterial occlusive disease (physical therapy, medical treatment) and surgical vascular interventions (thromboendarterectomy, bypass procedures), balloon catheters introduced percutaneously under local anesthesia have found increased application in virtually all areas for the dilatation and recanalization of obstructive lesions in the past few years. The technique of balloon dilatation, as introduced by Griintzig as a further development of the percutaneous interventions by Dotter and Judkins, is no longer the decisive issue. The main objective of present clinical research is to de termine the proper indications and patient selection for this procedure, which has found its place between conservative treatment and surgery. The contents of this Symposium should provide some guidelines for the indi cations and postprocedural therapy for the referring physician, the angiologist, and for the radiologist and cardiologist performing the dilatation. We owe our thanks to all the specialists involved, who have provided us with the benefits of their experience. Mrs. H. Beilmann and Dr. M. Wojtowycz con tributed substantially to the preparation of the text. I would like to thank them and the staff of Springer-Verlag for their conscientious work in the interest of composition and publication quality.

Available in PDF, EPUB and Kindle. Book excerpt: Chronic total occlusions (CTO) are common, and found in approximately one third of patients with significant coronary artery disease who undergo angiography. Over the past 20 years, CTO lesions have represented the most difficult anatomy for treatment — with lower success rates and higher complication rates. Chronic Total Occlusions provides interventionalists insight into the world of CTOs with introductory chapters that describe the pathology and indications of CTOs along with a review of clinical trials. Imaging modalities including CT angiography, magnetic navigation wire, and IVUS guided recanalization of CTO are also introduced, together with information on new wires technology and devices for CTOs. With numerous illustrations of these devices, technologies, and strategies to improve the CTO success rate, this clinical guide, headed up by Ron Waksman, will prove to be the ideal companion for interventional cardiologists and cardiac surgeons who are required to perform angioplasty and coronary stenting.

Book excerpt: This book is open access under a CC BY 4.0 license. This quick-reference handbook offers a concise and practical review of key aspects of the treatment of ST-segment elevation myocardial infarction (STEMI) in the era of primary percutaneous coronary intervention (PPCI). In the context of STEMI, PPCI is the preferred mode of emergency revascularization. Access to PPCI is rapidly increasing and is now routinely practiced in both general and specialist hospitals and there has been a recent emphasis on developing STEMI networks to enhance and expedite the referral pathway. This coupled with concurrent developments to enhance the safety and efficacy of the PPCI procedure has heralded an era where STEMI interventions are increasingly considered an important subspecialty within interventional cardiology. Written by leading cardiologists who have been instrumental in the adoption of PPCI in their respective institutions, the book provides junior and senior cardiologists alike with insightful and thought-provoking tips and tricks to enhance the success of PPCI procedures, which may in turn translate into direct improvements in outcomes. The book is also relevant for healthcare providers and emergency department physicians.

Book excerpt: Operator skills, and in particular decision-making and strategic skills, are the most critical factor for the outcome of catheter-based cardiovascular interventions. Currently, such skills are commonly developed by the empirical trial and error method only. In this textbook, for the first time, an explicit teaching, training, and learning approach is set out that will enable interventional operators, whether cardiologists, vascular surgeons, vascular specialists, or radiologists, to learn about and to develop the cognitive skills required in order to achieve consistent expert-level catheter-based interventions. It is anticipated that adoption of this approach will allow catheter-based interventions to become a domain of excellence, with rapid transfer of knowledge, steep learning curves, and highly efficient acquisition of complex skills by individual operators — all of which are essential to meet successfully the challenges of modern cardiovascular care.

Available in PDF, EPUB and Kindle. Book excerpt: In the past two decades a number of studies have shown that abnormalities in the function and structure of coronary microcirculation can be detected in several cardiovascular diseases. On the basis of the clinical setting in which it occurs, coronary microvascular dysfunction (CMD) can be classified into four types: CMD in the absence of any other cardiac disease; CMD in myocardial diseases; CMD in obstructive epicardial coronary artery disease; and iatrogenic CMD. In some instances CMD represents an epiphenomenon, whereas in others it represents an important marker of risk or may contribute to the pathogenesis of myocardial ischemia, thus becoming a possible therapeutic target. This book provides an update on coronary physiology and a systematic assessment of microvascular abnormalities in cardiovascular diseases, in the hope that it will assist clinicians in prevention, detection and management of CMD in their everyday activity.

Book excerpt: This book offers a systematic introduction to Budd-Chiari syndrome, a rare but life-threatening vascular disorder of the liver, which refers to obstructions at any site from the hepatic veins to the suprahepatic inferior vena cava. The book mainly covers the history, epidemiology, etiology and risk factors, pathology, diagnosis, pharmacological and interventional aspects, surgical treatment, etiological treatment, and prognostic assessment of Budd-Chiari syndrome. Notably, it also presents cutting-edge research findings in this field. The early diagnosis of Budd-Chiari syndrome is vital, as it allows clinicians to pursue more targeted and effective treatment strategies, thus improving patient outcomes. However, much information, including the latest advances, has not yet been fully disseminated in current clinical practice. This book addresses that gap, helping physicians to improve diagnosis and establish standard treatment strategies, helping researchers conduct clinical and experimental studies, and even helping patients and their relatives to recognize this disease.
