Afin de répondre à cette question, quatre axes d'analyses ont été développés à travers l'évaluation des caractéristiques de l'enfant (le sentiment d'auto efficacité, les attitudes face à la maladie, les représentations de l'asthme) et de ses parents (le sentiment d'efficacité dans la gestion de la maladie de leur enfant, les représentations de l'asthme et leur implication dans le traitement). Puis, à partir de ces quatre axes, deux propositions de modélisations de l'influence de ces variables sur la qualité de vie et l'adhérence thérapeutiques des enfants ont été réalisées. Cette recherche a été menée auprès de 144 enfants asthmatiques âgés de 8 à 12 ans et de 92 parents. L'analyse des données empiriques a permis de mettre en évidence, pour la première fois, le rôle essentiel des représentations cognitives et émotionnelles de l'asthme chez les enfants, sur leur qualité de vie et leur adhérence. Accéder à ces représentations de la maladie que l'enfant asthmatique et ses parents se construisent, devrait permettre d'adapter les interventions au sein des programmes d'éducation des Ecoles de l'asthme avec comme objectifs une amélioration de la qualité de vie et une meilleure adhérence thérapeutique de l'enfant.

C'est la pathologie chronique la plus fréquente chez l'enfant. Les dépenses liées aux hospitalisations ou aux recours aux services d'urgences sont conséquentes. La qualité de vie des enfants et de leurs parents est directement affectée et ce, malgré l'efficacité reconnue des traitements. Ces hospitalisations seraient en grande partie évitables par une éducation thérapeutique des enfants et de leurs parents. Pour être efficace, cette éducation doit être adaptée aux croyances et aux représentations personnelles. Nous savons que ces dernières influencent les conduites et l'observance. Objectif. Nous voulons évaluer l'impact d'une fiche d'information standardisée distribuée aux parents d'enfants asthmatiques et les informant sur la maladie. Nous évaluerons l'état des connaissances et les représentations liées à cette maladie. Matériel et Méthode. Les enfants sont inclus aux urgences pédiatriques du CHU de Saint-Étienne et en cabinets de médecine générale sur une période totale de dix-huit mois. Leurs parents sont invités à remplir un questionnaire caractérisant la maladie de leur enfant, leurs connaissances et leur représentation de l'asthme. Ils reçoivent à cette occasion une fiche d'information sur l'asthme. A six semaines, ils sont recontactés pour évaluer l'évolution de leur enfant et leur satisfaction concernant la fiche informative. Résultats. Trente-huit parents ont été inclus. En un an, 74 % des enfants utilisaient au moins 2 flacons de béta-2-mimétiques de courte durée d'action. Plus de la moitié des enfants (66 %) faisait plus de 4 crises par an. Cinquante-huit pourcents des parents pensait que les enfants ne guériraient pas de leur asthme à l'âge adulte ou ne savait répondre. Quatre-vingt-sept pourcents pensaient que l'asthme altérait la qualité de vie d'un enfant. Quatre-vingt-quatre pourcents des parents pensaient que l'environnement (tabac, poussière, poils d'animaux) prédisposait les enfants à devenir asthmatiques. Soixante pourcents des parents pensaient que les enfants asthmatiques étaient prédisposés génétiquement à le devenir. Cinquante-huit pourcents des parents pensaient que les traitements étaient moins efficaces pris tous les jours ou ne savait se prononcer. Cinquante-trois pourcents pensait que les effets secondaires de la cortisone orale pouvaient être graves. Cinquante-et-un pourcents se déclarait anxieux, angoissés ou stressés au moment d'une crise. Concernant la fiche d'information, 97 % des parents jugeait la fiche utile et la moitié d'entre eux se déclarait plus à l'aise avec la maladie. Soixante-huit pourcents des parents déclarait qu'elle avait modifié leurs connaissances et soixante-cinq pourcents qu'elle avait modifié l'image portée sur l'asthme de leur enfant. Discussion et conclusion. La connaissance des parents au sujet de l'asthme de leur enfant ainsi que le contrôle de la maladie sont insuffisants. Les croyances font obstacle à une bonne prise en charge. La qualité de vie des enfants et des parents est directement affectée par cette maladie. Les parents semblent satisfaits de recevoir une fiche informative sur la maladie.

The study, which combines qualitative and quantitative analysis, is based on a corpus of spontaneous spoken French, comprising 155,000 words, 95 interviews and subjects ranging in age from 7 to 88 years. The sample contains speakers from a broader range of educational backgrounds than is often the case: a butcher, a video-salesman and a toiletteur canin rub shoulders with counter assistants, teachers and doctors.

The social sciences have been accumulating knowledge since the early fifties as to how we are regulated by others. S. Milgram, S. Ashe, P. Zimbardo and J. Calhoun, detail the vulnerability to being duped and deceived and the difficulty of cooperating when values differ. Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
