My conclu sions met with the approval of two eminent surgeons, themselves Professors Of Surgical Anatomy in the College Of Physicians and Surgeons and the Bellevue Hospital College, and within the last year several younger American surgeons have with creditable skill ligatured the branches of the external carotid, rather than tie the common trunk.l These few instances, with those gathered in these pages, will, I hope, inaugurate a new era in the operative surgery Of the neck, and will furthermore encourage younger men not to accept any procedure, no matter how high the authority which eu dorses it, unless it stands the test of a critical analysis. In gleaning from the almost boundless field Of professional litera ture the Surgical Histories of the Great Vessels of the Neck, it has been my aim to arrive at the truth, positive and indisputable. I have accepted nothing that was not satisfactorily stated, not wish ing to swell the manuscript with irrelevant matter. TO this end I have omitted several hundred cases of deligation Of these vessels, heretofore published those of the subclavian2 artery when from the description Of the Operation I was satisfied that the axillary was the vessel tied; those of the other vessels when the results were not given, or the operation couched in uncertain terms. In 1830, having received his doctorate in medicine three years prior, Bourgery began work on his magnificent atlas in cooperation with illustrator Nicolas Henri Jacob (1782-1871), a student of the French painter Jacques Louis David. The first volumes were published the following year, but completion of the treatise required nearly two decades of dedication; Bourgery lived just long enough to finish his labor of love, but the last of the treatise's eight volumes was not published in its entirety until five years after his death. The four parts of Bourgery's treatise cover descriptive anatomy, surgical anatomy and techniques (exploring in detail nearly all the major operations that were performed during the first half of the 19th century), general anatomy and embryology, and microscopic anatomy. Jacob's spectacular hand-colored, life-size lithographs are remarkable for their clarity, color, and aesthetic appeal, reflecting a combination of direct laboratory observation and illustrative research; the images are to this day unsurpassed in anatomical illustration. Book excerpt: The decision of Harvey Cushing to leave general surgery and concentrate on the infant field of central nervous system surgery was in retrospect a landmark in the history of neurosurgery. His concentrated work, and also that of his colleague Walter Dandy, originated with the desires of both pioneers to understand surgical anatomy and neurophysiology. The fundamental knowledge and surgical techni ques that they provided became the standard of excellence for several generations of neurosurgeons; so much so that the general belief was that the surgical techniques could not be improved upon. Twenty-five to thirty years ago microtechniques began to appear in a few surgical research centers, they were then gradually applied to clinical neurosurgery and have contributed to a new level of understanding in surgical anatomy and neurophysiology. We are now fortunate to have a new standard of morbidity and mortality in the surgical treatment of intrathecal aneurysms, angiomas, and tumors. It has been said that microneurosurgery was reaching its limits, especially when treating lesions in and around the cavernous sinus and skull base; those lesions notorious for involvement of the dural and extradural compartments, with a tendency to infiltrate adjacent nerves and blood vessels. The dangers of uncontrollable hemorrhage from the basal sinuses and post-operative CSF rhinorrhea appeared unsurmountable. The lateral aspects of the petro-clival region have been of interest to a few pioneering ENT surgeons and neurosurgeons but the cavernous sinus in most respects has remained the final unconquered summit.
