However, in the 1990s, reactive oxygen species have been recognized to be involved in reversible structural alterations of molecules: The homeostatic modulation of oxidant levels is a highly efficient mechanism that allows all cells to tightly control their redox status within a very narrow range. Especially the activity of the Rel/NF-kB family of transcriptional factors that regulate responses in inflammation, reperfusion injury and apoptosis is controlled by intracellular reactive oxygen species levels. Our understanding of how these transcriptional factors are negatively or positively regulated by oxidative stress has since greatly increased. Questions for future investigation center on the modulation of oxidative stress by personal genome information for the prevention of several pathologic states.

Book excerpt: Oxidative stress, which is caused by endogenous and exogenous reactive-oxygen-species (ROS) formation, may significantly affect health-span and longevity. Endogenous oxidative stress could be attenuated in two ways: by the reduction of ROS formation and by quenching ROS with antioxidants. Numerous results of clinical trials in which individuals received one or more synthetic antioxidants failed to demonstrate conclusive benefits of antioxidant supplementation. Even oral supplementation with endogenous antioxidant enzymes cannot alter the antioxidant balance due to their degradation in the digestive system. Likewise, life-span or health-span is not increased significantly in genetically modified models overexpressing antioxidant enzymes. An alternative approach to attenuate the ROS-induced stress/damage may be through triggering an adaptive stress response in order to increase the endogenous antioxidant and damage repair processes. Moderate stress induced by CR, physical activity or mimetic compounds may induce such activation of endogenous antioxidative defence and cellular repair processes. These processes may increase cellular resistance to subsequent more severe stress and do not seem to interfere with ROS-dependent cellular signalling.

Med. 16,315.] Awareness of the digestive system began with the dawn of civilization, when man, observing the feeding habits of animals in the surrounding environment, experimented with foods, edible and inedible. Identity came with discoveries of the digestive organs during the 16th and 17th centuries. Function was revealed by physiologic studies of digestion, absorp tion and secretion, metabolism, and motility during the 18th and 19th centuries. Diagnostic access improved with the technological advances of the 20th century. Understanding of gas trointestinal (GI) disease followed the growth of the basic sciences and gastroenterology's involvement in scientific research during the latter half of the 20th century. Early in the 20th century, gastroenterology was yet an undefined activity without clinical or scientific guidelines. Diagnostic approach to the digestive tract was minimal. Valid con cepts of disease were lacking. Visceroptosis, sitophobia, and "colonic autointoxication" were common "diagnoses." Therapeutic resources were scarce.

Book excerpt: One of the major biomedical triumphs of the post-World War II era was the defmitive demonstration that hypercholesterolemia is a key causative factor in atherosclerosis; that hypercholesterolemia can be effectively treated; and that treatment significantly reduces not only coronary disease mortality but also all cause mortality. Treatment to lower plasma levels of cholesterol - primarily low density lipoprotein (LDL) cholesterol - is now accepted as best medical practice and both physicians and patients are being educated to take aggressive measures to lower LDL. We can confidently look forward to important decreases in the toll of coronary artery disease over the coming decades. However, there is still uncertainty as to the exact mechanisms by which elevated plasma cholesterol and LDL levels initiate and favor the progression of lesions. There is general consensus that one of the earliest responses to hypercholesterolemia is the adhesion of monocytes to aortic endothelial cells followed by their penetration into the subendothelial space, where they differentiate into macrophages. These cells, and also medial smooth muscle cells that have migrated into the subendothelial space, then become loaded with mUltiple, large droplets of cholesterol esters . . . the hallmark of the earliest visible atherosclerotic lesion, the so-called fatty streak. This lesion is the precursor of the more advanced lesions, both in animal models and in humans. Thus the centrality of hypercholesterolemia cannot be overstated. Still, the atherogenic process is complex and evolves over a long period of time.

Like the brain, the ENS is composed of neurons that are surrounded by glial cells. Enteric glia are a unique type of peripheral glia that are similar to astrocytes of the CNS. Yet enteric glial cells also differ from astrocytes in many important ways. The roles of enteric glial cell populations in the gut are beginning to come to light and recent evidence implicates enteric glia in almost every aspect of gastrointestinal physiology and pathophysiology.
