Book excerpt: Introduction : la transition entre l'hôpital et le domicile est une période clé en termes de continuité des soins. Avec 2 ordonnances sur 3 modifiées au décours de l'hospitalisation, le risque d'erreurs médicamenteuses est particulièrement important chez les personnes âgées, d'autant que cette population est à risque accru d'effets indésirables. Nous avons donc cherché à recueillir l'avis des patients face à ces modifications, les difficultés éventuelles en découlant et à évaluer leur niveau de satisfaction quant à leur ordonnance de sortie. Matériels et Méthodes : une étude qualitative par entretiens individuels semi-directifs a été menée d'aout 2016 à janvier 2017 auprès de sujets âgés de plus de 75 ans sans démence connue. Ils ont été recrutés auprès de médecins généralistes et hospitaliers de Maine et Loire dans le mois suivant leur hospitalisation et avaient un minimum de 4 médicaments sur leur ordonnance. Résultats : 12 patients ont été recrutés. 8 types de modifications ont été recensés avec en moyenne 6,3 modifications par patient entre ordonnance d'entrée et de sortie. Ce nombre important de modifications n'est pas sans susciter diverses réactions chez les personnes âgées qui s'interrogent sur la pertinence de ces divergences. Parmi les modifications, les ajouts et substitutions de classe sont bien acceptés mais induisent une perte de connaissance des médicaments prescrits. Les modifications de prise ne posent pas de véritables difficultés. Les modifications de doses peuvent pour leurs part passer inaperçues si elles ne sont pas notifiées. Les arrêts eux sont plus au moins bien vécus en fonction de la représentation que se fait la personne âgée de ce médicament. Enfin, les omissions et suspensions sont source de confusion, et les modifications de nom source de mécontentements. Certaines de ces discordances sont pourtant évitables et impactent directement sur la satisfaction et donc l'adhésion de la personne âgée vis-à-vis de son ordonnance de sortie. Conclusion : un travail de comparaison entre ordonnance d'entrée et de sortie semble donc indispensable afin de limiter les erreurs, ainsi que l'explication et la notification de chacune de ces modifications en fin d'hospitalisation.

Book excerpt: Afin d'évaluer l'impact d'une hospitalisation dans sur les prescriptions renouvelées au long cours, qui est peu analysé dans la littérature, nous avons procédé à une étude comparée des ordonnances des patients sortant hospitalisation entre le 1er mars et le 30 avril 2012. Pour ces 84 patients, l'ordonnance d'entrée était comparée à celle de sortie, puis celles-ci étaient comparées à l'ordonnance délivrée par le médecin traitant 2 mois après la sortie d'hospitalisation. Nos résultats montrent que les traitements prescrits lors de l'entrée ont été le plus souvent conservés (17.7% de traitements retirés et 7% de changements). Deux mois après la sortie, les retraits étaient maintenus à 77% et les changements à 89%. Lorsqu'un médicament était ajouté pendant l'hospitalisation, il était maintenu dans 91% des cas deux mois après la sortie. Les modifications apportées à l'ordonnance qui étaient explicitées dans le compte-rendu d'hospitalisation étaient maintenues dans 100% des cas à deux mois. Cette étude montre une continuité satisfaisante des prescriptions à l'interface ville/hôpital, probablement encore améliorée par la communication via le compte-rendu d'hospitalisation.

La proportion d'ordonnances non conformes à la législation est significativement importante. Ainsi 27% des prescripteurs oublient de préciser leur nom et 57% leur qualité, 9% oublient de noter le nom du patient. En ce qui concerne le médicament, le nom n'est pas identifiable dans 1,8% des ordonnances, la forme galénique n'est pas précisée dans 29,2% des cas, le dosage dans 51,7% des cas, la posologie dans 18,2% des cas, le mode d'emploi dans 36,5% des cas, la quantité requise dans 27,7% des cas, la durée du traitement dans 7,3% des cas et le renouvellement est non ou mal formulé dans 35% des ordonnances.

Les séjours hospitaliers sont responsables de changements de la liste des médicaments pouvant être sources de iatrogénie. Une étude rétrospective sur dossiers de patients hospitalisés de janvier à février 2013 a été réalisée au Centre Hospitalier de Dinan. L'objectif est de comparer les traitements médicamenteux pris avant l'hospitalisation avec ceux prescrits sur l'ordonnance de sortie et d'identifier les modifications appropriées ou non. Sur 31 patients inclus, 87% sont concernés par au moins une modification appropriée et 93,5% par au moins une modification inappropriée de leur traitement. Les modifications inappropriées se repartissent en 71% de substitutions thérapeutiques, 19% d'erreurs d'anamnèse et 10% d'oublis. Plusieurs axes notamment la réalisation de la conciliation médicamenteuse sont discutés afin d'améliorer la continuité de la prise en charge médicamenteuse des patients.

Book excerpt: Dans la prise en charge des personnes âgées, le Bon Usage du Médicament est un objectif primordial à cause des modifications physiologiques liées à l'âge, de la polypathologie, et de la polymédication. De nombreux outils ont été développés afin d'améliorer la qualité des prescriptions, de limiter les médicaments potentiellement inappropriés (MPI). Les praticiens du service de gériatrie de Lannemezan réévaluent leurs ordonnances de sortie via la grille DICTIAS. Cette étude quantifie les modifications des ordonnances de 80 patients jusqu'à 6 mois après le retour à domicile. Il en ressort que le nombre de traitements recensé est stable vers 5,2 ±2,7 médicaments par patient. Quarante pourcent ±1% en consomment 4 ou moins, et 6-7% en prennent 10 ou plus. Les modifications apportées concernent à 49% les médicaments du système nerveux. Le taux de MPI est stable à 17%. Seules les ordonnances à charge atropinique de 1 ou 2 subissent une augmentation statistiquement significative de 55 à 70% entre le retour à domicile et le 3e mois. Ces données sont cohérentes avec la littérature et tendent à prouver que l'amélioration de la communication ville-hôpital porte des fruits. Les médicaments phares de la iatrogénie, les benzodiazépines, subissent un traitement satisfaisant. Les inhibiteurs de la pompe à protons sont insuffisamment déprescrits à l'hôpital et leur consommation n'augmente pas en ambulatoire. Connaître les raisons de la prescription des médicaments atropiniques nous permettrait de comprendre la cause de leur augmentation en médecine générale et d'oeuvrer conjointement pour les faire diminuer.

De nombreux outils ont ainsi été élaborés pour guider les prescripteurs dans cette démarche d'amélioration des prescriptions. Cette étude, réalisée dans les unités de soins avec présence pharmaceutique de notre établissement, a permis de comparer les ordonnances d'entrée et de sortie des patients hospitalisés de plus de 75 ans selon la nouvelle version des critères STOPP and START. Sur les 166 patients inclus, la prévalence des médicaments potentiellement inappropriés (MPI) était de 56% à l'admission. Au cours de l'hospitalisation et malgré une augmentation du nombre moyen de médicaments par patient de 18,6%, la prévalence des MPI diminuait à 37% à la sortie. Plus de la moitié des MPI retrouvés à l'admission ont été arrêtés durant le séjour hospitalier (56%). Les médicaments à risque les plus détectés au cours de l'étude sont les diurétiques de l'anse, les benzodiazépines et les opiacés. La communication entre tous les acteurs de santé est primordiale pour favoriser le maintien ou la réévaluation de ces traitements introduits durant le séjour dans un contexte parfois particulier. Une collaboration multidisciplinaire impliquant le pharmacien clinicien permet un suivi précis des modifications de traitement : de la conciliation d'entrée à l'analyse d'ordonnances et jusqu'à l'élaboration d'un bilan médicamenteux de sortie, transmis au médecin traitant. Le développement d'un réflexe gériatrique quelle que soit l'unité de soins, doit être poursuivi, notamment grâce à la mise à jour et à la diffusion d'un guide de prescription institutionnel chez le sujet âgé.
