High turn-over rates, increased medical mistakes, and decreased patient satisfaction are a few outcomes of this preventable and treatable psychological phenomenon. Research indicates organizations provide accessible on site resources from 0 to 60% of the time to support those who experience signs and symptoms of CF. Organizations can improve support to nursing staff by implementing a formal interventional educational program led by a professionally trained psychologist to encourage, empower, and support oncology nurses who suffer from this often overlooked issue. This program is intended to quantify and qualify the results of this program to understand and increase the number of organizations who promote compassion satisfaction among their nursing staff. The Professional Quality of Life Survey (ProQol) will be utilized. This program will also be descriptive in nature in order to specify pertinent thoughts, feelings or concerns regarding experiences of oncology nurses. This program has the opportunity to increase revenue and reduce turnover rates.

Compassion fatigue is the terminology that best describes this phenomenon. This type of emotional exhaustion can lead to rapid pulse, high blood pressure, decreased immune response, insomnia, lack of empathy, and depersonalization of the patients they care about (Boyle, 2011). Compassion fatigue affects the nurse's professional capability therefore creating a negative impact on patient care and overall satisfaction. Several professions including the military workforce and palliative care providers have developed programs to teach compassion fatigue training (Melvin, 2012). This important educational offering teaches professionals how to care for themselves in times of stress. It allows them the opportunity to understand that compassion fatigue is real and if not managed properly, it can lead to poor physical health and declining overall job performance. This paper will provide the rationale for the necessity of compassion fatigue training to the oncology workforce. This author is proposing a plan for a pilot program that can begin on the pediatric oncology unit of the institution. The pilot program will take part in a four hour workshop that includes the teaching of self-care techniques vital to minimizing the symptoms and prevention of compassion fatigue. With a thorough dissemination plan to the staff, managers, educators and clinical supervisors; their support and buy-in will encourage staff to participate in this course. Evaluating the program after implementation will require collection of discharge summaries, participant post-seminar questionnaires, and monitoring feedback of the rounding tools used with families during their inpatient stay. This author has a vision that this program could be utilized hospital wide. The hope is that it will contribute to staff satisfaction and retention which will lead to improved patient care and increased patient and family satisfaction.

Close relationships are formed with patients and their families as a result of the compassionate care given. This paves the way for a phenomenon known as compassion fatigue. In 1992, Carol Joinson described nurses who shut down their own feelings as a way to deal with the stress of caring for patients undergoing traumatic experiences and recognized nurses at risk for compassion fatigue. When a nurse's capacity to give compassionate care is compromised, the quality of care delivered could potentially be affected. Oncology nurses are recognized as an at-risk group for experiencing compassion fatigue. No existing educational materials on this topic were identified that targeted on oncology nurses. Therefore, an educational resource was created specifically for oncology nurses. Method: A comprehensive resource booklet was developed to promote awareness of compassion fatigue and offer self-care strategies aiming at prevention and interventions to decrease its effects. Stamm's (2012) Theoretical Model of Compassion Satisfaction and Compassion Fatigue was used as a framework in the development of the booklet. A team of five experts reviewed the booklet through a series of three evaluations using the Delphi Method. Results: Final topics deemed appropriate for an oncology setting included an overview of compassion fatigue, impact on patient care, prevention and self-care, impact on work environment, and leadership and organization responsibility. Conclusion: The development of an educational resource for oncology nurses is a vital step to bringing awareness about compassion fatigue. Future goals include distributing the resource to oncology nurses to evaluate its effectiveness on preventing and decreasing compassion fatigue. Education on compassion fatigue needs to be a priority so oncology nurses can continue to give compassionate care.

Book excerpt: Aim: The aim of this project was to plan, develop, and implement "The Watson Room" or "Quiet Room" in an inpatient oncology nursing population and evaluate changes in Professional Quality of Life (ProQOL). Background: Oncology Nurses are at high risk for compassion fatigue (CF), burn-out (BO), and secondary traumatic stress (STS) related to the effects of living the traumas of oncology patients and their families through their cancer journey and the innate stress in a complex and intense workplace. High levels of compassion fatigue, burn-out, and secondary traumatic stress that are poorly managed reduces the nurse's ability to self-regulate their empathy and compassion leading to negative sequela for organizations, patients, and nurses. Preventative and restorative measures accessible in the workplace will aide nurses to manage and remediate the negative aspects of compassion fatigue experienced through the care of oncology patients and their families. Methods: Utilizing the Precede-Proceed Model, relief of compassion fatigue, burnout, secondary traumatic stress, and compassion satisfaction was measured before and after utilizing the "The Watson Room" in a pre- post-test design. Outcomes were measured with the Professional Quality of Life version V Survey (ProQOL-V). Results: The pre-surveys indicated below average scores in CS (37.20; avg score 50), BO (22.60; avg 50), and STS (25.65; avg 50) and post-survey results were not appreciably different (CS 35.82, BO 23.65, STS 24.82). Of the interventions it was found that Modeling Clay and the Buddha Board had the most significant number of participants and were found to be moderately or extremely effective. Implications: Although the results did not demonstrate an appreciable difference, it did show an interesting finding. The interventions used in "The Watson Room" showed that when modalities were easily accessible, they were utilized. Conclusion: "The Watson Room" is a concept first conceived as a "Quiet Room". C. Crewe (2016) renamed the room as the "The Watson Room" in honor of nursing theorist, Jean Watson. Her intention was to connect the use of the room to the importance of self-care.

Book excerpt: This research explores the phenomenon of compassion fatigue as experienced by oncology nurses working in an outpatient unit in a mid west city of The United States. The researcher sought to understand how compassion fatigue was affecting the lives of the nurses and how self care approaches using art therapy techniques could be taught to relieve self-reported symptoms. It was hypothesized that over 50% of the particpating nurses would report experiencing compassion fatigue. The hypothesis was confirmed by the study identifying 78% of participants felt they experienced symtoms. the research process included a standard assessment measuring compassion fatigue experience for each participant, a group therapy session for developing self awareness and psychoeducation on compassion fatigue, and a visual journaling assignment for implementing self care art therapy techniques. Participants evaluated their experience in a survey collected three weeks after the group session. Results displayed that visual journaling proved effective for combating perceived symptomes of compassion fatigue. A thematic content analysis of the qualitative and quantitative data revealed themes of the nurses' negative experience of compassion fatigue as well as successful themes of art therapy techniques for relief. This study calls for further research to be completed with a larger sample study size to explore the effects and development of compassion fatigue in oncology nurses as well as interventions to prevent onset. A call for health care organizations to develop and implement effective programs to prevent decreased productivity and devastating clinical outcomes is identitified.

The idea of caring for terminally ill patients day in and day out can bring mental, physical, and emotional symptoms that jeopardize the healthcare staff member to patient relationship and ultimately compromise patient outcomes. There is a gap in healthcare settings throughout our nation where healthcare providers and staff members are not properly equipped with the self-care tools and the knowledge and awareness of how to cope with compassion fatigue. The Joint Commission has identified compassion fatigue and burnout as a current, serious problem within the healthcare community and has found that it is directly affecting patient outcomes and the personal wellbeing of healthcare staff members. Currently, at the pediatric oncology outpatient facility in which this project occurred, there is no existing intervention to decrease the prevalence of compassion fatigue, burnout, and secondary traumatic stress or increase of the prevalence of compassion satisfaction. There is no method in place to assist the staff member of this facility in coping with the high demand of daily patient caring. A need exists to aid these healthcare staff members in increasing their compassion satisfaction and ultimately improve outcomes through improved healthcare staff and patient relationships. This project reinforces the need for an intervention to assist healthcare staff members in decreasing compassion fatigue and increasing compassion satisfaction in the pediatric oncology clinical setting.

Book excerpt: The purpose of this project was to explore Compassion Fatigue (CF), the associated signs and symptoms, and how to combat it through interventions, programs, and activities. Healthcare workers, especially those working in high acuity areas are at high risk for developing Compassion Fatigue. The focus has moved from accepting burnout as a definite to researching and implementing interventions to avoid it. Little has been published about Advanced Practice Providers (APPs), specifically Nurse Practitioners and Physician Assistants, especially in the Emergency Department (ED) setting. The research needed to provide specifics on the degree in which it affects personal and professional well-being for this group remains to be studied. Interactions with patients, patient outcomes, and the quality of professional life, specifically job satisfaction, are plentiful about nurses, but not APPs. This project looked to expand on these terms in relation to APP's. The ProQOL scale was administered after APPs were given modules at baseline, two months, and five months and scale scores were calculated. The three modules included definitions of key concepts, interventions to combat Compassion Fatigue, and ways to increase awareness in one self and others. Each module built on the foundation of the prior content. The thought was that after providing incremental modules, APPs would have lower levels of Compassion Fatigue on the ProQOL scale due to integration of the suggestions into daily practice. Professional quality of life is the balance between Compassion Fatigue and Compassion Satisfaction; it is finding the balance to continue to provide good care to patients. Findings of the study showed a weak correlation between increased hours worked per week and a higher score within the subset of burnout within CF category of the ProQOL scale. This was addressed with the scheduler to limit hours worked per week.
