Previous studies have shown that women in the US workforce value health insurance more highly than men do. Because prior to the ACA self-employed individuals did not have guaranteed access to affordable health insurance coverage, women's relatively lower rate of self-employment may partly have reflected their greater 'job lock' due to employer-based health insurance. This paper employs nationally-representative survey data for 2009-2018 and a quasi-experimental difference-in-difference modeling approach and finds that unmarried women's probability of self-employment increased by 1.2 percentage points in 2015-2018, after the ACA's expansion of non-group health insurance came into effect. Among women who have never married, overall probability of self-employment increased by 1.2-1.5 percentage points versus trend, and the probability of transitioning into full-time self-employment increased by 0.9 percentage points.

Book excerpt: There has been substantial public policy concern over the relatively low rates of health insurance coverage among the self-employed in the United States. We use data from the Medical Expenditure Panel Survey conducted in 1996 to analyze how the self-employed and wage-earners differ both with respect to insurance coverage and utilization of a variety of health care services. Our results suggest that for the self-employed, the link between insurance and utilization of health care services is not as strong as assumed in the policy debate. For a number of medical care services, the self-employed have the same rates of utilization as wage-earners, despite the fact that they are substantially less likely to be insured. And when the self-employed are less likely than wage-earners to utilize a particular medical service, the differences generally do not seem very large. The self-employed thus appear to be able to finance access to health care from sources other than insurance. Further, analysis of out-of-pocket expenditures on health care suggests that doing so does not lead to substantial reductions in their ability to consume other goods and services. Finally, there is no evidence that children of the self-employed have less access to health care than the children of wage-earners. Hence, the public policy concerns that the relative lack of health insurance among the self-employed substantially reduces utilization of health care services or creates economic hardship appear to be misplaced.

The Affordable Care Act (ACA) improved and expanded the availability of non-employer-based health insurance, with protections for pre-existing conditions, guaranteed issue, and community rating in non-group markets. This paper uses nationally-representative survey data for 2009-2018 and a difference-in-difference modeling approach to examine the effects of the ACA's health insurance expansion on U.S. adults' probability of self-employment. Results indicate that the probability of self-employment increased by 1.5 percentage points among adults ages 30 to 64 who have one or more pre-ACA declinable medical conditions and no alternative source of health insurance through a spouse's employer. Transitions into self-employment increased by 1.3 percentage points, and the probability of full-time self-employment increased by 1.1 percentage points. Evidence indicates that the ACA's provisions effective in 2014 reduced entrepreneurship lock in 2015 and 2016 among adults with higher demand for health insurance. However, these effects were short-lived. As political uncertainty about the long-term viability of the exchanges increased in 2017 and 2018, the probability of self-employment among individuals with high demand for insurance fell back to pre-ACA levels.
