Book excerpt: The project that is being proposed aims to show through evidence-based research that bedside shift reporting is more effective in increasing patient satisfaction, patient safety, and nurse accountability compared to the traditional method of reporting at the nursing station. Bedside shift reporting promotes enhanced relationships between nurses and patients which creates an increase in communication. Verbal reports offer minimal communication between nurse and patient. Patients are often not involved in their plan of care of daily shift goals with traditional reporting methods. Patient safety, patient satisfaction and nurse accountability are compromised when the off going nurse is given report at the nursing station. Many patients often suffer for no reason because ineffective communication and patient safety is overlooked during a traditional shift report during the shift change. Financially, hospitals often feel the losses when major events occur to patients. A pilot study at Mount Eagle Community Hospital is being proposed on the 3 South unit, which is a medical-surgical unit. A bedside shift reporting pilot will be implemented using Lewin's Change Theory as the foundation. A hand tool will be created using evidence-based research on bedside shift reporting will be created to implement bedside shift report so that an effective report can be given to allow patients to become more proactive in their health care management. Bedside shift reporting increases the communication level between the patient and nurse; thus increasing patient safety and patient satisfaction. Nurses are more accountable to the patient at the bedside.

By providing adequate knowledge and involving the patients during change-of-shift reporting is essential in promoting patient/family and nursing satisfaction including patient safety. In addition, involving the patients in decisions regarding their plan of care will assist to promote quality care. This article focused on the significant role end of shift bedside nurse report in increasing patient and family gratification, promoting patient safety while admitted in acute care settings, and standardized tools used to establish the change such as the implementation plan, evaluation, dissemination strategies, and review of literature from previous studies. Though the bedside nurses handover during acute care hospitalization lead to increase patient and family gratification, it also reduced the length of hospital stay and healthcare costs and nursing satisfaction without nurses' burnout/turnover.

Book excerpt: Research has been performed to show that bedside nursing increases patient care by allowing better communication between nurses at change of shift. Bedside handoff is more beneficial for patients and nurses rather than the current practice happening in hospitals. Through a literature review of articles it has been determined that there is no standard for nursing shift handoffs. The Joint Commission has made a patient safety goal and proposed that there be a standardized handoff communication in hospitals. Performing shift change at the bedside with a standard template to be used at the change can be the new standard and will increase patient care and help nurses feel more empowered with the communication received and given. The results showed that the information being reported previously to bedside shift change was often poor, and that much of the information was easily accessed in the chart and did not need to be part of report. They also found that the inefficiencies in communication is a factor associated with sentinel events. Making the switch to bedside shift reporting and making it the standard in hospitals around the world can make a safer, smarter tomorrow. This project will outline the problem of the current shift change process, how to implement the change, the theory that goes into the change, how to evaluate the implementation, and how dissemination can be done. There is a literature review, an appendix and a complete reference list that are all included.

It is a time to review the plan of care, reconcile medications and infusions and answer questions. Traditionally, this has occurred in a central area such as a conference room or the nurses' station, away from the patient and family. Moving shift report to the bedside has been proven to improve communication between caregivers and the patient. Critical information can be exchanged and the bedside location allows for direct visualization of the patient along with verification of infusions, equipment settings and patient acuity. When this communication occurs at the bedside, patients and families are brought into the conversation and become active participants in the plan of care. Research has shown that conducting shift report at the bedside can significantly improve both patient and nurse satisfaction, leading to a more patient centered care approach, which in turn, may translate into better patient outcomes (Anderson and Mangiano, 2006). Bedside shift report (BSR), is a nursing intervention that may also assist with the Joint Commission directive to' involve both the patient and his or her family to develop a treatment plan that is tailored to the patient's unique needs. Staff should encourage the patient and family to ask questions throughout the course of care and provide opportunities for them to participate in care discussions' (The Joint Commission, 2010 pg. 18). To examine the effect of implementing BSR, a pilot study will be conducted in a 25-bed unit within an acute care hospital in the northeast United States. Presently, handoff report is conducted at the nurses' station; it is not uncommon for there to be questions later when the nurse actually visualizes the patient and the previous caregiver is gone. The process can also leave the patient unattended for up to an hour while report is taking place. This may result in delay of care or potentially, missed opportunities to avert crises. Based on the results of this pilot study, a decision will be made regarding hospital wide implementation.

A review of literature of both staff and patient's perceptions of bedside hand off describes studies using qualitative, anecdotal and/or indirect measurement along with quantitative and direct measurement. Statistical implications show that bedshand offdoff is most effective when completed consistently.

Ineffective communication during shift-to-shift reports can contribute to gaps in patient care and breaches in patient safety, including medication errors, falls, and sentinel events. The greatest risk of communication breakdown is during transitions in care. CONTEXT: St. Louise Regional Hospital, a small community hospital, consisting of an eight-bed medical-surgical intensive care unit lacked structure in how handoff should occur and had variances in shift handoffs. Observation of the shift handoff at the nurses' station revealed many communication gaps that have shown negative impacts on patient safety and outcomes. The improvement project described in this paper focused on evidence-based practices that support the benefits of bedside shift and implementations of standardized handoff tools. INTERVENTION: Intervention included an education in-service for unit managers, educational coordinators, and staff nurses to emphasize the important benefits of bedside shift report. MEASURES: The three components for evaluating improvement include outcome, process, and balancing measures to determine whether the improvement project has had the desired impact. The outcome measure expected to yield the following: direct observation of nurses during bedside report to calculate how often is being done, monitor for decrease in overtime due to more efficient shift-to-shift bedside reports, and a review of risk management reports observing for effect on the number and severity of medication errors. RESULTS: The outcome measure is to increase the nurses' compliance with bedside shift report in the intensive care unit to at least 80% within six months of implementation. CONCLUSION: With the proposed change, bedside shift report will addresses all the safety hazards by reducing adverse events, such as medical errors, patient falls at shift change, and sentinel events. Bedside shift report improves patient safety, enhances the quality of care, improves patient and nurse satisfaction, decrease unnecessary healthcare expenditure, and saves time.

Furthermore, regulatory and institutional obstacles-including limits on nurses' scope of practice-should be removed so that the health system can reap the full benefit of nurses' training, skills, and knowledge in patient care. This paper will describe the problem, solution to the process of the issue, implementation plan, evaluation, and dissemination plan. It will also describe studies before and after the implementation with nurse and patient's perception of the process. Results from the review of studies shown reveal how other clinical settings have benefited from bedside shift report, improving patient's safety.

This project will provide a description of the problem, proposed solution, and a detailed implementation, evaluation, and dissemination plan. Evidence suggests that bedside reporting is more patientcentered and comprehensive ensuring that quality care is delivered from shift to shift. Furthermore, this reporting method improves the patient's experience of care, promotes nurse and patient engagement, increases time spent providing direct care, improves communication, and enhances nursing practice. Furthermore, research shows that bedside shift report allows the oncoming nurse to visualize the patient, perform a quick assessment, answer patient questions, confirms the plan of care, and establishes patient goals for the shift. This new method of reporting will improve nursing practice, outcomes, and patient satisfaction.

The purpose of this study is to show bedside shift report increases patient safety as well as staff satisfaction. This study found bedside report lead to less falls and medication errors. Communication breakdowns are the cause of a significant number of sentinel events. Staff was satisfied with shorter report times and improved communication between shifts. Nurses were better prepared and were able to better assess patient needs after implementation of bedside shift report. Nurses were held more accountable and patients became more involved in their care. This new process gave the oncoming nurse the opportunity to visualize the patient and ask questions. Finding of this report confirmed how important having a standardized report system is in reducing errors. Accurate and consistent communication was improved after the implementation of bedside reporting. Effective communication at shift change is a fundamental component of nursing and the culture of safety. This study was conducted within a large hospital on medical and surgical units. A survey was obtained before the implementation of bedside report and several times over the first year post. Along with the use of surveys visual observations were done as well as in person interviews. Results of this study show that bedside report increased nurse's satisfaction, helped in the prioritization of patients which increased work flow, and decreased the amount of time for handoff.
