Both structurally and functionally, there are many peculiarities that differentiate the brain from all other organs. The brain is our connection to the world around us and by governing nervous system and higher function, any disturbance induces severe neurological and psychiatric disorders that can have a devastating effect on quality of life. Our understanding of the physiology and biochemistry of the brain has improved dramatically in the last two decades. In particular, the critical role of cations, including magnesium, has become evident, even if incompletely understood at a mechanistic level. The exact role and regulation of magnesium, in particular, remains elusive, largely because intracellular levels are so difficult to routinely quantify. Most PTSD authors agree that Abram Kardiner's "Traumatic Neuroses of War" is the seminal psychological work on PTSD. In this work Kardiner distilled much psychiatric thought on the traumatic syndrome resulting from World War II, with what he had termed "neurosis of war." The symptoms of this syndrome included features such as fixation on the trauma, constriction of personality functioning and atypical dream life. Kardiner provided powerful new insights in these classic texts on the phenomenology, nosology, and treatment of war-related stress, thereby anticipating virtually every aspect of contemporary research on PTSD. Although Kardiner had observed war neuroses since 1925, when he was attending specialist at the U.S. Veterans Hospital, he was only able to theorize them to his satisfaction after he had written "The Individual and His Society," which dealt with the problems of adaptation. He came to see that in the traumatic neurosis of the war the defensive maneuver to ward off the trauma sometimes destroyed the individual's adaptive capacity. Thus, the traumatic neurosis of war was the result of an adaptive failure, not a conflictual illness. So concluding, Kardiner re-introduced the concept of traumatic neurosis into psychoanalytic theory.

It is the draft of a lost metapsychological paper, one of twelve essays written during World War I at the peak of the master's powers. Freud intended to publish all twelve in book form, under the title Preliminaries to a Metapsychology, and thereby set out the theoretical foundations of psychoanalysis. Scholars have long lamented the disappearance without a trace of seven of these important essays. Only in 1983 did Ilse Grubrich-Simitis happen upon this draft, in Freud's handwriting, in an old trunk containing papers and documents of his Hungarian collaborator Sándor Ferenczi. With the help of a brief letter Freud had written on the back of the last page, she soon realized that the manuscript she had found was the draft of the final paper in the series. That draft is published here in facsimile, together with a transcription in German of the facsimile and the English translation. In the first part of the draft, which is written in a kind of shorthand, Freud contrasts the three classic transference neuroses: anxiety hysteria, conversion hysteria, and obsessional neurosis. In the second part, which is written in complete sentences, Freud undertakes a daringly speculative "phylogenetic fantasy" He explores whether the debilitating illnesses of the neurotic and the psychotic today might have originated long ago as adaptive responses of the entire species to threatening environmental changes or to traumatic events in the prehistory of mankind. In the draft "Fantasy" Freud modifies and expands the line of reasoning he began in Totem and Taboo (1912-13) after an intensely productive exchange with Ferenczi about Lamarckian concepts, making this recovered draft of major significance to students not only of psychoanalysis but also of the social sciences, humanities, and natural sciences. Ilse Grubrich-Simitis has contributed a detailed essay, setting the overview in the context of Freud's life, his work, and his historical and scientific prominence. She quotes from relevant letters of Freud and Ferenczi, some published here for the first time.

Book excerpt: Psychoanalysis began over a century ago as a treatment for neurosis. Rooted in the positivistic mindset of the medicine from which it stemmed, it trained its empiricist gaze directly upon the symptoms of the malaise, only to be seduced into attributing it to causes as numerous as there are aspects of human experience. Edifying as this was for our understanding of the life of the psyche, it left the sickness of the soul that was its actual subject matter, the neurosis which it was supposed to be about, out of its purview. The crux of this problem was of a conceptual nature. As psychology increasingly gave up on its constituting concept, its concept of soul, it succumbed to the same extent to treating its patients without an adequate concept of what both it and neurosis were about. Attention was paid to mishaps and traumas, the vicissitudes of development, and the Oedipus complex. But neurosis, according to the thesis of this ground-breaking book, comes from the soul, even is soul; the soul in its untruth. Indeed, both it and the modern field of psychology are successors of the soul-forms that preceded them, religion and metaphysics, with the difference that psychology's reluctance to recognize and take responsibility for its status as such has been matched by the neurotic soul's clinging to obsolete metaphysical categories even as the often quite ordinary life disappointments of its patients are inflated with absolute importance. The folie à deux has been on a massive scale. Owing their provenance to the supplement they each provide the other, psychology and neurosis are entwined in a Gordian knot, the cutting of which requires insight into the logic that pervades both. Taking up this sword, Giegerich exposes and critiques the metaphysics that neurosis indulges in even as he returns psychology to the soul, not, of course, to the soul as some no longer credible metaphysical hypostasis, but as the logically negative life of the mind and power of thought. Using several fairy tales as models for the logic of neurosis, he brilliantly analyses its enchanting background processes, exposing thereby, in a most lively and thoroughgoing manner, the spiteful cunning by which the neurotic soul, against its already existing better judgement, betrays its own truth. Topics include the historicity of neurosis, its soulful purpose as a general cultural phenomenon, its internal logic, functioning, and enabling conditions, as well as the Sacred Festival drama character of symptomatic suffering, the theology of neurosis, and ‘the neurotic’ as the figure of modernity's exemplary man. A collection of vignettes descriptive of various kinds of neurotic presentation routinely met with in the consulting room is also included in an appendix under the heading, ‘Neurotic Traps.’
