Book excerpt: Woodward Dam, on Little Shawangunk Kill, is 1 mile east of Mount Hope, New York. The dams and reservoir are used for water supply. The visual inspection of Greenleaf Dam revealed several deficiencies in this structure. The following item were noted: A wet area at the downstream toe of the dam near the buried outlet for the abandoned outlet works; Minor erosion (riprap displacement) at normal pool level on the upstream face of the dam; Trees and brush growing on the downstream face and toe of the dam. There are two trees 24 inches in diameter. A small depression on the downstream face of the embankment slightly to the right of the center of the dam and approximately half-way down the down stream face, and An animal burrow in the downstream toe of the dam approximately 50 feet from the right abutment.

Book excerpt: The Vly Creek Dam is composed of a 24.5 feet high and 338 feet long earth embankment dam with spillway, and a 39.5 feet high and 1720 feet long earth embankment dike with control tower for water supply regulation. Visual inspection did not reveal conditions that are considered to be unsafe. The total discharge capacity of the spillway is adequate to pass half the Probable Maximum Flood (PMF) regardless of the flashboards. The spillway is also capable of discharging PMF without flashboards, but not with flashboards.

Book excerpt: Visual inspection of this dam and engineering analyses which have been performed revealed that several serious deficiencies exist with this structure. Structural stability analyses, based on available information, indicate that this dam is marginally stable under normal loading conditions and is unstable when subjected to severe loading conditions (such as flood flows or ice loading). Seepage noted under and through the structure, creating wet, soft areas along the downstream toe along both ends of the dam, adversely affect the stability of the structure. Immediately upon receipt of this notification, a system for providing around-the-clock surveillance of the dam during periods of unusually heavy precipitation should be developed and implemented. An emergency action plan for the notification of downstream residents should also be developed. It is recommended that within 3 months of the data of notification of the owner, investigations into the deficiencies on this structure should be commenced. Studies of the structural stability and seepage problems, including subsurface and structural explorations are necessary. Information obtained should be incorporated into a detailed stability evaluation.

Book excerpt: Examination of available documents and visual inspection of the dam and appurtenant structures did not reveal any conditions which constitute an immediate hazard to human life or property. Using the Corps of engineers' screening criteria, it has been determined that the dam would be overtopped for all storms exceeding 33 percent of the Probable Maximum Flood (PMF). Therefore, the spillway is adjudged as 'seriously inadequate' and the dam is assessed as 'unsafe, non-emergency.' Structural stability analyses based on available information, indicate that factors of safety against overturning are generally low, and the locations of the resultants fall outside of the middle 1/3. The factor of safety against sliding was less than the recommended value of 3 for Cases 3 and 4. Therefore, when the dam is subjected to severe loading conditions such as a 1/2 PMF of PMF event, the factors of safety fall to below critical levels.

Book excerpt: This report provides information and analysis on the physical condition of the dam as of the report date. Information and analysis are based on visual inspection of the dam by the performing organization. The examination of documents and the visual inspection of the Goshen Reservoir did not reveal conditions which constitute an immediate hazard to human life or property. However, the dam has some problem areas which require additional studies and remedial work. The most serious of these deficiencies are the two areas of seepage on the downstream slope of the dam. First is at the end of a grouted stone spillway channel and the second is at the toe in the center of the dam. It is recommended that within six months from notification, an engineering investigation is initiated to determine the sources of the seepage found. Using the Corps of Engineers' screening criteria for the initial review of spillway adequacy, it has been determined that the embankment would be overtopped by all storms in excess of 51% of the Probable Maximum Flood (PMF). Therefore, the spillway is adjudged as 'inadequate.' (Author).

Book excerpt: The examination of documents and visual inspection of the dam and appurtenant structures did not reveal conditions which constitute an immediate hazard to human life or property. The dam, however, has a number of problem areas which should be investigated, evaluated, and remedied. The following remedial measures should be undertaken within one year: (1) Eliminate leakage into the interior core of the dam and repair deteriorated concrete on the north abutment and at the pier separating the two sections of the dam; (2) A formalized inspection program should be initiated to develop data on conditions and maintenance operations at the facility; and (3) A flood warning and emergency evacuation plan should be developed and implemented to alert the public in the event conditions occur which could result in failure of the dam.

Phase I Inspection Report written by George Koch and published by . This book was released on 1981 with total page 140 pages. Available in PDF, EPUB and Kindle. Book excerpt: The examination of documents and visual inspection of Basic Creek Dam and appurtenant structures did not reveal conditions which constitute a hazard to human life or property. The discharge capacity of the spillway is inadequate for all storms in excess of 52% of the PMF (Probable Maximum Flood). During the 1/2 PMF event the water surface will approximate the top of dam elevation and the outflow will be 6801 cfs. The spillway is, therefore, assessed as 'inadequate'. The following problem areas were observed which require remedial action within 1 year of notification to the owner: Repair the areas of deteriorated concrete which are leaking (approx. 50 gpm) between the 24 inch and 48 inch valves within the intake chamber; Repair the deteriorated concrete and control the seepage within the diversion tunnel; Monitor the seepage within the intake chamber, particularly above the 12 inch valve, and repair as required; Repair the voids in the concrete spillway apron; Monitor the seepage from the horizontal joints of the spillway; Periodically monitor the concrete deterioration of the dam and appurtenances; Periodically remove the debris in the downstream channel; Remove the trees and brush on the slopes, crest and abutments of the embankments.

Book excerpt: The examination of documents and the visual inspection of Lake Mahopac Dam and its appurtenant structures did not reveal conditions which constitute an immediate hazard to human life and property. Using Corps of Engineers screening criteria, it has been determined that the dam would be overtopped for all storms exceeding approximately 56 percent of Probable Maximum Flood (PMF). The maximum spillway discharge capacity is 7.9 percent of the PMF peak outflow. The spillway is, therefore, adjudged as 'inadequate'. The following remedial measures should be performed within 12 months from notification: (1) Missing portions of the upstream stone masonry wall should be repaired. (2) Brush and shrubbery should be removed from the embankment, spillway-reservoir drain structure and downstream channel. Provide a program of periodic cutting and or mowing of the dam surfaces, spillway-reservoir drain structure and the downstream channel. (3) The collected debris at the spillway-reservoir drain structure and in the downstream channel should be removed and hauled away from the dam. (4) A program of periodic inspections and maintenance of the dam and appurtenances should be provided, including yearly operation and lubrication of the reservoir drain and its control facilities. This information should be documented for future reference. (5) An emergency action plan for the project should be developed.

Phase I Inspection Report written by George Koch and published by . This book was released on 1981 with total page 105 pages. Available in PDF, EPUB and Kindle. Book excerpt: This report provides information and analysis on the physical condition of the dam as of the report date. Information and analysis are based on visual inspection of the dam by the performing organization. The examination of documents and the visual inspection of Glenmere Lake Dam did not reveal conditions which constitute an immediate hazard to human life or property. However, the dam has some deficiencies which require further investigation and remedial action. Using the Corps of Engineers' 'screening Criteria' for the initial review of spillway adequacy, it has been determined that the embankment would be overtopped for all storms in excess of 17% of the Probable Maximum Flood (PMF). The spillway is, therefore, adjudged as 'seriously inadequate' and the dam is assessed as unsafe, non-emergency. The classification of 'unsafe' applied to a dam because of a 'seriously inadequate' spillway is not meant to connote the same degree of emergency as would be associated with an 'unsafe' classification applied for a structural deficiency. It does not mean that there appears to be a serious deficiency in spillway capacity; and if a severe storm were to occur, overtopping and failure of the dam could take place significantly increasing the hazard to loss of life downstream of the dam.
