Book excerpt: Base on a visual inspection, operational history, and available engineering data, the dam is considered to be in fair condition. The size classification of the facility is small and its hazard classification is considered to be high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for the facility is the Probable Maximum Flood (PMF). Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store about 62 percent of the PMF prior to embankment overtopping. Consequently, the spillway is assessed as being inadequate, but not seriously inadequate. Deficiencies noted by the inspection team included heavy overgrowth across the downstream toe, a questionable structural and foundation design associated with the spillway, and an inadequate emergency warning system.

Book excerpt: Gallo Dam is a 29-foot high earth embankment approximately 690 feet long (including spillway). The embankment is reportedly comprised of 3 sections which include an impervious core, a semi-pervious upstream section and a pervious downstream section. The facility is served by a chute channel spillway, with discharges controlled by a free overfall, concrete weir structure. The spillway is located near the right abutment, and has a crest length of 39 feet. In addition, the facility is equipped with a 18-inch diameter steel pipe encased in concrete used as a low level outlet as well as a 16-inch diameter steel siphon which was installed in 1973.

Book excerpt: The visual inspection, operational history, and hydrologic/hydraulic analysis indicate the facility is in good condition. It is recommended that the owner: (a) Immediately fill and seal all cracks within the interior of the outlet conduit. (b) Specifically address the seepage condition observed to the right of the spillway in all future inspections and note any turbidity or changes in the rate of flow. (c) Re-evaluate and revise the present standard flood emergency procedures in accordance with, but not limited to, the following items: (1) Include a definite procedure of notifying downstream residents of a possible emergency. (2) Provide for an alternate means of communication in the event telephone lines become inoperative. (3) Provide for around-the-clock surveillance of the facility during periods of unusually heavy precipitation. (d) Provide additional riprap protection along the right sidewall of the spillway approach channel.

Phase I Inspection Report written by Bernard M. Mihalcin and published by . This book was released on 1979 with total page 95 pages. Available in PDF, EPUB and Kindle. Book excerpt: Based on a visual inspection, review of post-construction engineering studies, and available engineering data, the Hinckston Run Dam is considered to be in good condition. Deficiencies were limited to general deterioration of the concrete spillway section, and apparent inoperability of the valves within the intake tower. The size classification of the facility is intermediate, and the hazard classification is considered to be high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for the facility is the Probable Maximum Flood (PMF). Results of the hydrologic and hydraulic analysis indicate the facility can pass and/or store about 38 percent of the PMF prior to overtopping of the embankment. Overtopping, even under floods of PMF magnitude, is not expected to cause failure of the unusually massive embankment configuration which is composed of erosion-resistant, hot-poured slag. Thus, the spillway system is considered to be inadequate, but not seriously inadequate.

Phase I Inspection Report written by Bernard M. Mihalcin and published by . This book was released on 1979 with total page 103 pages. Available in PDF, EPUB and Kindle. Book excerpt: Cherry Run Dam is a zoned earth embankment with a central concrete corewall. The embankment measures approximately 430 feet long (including spillway) and 22 feet high. The facility is provided with an ogee-shaped concrete spillway located about 100 feet from the right abutment. The spillway crest is 132 feet long. Based on a visual inspection, maintenance of Cherry Run Dam appears minimal to non-existent and the facility is considered to be in poor condition. Deficiencies noted by the inspection team included heavy overgrowth of the embankment sections (particularly to the right of the spillway), delamination of the spillway surface, cracking and misalignment of the spillway wingwalls, heavy overgrowth within the discharge channel, an inoperable outlet works, and no emergency warning systems in effect. Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store only 40 percent of the PMF prior to embankment overtopping. A Breach analysis indicates that failure under less than 1/2 PMF conditions could lead to increased downstream damage and potential for loss of life. Thus, based on screening criteria provided in the recommended guidelines, the spillway is considered to be seriously inadequate and the facility unsafe, non-emergency.

Book excerpt: Star Junction No. 1 dam consists of an earthfill embankment and masonry spillway on the right abutment. The dam has a crest length of 545 feet, a maximum height of 35 feet and a storage volume of 149.3 acre-feet at the spillway crest level. The dam is classified by Corps of Engineers guidelines to be a small size, high hazard structure. Based on the visual observations and the data available, the dam is categorized as being in an unsafe, non-emergency condition. The visual inspection revealed that the masonry spillway on the right abutment has deteriorated to the extent that its structural integrity during extended, large discharge conditions, is questionable.

Book excerpt: Based on a review of available design information, engineering analyses, and visual observations of conditions as they existed on the date of the field inspection, the general condition of the Colonial Dam No. 1 is considered to be fair. This assessment is based on visual observations and hydrologic calculations that indicated: (1) that the earth embankment is in good condition, (2) that the principal spillways are in poor physical condition and have 'inadequate' capacity but are not 'seriously inadequate', and (3) the existence of non-serious deficiencies which are considered correctable. The structure is classified as a 'small' size, 'high' hazard dam. Corps of Engineers guidelines recommend 1/2 to one times the Probable Maximum Flood (PMF) for 'small' size, 'high' hazard dam. Colonial No. 1 Dam's Spillway Design Flood is 1/2 the Probable Maximum Flood (PMF).

Book excerpt: Based on a visual inspection, operational history, and available engineering data, the dam is considered to be in fair condition. Structural deficiencies noted during the inspection include an inoperable drawdown mechanism, seepage and ponded conditions at the downstream toe, dense overgrowth (bushes, weeds, small trees) on the down stream slope, and minor concrete deterioration in the spillway. The size classification of the facility is intermediate and its hazard classification is considered to be significant. In accordance with recommended guidelines, the Spillway Design Flood (SDF) for this facility is 1/2 the Probable Maximum Flood (PMF). Results of the hydrologic and hydraulic analysis indicate that the facility can accommodate approximately 32 percent of the PMF and/or 64 percent of SDF (1/2 PMF). Thus, the spillway system is considered inadequate, but not seriously inadequate.

Book excerpt: Star Junction No. 2 Dam consists of an earthfill embankment and masonry spillway on the left abutment. The dam has a crest length of 341 feet, a maximum height of 30 feet and a storage volume of 31 acre-feet at the spillway crest level. The dam is classified by Corps of Engineers guidelines to be a small size, high hazard structure. Star Junction No. 2 Dam's embankment is assessed to be in poor condition. This is based on visual observations of the non-uniform slope, wet zones and state inspection reports. The downstream control was observed to be damaged and probably is not operable. No control or mechanism was observed to permit upstream flow control. The principal spillway is assessed to be in very poor condition. This is based on visual observations of the weir wall and training dike retaining wall. Both structures are partially collapsed and badly deteriorated and safe performance is questionable in the event of long-term, high discharge conditions.

Phase I Inspection Report written by GAI CONSULTANTS INC MONROEVILLE PA. and published by . This book was released on 1979 with total page 140 pages. Available in PDF, EPUB and Kindle. Book excerpt: Beaver Dam is an earth embankment approximately 32 feet high and 385 feet long (including spillway). The dam is constructed with a masonry spillway located about 35 feet left of the right abutment. The crest of the spillway is 100 feet long and about 11 feet below the top of its wingwalls. Based on a visual inspection, operational history, and available engineering data, the dam is consisered to be in poor condition. The defunct facility has been essentially abandoned since 1952 and its reservoir is sediment filled. Results of the hydrologic and hydraulic analysis indicate that the facility is capable of passing and/or storing only 36 percent of the PMF without embankment overtopping. Structural deficiencies noted by the inspection team included substantial embankment overgrowth, an access road cut through the embankment near the left abutment, cracking of the masonry spillway wingwalls, and a sediment filled reservoir. Due to the seriously inadequate spillway and overall poor condition, the facility is considered unsafe but non-emergency as failure is not considered imminent.

Book excerpt: Based on the evaluation of the existing conditions on March 14 and April 16, 1979, the condition of Little Blue Run Dam is considered to be good. No conditions were noted at this time that would significantly affect the structural performance of the dam. The flood discharge facilities for the dam were designed to pass the probable maximum flood (PMF) without overtopping the embankment. Therefore, according to the recommended criteria, the flood discharge facilities of the dam are adequate. Presently, the pool elevation is approximately 170 feet below the dam crest level. The pool elevation is expected to reach spillway crest level in about the year 2010. The following recommendations should be implemented on a continuing basis: (1) The cracking observed on the crest should continue to be evaluated by the design engineers. Also, attention should be given to the slump and seepage observed on the left abutment. Erosion ditches on the right abutment should be filled; and (2) A formal warning system should be developed to alert the downstream residents in the event of emergencies.

Book excerpt: Based on a visual inspection, operational history, and available engineering data, the dam is considered to be in good condition. The size classification of the facility is intermediate and its hazard classification is considered to be high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for the facility is the Probable Maximum Flood (PMF). Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store about 92 percent of the PMF prior to embankment overtopping. Conseqently, the spillway is assessed as being inadequate, but not seriously inadequate. Deficiencies at the facility are confined to seepage along the abutment-embankment contacts (particularly the left abutment) and poorly drained conditions downstream of the outlet conduit.
