Deficiencies noted by the inspection team included slumping and bulging of the rock surface on the steep downstream embankment slope, minor seepage along the downstream embankment toe and minor deterioration of the control tower. In addition, the growth of large trees in the immediate area of the downstream toe is a hindrance to expedient visual evaluation of the embankment. The size classification of the facility is small and its hazard classification is considered to be high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for the facility ranges between the 1/2 PMF (Probable Maximum Flood) and the PMF. Due to the high potential for damage to downstream structures and possibly loss of life, the SDF is considered to be the PMF. Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store only 27 percent of the PMF prior to embankment overtopping. A breach analysis indicates that failure under less than 1/2 PMF conditions could lead to increased downstream damage and potential for loss of life. Thus, based on screening criteria provided in the recommended guidelines, the spillway is considered to be seriously inadequate and the facility unsafe, non-emergency.

Consequently the spillway is assessed as being inadequate, but not seriously inadequate. Deficiencies noted by the inspection team included the following: (1) large trees rooted within the downstream embankment face; (2) a damaged and inadequate spillway discharge channel; and (3) a bent gate stem and partially obstructed outlet associated with the blowoff conduit.

However, some undesirable seepage was noted to be occurring through and immediately above the downstream rock toe drain. Seepage was also noted immediately above the impact basin through the rock toe drain. Flow was also noted through the pond drain although it was reported that the sluice gate was closed. This flow through both the rock fill toe and impact basin is unexplained and should be evaluated. Considering the adjudged good condition of the dam, the recommendations presented are suggested to insure that the dam continues to function as designed and to insure that the residents downstream are warned when impending high flows are expected along the creek. These recommendations are presented in order of priority, but does not infer that the latter recommendations are not important.

The auxiliary spillway is an excavated channel at the right abutment of the dam. It is grass-lined except for portions that were excavated into rock. The hazard classification is high hazard. Downstream conditions indicate that a high hazard classification is warranted for PA-468 Dam. It is owned by the Susquehanna County Commissioners. Its purpose is for flood control.

Book excerpt: Pole Run Dam No. 4 is a 30 foot high earthen embankment across Pole Run, a tributary of North Mahanoy Creek. The 780 foot long dam impounds a nine acre reservoir within an 0.58 square mile drainage basin. Very limited data exists regarding the physical features of the dam. The average measured downstream slope is 2H:1V, and the slope is covered with stone and vegetation. Since the average freeboard of the structure is less than one foot at normal pool level, the upstream slope could not be measured. A portion of the upstream slope is covered with stone, and this stone could be seen to extend at least two feet below the water and it is probable that it extends several feet below that elevation. The crest width ranges from 15 feet to 27 feet. The spillway is located at the right abutment, and consists of an open concrete channel with concrete retaining walls. The water supply intakes could not be inspected as they are below water and the pipes are buried in the embankment. The outlet pipes were inspected and three pipes were found. Near the toe of the dam, valves were located which are presumably used to control flow through the pipes.

Book excerpt: Warner Dam is owned by William S. Newlin, Jr., and is classified as a 'Low' hazard - 'Small' size dam. The dam was found to be in fair overall condition at the time of inspection. Hydraulic/hydrologic evaluations revealed that the spillway capacity is less than the peak inflow to the impoundment during the 100-year flood. A spillway design flood (SDF) in the range of the 50- year flood to the 100-year flood is required for Warner Dam. The 100-year flood was chosen as the SDF. The spillway is therefore considered 'Inadequate.' It is recommended that the owner immediately develop remedial measures to provide adequate spillway capacity. The inspection revealed certain items of remedial work which should be performed by the owner without delay. These include: (1) Provide adequate spillway capacity; (2) Repair the spalling on the downstream face of the concrete corewall; (3) Ensure that the outlet works are operable, or develop an emergency drawdown plan; and (4) Cut all trees on the embankment at ground level. All trees with a trunk diameter greater than 3 inches should have their root systems removed. All resultant areas of erosion and cavities should be filled, graded, compacted and seeded. In addition, the following operational measures are recommended to be undertaken by the owner: (1) Develop a detailed emergency operation and warning system; (2) During periods of unusually heavy rainfall, provide around-the-clock surveillance of the dam; and (3) When warning of a storm of major proportions is given by the National Weather Service, activate the emergency operation and warning system.

Book excerpt: The assessment of Pioneer Lake Dam is based upon visual observations made at the time of inspection, review of available records and data, hydraulic and hydrologic computations and past operational performance. The dam and appurtenant structures appear to be in fair condition. No major erosion was observed on the embankment crest. An erosion area was observed at the outlet for the 12 in. diameter cast iron pipe. Erosion at the outlet of the 12 in. diameter pipe exists on the downstream slope of the dam near the toe. Seepage 100 ft. right of the spillway channel was measured to be approximately 50 gallons per minute. A 30 in. corrugated metal pipe exists beneath the roadway near the outlet for the emergency spillway. It was observed during the inspection that the pipe is collapsing near the center of the pipe. Trees were observed growing along the upstream edge of the crest, and dense vegetation was observed along the entire length of the downstream slope. The Pioneer Lake Dam is a high hazard-small size dam. The recommended spillway design flood (SDF) for a dam of this size and classification is in the range of the 1/2 PMF to PMF. Due to the downstream potential for loss of life and property damage, the spillway design flood has been selected as the PMF. The spillway and reservoir are capable of controlling approximately 11% of the PMF without overtopping the embankment. Based on criteria established by the Corps of Engineers, the spillway is termed seriously inadequate. Pioneer Lake Dam is classified as unsafe, non-emergency.

The structure is classified as a 'small' size, 'high' hazard dam. Corps of Engineers guidelines recommend one half to one times the Probable Maximum Flood (PMF) as the Spillway Design Flood for a 'small' size, 'high' hazard dam. Upper Dam's Spillway Design Flood is one half the Probable Maximum Flood. Spillway capacity is 'adequate' because the non-overtopping flood discharge was found, by using the HEC-1 computer program, to be 84 percent of the PMF. (Author).

Book excerpt: Based on available records, visual inspection, calculations, past operational performance, and consideration of the existing normal pool level, Wanamie Dam is judged to be in fair condition. The existing spillway is a gorge that was eroded when the spillway washed out in 1955. The spillway can pass the Probable Maximum Flood (PMF), which is the Spillway Design Flood (SDF), with 7.9 feet of freeboard. Based on the size and hazard classification of the dam, the recommended SDF varies between the 1/2 PMF and the PMF. The selected SDF is based on the criteria and the downstream conditions. The pool level at maximum PMF level is 5.2 feet below the normal pool level that existed prior to 1955. The spillway capacity is rated as adequate. This rating is dependent upon removal of the remains of the timber structure in the spillway. The only significant stability problem that exists at the embankment is a bulge near the left abutment of the embankment. The owner presently has no intention of returning the dam to its design condition because there is no present requirement for increased water supply storage at the dam. There are no functional emergency drawdown facilities at the dam. Maintenance of the dam is inadequate.

Hydraulic/hydrologic evaluations, performed in accordance with procedures established by the Baltimore District, Corps of Engineers, for Phase I Inspection Reports, revealed that the spillway capacity is less than the peak inflow to the impoundment during the 100-year flood. A spillway design flood (SDF) in the range of the 100-year flood to the 1/2 Probable Maximum Flood (1/2 PMF) is required for Steinhauer Dam. Because the dam is on the low end of the 'Small' size category in terms of storage capacity and height, the 100-year flood was chosen as the SDF. The spillway will not pass the SDF without overtopping the dam. The spillway is therefore considered inadequate.
