Book excerpt: In Britain, between 1980 and 1998, the number of people classified as obese tripled to 21per cent of women and 17 per cent of men. It is estimated that this costs the economy, as a whole, �2 billion and the NHS �0.5 billion in treatment. However the response of the NHS is patchy, with no national guidelines and only 28% of health authorities taking action to address the problem. There is little activity related to the management of obesity outside of general practice but only a small proportion of GPs follow a protocol. This report recommends that there should be strategies to reduce obesity and that the Department of Health should build on the plan in the National Service Framework on coronary heart disease and work with partners and professional bodies to clarify responsibilities. It should also work with the National Institute of Clinical excellence to disseminate information. The Department of Health should also lead a cross government strategy to promote the benefits of physical activity and there should be local targets to encourage cycling and walking. It should also work with the food industry to improve the balance of diet. Much of this work needs to be aimed at school children to promote a healthy lifestyle throughout life and guidance to schools on commercial sponsorship should be strengthened so that there is no conflict with messages on healthy eating.

The NAO concludes that the Levy Control Framework is a valuable tool for supporting control of the costs to consumers that arise from the Government's energy policies, and has prompted the Department of Energy and Climate Change to monitor actual and expected costs to consumers from the schemes it covers. However, the operation of the Framework has not been fully effective in some key areas. Spending and outcomes have not been linked in deliberations by the joint Treasury and departmental levy control board and reporting on Framework schemes has not supported effective public and parliamentary scrutiny of the overall costs and outcomes from levy-funded spending. As consumer-funded spending on energy policies increases and new schemes are introduced, the Department needs to assure Parliament and the public that it has robust arrangements to monitor, control and report on consumer-funded spending, and the outcomes it is intended to secure. The spending cap under the Levy Control Framework is set to rise from £2 billion in 2011-12 to £7.6 billion in 2020-21 (in 2011-12 prices). By establishing this cap, the Department has provided greater certainty for investors. The NAO's report highlights that the Framework does not cover the consumer-funded Energy Companies Obligation scheme and that it is not yet clear whether it will cover the new Capacity Market including electricity demand reduction measures.

Local authorities have, so far, managed with reduced funding, but more are facing the challenge of avoiding financial difficulties while meeting their obligations. Central government planned at the 2010 spending review to reduce funding of local authorities by £7.6 billion (26 per cent) in real terms between April 2011 and March 2015. The effects on local authorities vary. In 2012-13, the overall reduction in spending power ranges from 1.1 per cent to 8.8 per cent. At the same time, demand for high-cost services, such as adult and children's social care, is increasing. The scope is diminishing for absorbing cost pressures through reducing other, lower cost, services given that spending on these services has already been reduced. Departments have assessed the impact of changes to local authority funding, but their approach needs to be more comprehensive in the future. The accountability framework for addressing widespread financial failure in local government is untested. Where there have been one-off failures requiring central government intervention, the failure regime has managed to resolve them. It is not known how the system would respond in the case of multiple financial failures in more challenging times for local authorities.

There are 205 projects in the Government Major Project Portfolio, with a combined whole-life cost of £376 billion, and annual cost of £14.6 billion. Thirty nine of those projects have a delivery confidence rating of 'red' or 'amber/red'. The report supports the changes that government has made to the central assurance system for such projects - in particular, the launch of the Major Projects Authority - and finds that they have resulted in some significant impacts. However, the system is not yet 'built to last'. Processes need to be formalised, and the Authority, HM Treasury and departments need to co-operate more if these improvements are to continue. Transparent reporting of project data would also create a more effective and enduring system. An effective central system that gives assurance over project progress is critical for ensuring successful outcomes. Capacity constraints have had an impact on the effectiveness of the assurance system. The Authority is reporting on 160 more projects as part of the portfolio and carrying out more in-depth assurance work, but has 40 per cent fewer staff than the body it replaced. As a result, processes for sharing project lessons and for continuously improving the system remain informal, and there is overdependence on key individuals in the system.

The National Audit Office has recommended that the Treasury ensure there are mechanisms to assess the cumulative impact of infrastructure investment on consumer bills. This report, which focuses on the energy, water and, to a lesser extent, telecoms sectors, recognizes that the UK requires significant investment in new infrastructure. The Treasury expects that over two-thirds of the £310 billion worth of the planned infrastructure it has identified will be privately financed, owned and operated but paid for by consumers through their utility bills. High levels of expected new investment in infrastructure mean that energy and water bills may rise significantly from current levels. The available projections suggest that increases in both energy and water bills will continue to outstrip inflation, on average, up to 2030. This is particularly concerning, given that energy and water bills have increased significantly in recent years, while incomes have not. The affordability of utility bills can be assessed only in the context of wider pressures on household expenditure, including an understanding of all household bills as well as potential trends in household incomes. There is a range of schemes to support vulnerable consumers but without a fuller understanding of affordability in the round, government and regulators cannot assess the adequacy of these schemes, now or in the future

Around two-thirds of this investment is expected to be financed and delivered by private companies but paid for by consumers through utility bills and user charges, such as rail fares. Economic regulators set the frameworks within which private companies deliver this infrastructure and they have legal duties to protect consumers by, for example, promoting competition, acting to prevent and address market abuses, and in some cases setting the prices consumers can be charged. Energy and water bills have risen considerably faster than incomes in recent years, and high levels of new investment in infrastructure mean that bills and charges are likely to continue to rise significantly. Furthermore, poorer households spend more of their incomes on household bills relative to richer households, meaning that funding infrastructure through bills is more regressive than doing so through taxation. Currently, consumers rely solely on Government and regulators to protect their interests. However no one in Government is taking responsibility for assessing the overall impact of this investment on consumer bills and whether consumers will be able to afford to pay. This is a particular concern given that the poorest households are hit hardest by increases in bills.
