Available in PDF, EPUB and Kindle. Book excerpt: Living organisms exhibit specific responses when confronted with sudden changes in their environmental conditions. The ability of the cells to acclimate to their new environment is the integral driving force for adaptive modification of the cells. Such adaptation involves a number of cellular and biochemical alteration including metabolic homeostasis and reprogramming of gene expression. Changes in metabolic pathways are generally short-lived and reversible, while the consequences of gene expression are a long-term process and may lead to permanent alternation in the pattern of adaptive responses. The heart possesses remarkable ability to adapt itself against any stressful situation by increasing resistance to the adverse consequences. Stress composes the foundation of many degenerative heart diseases including atherosclerosis, spasm, thrombosis, cardiomyopathy, and congestive heart failure. Based on the concept that excessive stress may play a crucial role in the pathogenesis of ischemic heart disease, attempts were made to design methods for preventing of myocardial injury. Creation of stress reactions by repeated ischemia and reperfusion or subjecting the hearts to heat or oxidative stress enables them to meet the future stress challenge. Repeated stress exposures adapt the heart to withstand more severe stress reactions probably by upregulating the cellular defense and direct accumulation of intracellular mediators, which presumably constitute the material basis of increased adaptation to stress. Thus, the powerful cardioprotective effect of adaptation is likely to originate at the cellular and molecular levels that compose fundamental processes in the prophylaxis of such diseases. Volume six of the Advances in Organ Biology series contains state-of-the-art reviews on myocardial preservation and cellular adaptation from the leading authorities in this subject.

Book excerpt: Interest in the ability of myocardium to adapt to ischaemic stress has continued to grow since the discovery of ischaemic preconditioning in 1986. In 1993, two reports heralded the recognition of a delayed preconditioning response in the heart, now commonly known as the `second window' of protection. Since then, a number of studies have described the ability of delayed preconditioning and related adaptive phenomena to protect against a variety of pathologies in the ischaemic and reperfused myocardium. Our understanding of the cellular mechanisms of sub-acute adaptive cardioprotection has advanced considerably during this period. This compilation of state-of-the-art reviews by those who have made significant contributions to this field provides detailed and critical analysis of this research, from molecular basis to potential clinical relevance. The book aims to provide an authoritative, comprehensive and thoroughly up-to-date overview for scientists and clinicians engaged in, or observing, this rapidly-developing area of heart research. It will also be of interest to those engaged in research on other tissues where ischaemia-reperfusion pathology is of major concern.

The first part describes the differences between hypoxia and ischemia, animal models, the effects of ischemia on myocardial function and metabolism, and the electrophysiological consequences of ischemia. The second part deals with the mechanisms of cardiomyocyte death in ischemia, structural aspects of irreversible ischemic injury, necrosis and apoptosis of cardiac cells, the role of calcium, and the concept of calcium antagonism. The third chapter is a brief description of reperfusion injury, its clinical relevance, and possible prevention. The fourth part summarizes changes in myocardial vasculature during ischemia and reperfusion. The fifth part is the survey of two main possibilities for increasing cardiac resistance to ischemia and hypoxia, i.e. long-lasting adaptation to chronic hypoxia and short-lasting preconditioning. The last part of the book deals with comparative and ontogenetic aspects of cardiac sensitivity to oxygen deprivation; this chapter also summarizes the ontogenetic differences and limitations in endogenous and exogenous protection of the ischemic/hypoxic heart.

Unlike the cir culations of most other organs, if the coronary circulation becomes insufficient to sustain myocardial contractile function, overall quality of life rapidly declines and life can abruptly end. In partnership with the cerebral circulation, the coronary circula tion plays a central role in sustaining life. However, unlike the cerebral circulation, whose function is self-sufficient to define life, the coronary circulation determines global blood flow and thus the initial state for the remainder of the body. This unique condition, together with the reality that coronary artery disease primarily affects peo ple in their productive years of life, has allowed physicians and scientists who study the heart to enjoy a privileged position in the field of medical research and clinical practice. Thus, it comes as little surprise that many new and exciting research developments involving the coronary circulation have recently come to the forefront of medical thinking. This reality, coupled with the large number of clinical trials of agents specif ically designed to sustain or improve coronary flow in many disease states, makes the timing of this monograph relevant. The book features papers presented at a recent international symposium, the fourth in a series of meetings on Applied Physiology of the Peripheral Circulation. The papers selected by the editors review the most impor tant advances in the fields of cardiology and the coronary circulation.

Available in PDF, EPUB and Kindle. Book excerpt: This book is about the unique ability of the heart to protect itself against the serious consequences of an abrupt reduction in blood flow to the myocardium - a phenomenon which is switched on by stress and termed myocardial preconditioning. This volume describes the main features and characteristics of preconditioning, with respect to the time-course of the protective effect of the phenomenon (first and second windows) and the different sequelae of myocardial ischaemia against which it protects, such as myocardial injury, arrhythmias and contractile dysfunction. The proposed mechanisms underlying the fascinating form of cardioprotection are discussed. Among these are the role of endogenous labile mediators, receptor-linked G proteins, ATP-sensitive potassium channels and protein kinase C activation.

Available in PDF, EPUB and Kindle. Book excerpt: The importance of the developmental approach for experimental and clinical cardiology is indisputable. Clinical-epidemiological studies have clearly shown that the risk factors of serious cardiovascular diseases, such as atherosclerosis and ischemic heart disease, are already present during the early phases of ontogenetic development. Furthermore, congenital cardiovascular malformations remain the single largest cause of infant mortality from congenital defects in industrial countries. It is therefore not surprising that the interest of theoretical and clinical cardiologists in the developmental approach keeps increasing. Advances in molecular biology accelerated this trend substantially. This book is based on contributions presented at the international symposium The Developing Heart in Prague in May 2000. It is our contention that the biological, electrophysiological, morphological, functional, biochemical and functional approaches employed by distinguished scientists worldwide will provide the reader with a global picture for changes characterizing the developing heart. It should stimulate the curiosity of cardiovascular scientists in gaining insight into the mechanisms of normal and pathological development.

The contributors review the initial adaptation to ischemia and the concept of preconditioning. Close attention is given to knowledge of myocardial stunning, including the role of calcium homeostasis and oxyradicals as well as the clinical impact and pharmacological modulation of myocardial stunning. Myocardial hibernation and its link with myocardial stunning are also discussed. The contributors attempt to integrate experimental data and clinical observations. The understanding of the basic pathophysiological mechanisms of these ischemic syndromes should help readers to correctly identify and treat patients with chronic ischemic left ventricular dysfunction.

Available in PDF, EPUB and Kindle. Book excerpt: This book explores the possibilities of protecting the heart against stress and ischemia through adaptation to intermittent hypoxia or to mild, nondamaging stress exposure. These possibilities are based on studies that show an increase in the potency and efficiency of stress-limiting systems when subjected to repeated stress actions, adaptation to high altitude hypoxia, or adaptation to increased physical loads. Any stress reaction is coupled with an activation of central and local stress-limiting systems, such as GABA-ergic, opioidergic, and serotonergic systems in the brain, as well as antioxidants, prostaglandins, and adenosine in executory organs. The book also presents the use of metabolites of stress-limiting systems rather than adaptation as a means to prevent cardiac arrythmias. It also discusses genetically determined deficiencies of stress-limiting systems and their role in the etiology of stress-induced diseases. Cardiologists and researchers studying stress and its effect on cardiovascular systems will find this book extremely useful.
