Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

Book excerpt: Does a prepaid groupe practice deliver less outpatient mental heath care than fee-for-service when they both serve comparable populations? to answer this question the authors used data from a randomized controlled trial which assigned families into a prepaid group practice and into fee-for-service insurance plans. The pattern of outpatient mental health care differs for those enrolled at the prepaid group practice and those enrolled in the fee for service. This type provides more expensive and more intensive therapy per user. The enrollees are 50% more likely to see a mental health specialist at leat once over a three years period when they depend of the prepaid group. This difference is due to a higher rate of "turnover" of patients in this type of plan.

Book excerpt: While health maintenance organizations (HMOs) have lower medical costs than fee-for-service plans with the same benefits, it has not been clear whether the cost reductions in HMOs, achieved largely by reductions in hospital admissions, have adverse effects on health. This study addresses this important issue for nonaged adults. It describes the RAND Health Insurance Experiment, including the sample and methods of analysis. The findings indicate that the nonpoor suffer no harm to health through participation in an HMO and their enrollment should be encouraged. Low-income people who have health problems when they join an HMO appear to be worse off at the HMO compared with a fee-for-service plan.
