Book excerpt: Des cas d' allongement de l'intervalle QT et de torsades de pointes (TdP) ont récemment été décrits chez des patients traités par méthadone, pour des posologies élevées, inhabituelles en substitution, mais aussi pour des posologies plus modérées. Les co-prescriptions médicamenteuses pourraient favoriser les effets indésirables de la méthadone. L'objectif principal de ce travail a été dans un premier temps d'évaluer la fréquence d'allongement de l'intervalle QTc et de sa dispersion (DQTc) chez les patients traités par des méthadone puis, dans un deuxième temps, de rechercher les facteurs associés à ces modifications de l'électrocardiogramme. Nous avons d'abord consulté les données de la Banque Nationale de Pharmacovigilance dans le but de décrire et d'analyser les effets indésirables, associés à la méthadone et à la buprénorphine, notifiés aux centres de pharmacovigilance. Nous avons ensuite réalisé une étude transversale chez des patients traités par méthadone. Nous avons trouvé une relation significative entre l'augmentation récente de posologie de méthadone et la co-prescription de médicaments allongeant le QTc et l'allongement du QTc et de sa dispersion. Beaucoup de ces patients étant traités par des neuroleptiques, connus pour allonger le QTc, nous avons complété notre étude par une comparaison des enregistrements ECG d'une série de patients hospitalisés dans le département de psychiatrie du CHU. Nous avons observé un allongement du QTc plus important chez les patients de psychiatrie et une DQTc plus élevée chez les patients traités par méthadone. Nous avons trouvé une association entre la consommation de méthadone et de neuroleptiques et l'allongement de la DQTc. La DQTc pourrait être un indicateur, plus sensible que le QTc, du risque de survenue de TdP chez les patients traités par méthadone.
