Book excerpt: "If the Department of Defense (DoD) moves toward a more integrated employee health system, a foundation of information about the current system and requisite elements for such integration will be needed. The authors reviewed the research literature and DoD policy documents and interviewed DoD personnel to make several observations about the current state of safety and occupational health (SOH) arrangements in DoD. Currently, SOH policy cuts across several organizations at high levels in the Office of the Secretary of Defense, and SOH programs are implemented by each of the military services. Recently, leadership attention has focused on safety, mostly apart from occupational health, as a separate priority. DoD and the services have made efforts to increase coordination, including both high-level formal councils and through informal relationships among SOH practitioners. Health promotion and wellness have received considerable attention within DoD through periodic health assessments and educational programs, yet these areas have not benefited from the same increased coordination. As DoD contemplates a more integrated approach, the authors considered what DoD might learn from civilian experience with integrating employee health activities. To address this, the authors reviewed civilian models of integration to identify promising approaches and practices that might inform DoD efforts. The review of activities related to employee health in DoD -- including industrial hygiene, safety, health promotion and wellness, occupational health, and its relatively mature health information technology infrastructure -- indicates that there might be little need for DoD to introduce new programs but more need to make use of the information generated by the existing programs in a more coordinated, integrated manner." -- provided by publisher.

Speakers examined how obesity and overweight are measured in the armed forces and how they affect recruitment, retention, resilience, and readiness; discussed service-specific issues related to these problems and highlighted innovative strategies to address them through improved nutrition, physical activity, and stress management; and offered perspectives from outside of the armed forces on approaches to prevent and treat obesity. They also discussed the challenges and opportunities related to overcoming the concerns posed by obesity and overweight in the armed forces, military families, and their communities, including potential cross-sector opportunities. This publication summarizes the presentations and discussions from the workshop.

Book excerpt: In 1986, DoD established a formal health promotion policy, but it was not until 1992 that DoN components began to comply and implement health promotion programs. In 1994, Navy Medicine appointed the Navy Environmental Health Center (NEHC) as the Health Promotion Program Manager. In 1998, due primarily to their population-based approach to health care delivery, NEHC was appointed the Program Manager for the Clinical Epidemiology Program (CEP). This study examines the resource and programmatic role that NEHC has played in implementing health promotion programs in Navy Medicine, particularly the CEP. Interviews and a review of the literature on health promotion and clinical epidemiology were undertaken to ascertain the significance and implications of health promotion programs in relation to the overall health care strategy of Navy Medicine. This thesis concludes that several problems affect implementation of health promotion, including the inability to capture the total cost of the program and identify cost savings and cost avoidance; the absence of a reliable benchmark of the health status of selected populations; and the absence of data to measure the efficacy of the program. Continued implementation and support of the CEP may permit Navy Medicine to document the significance of health promotion in enhancing population health and identify cost savings and cost avoidance directly related to health promotion efforts.

Book excerpt: This report describes the assessment of Perceptions of Wellness and Readiness (POWR) among active-duty Navy and Marine Corps personnel. Supported by the Defense Women's Health Research Program, the purpose of this study was to obtain baseline prevalence and risk factor information on a representative sample of active-duty Navy and Marine Corps women and men worldwide and to provide relevant comparative data with civilian populations. The POWR Assessment consisted of three separate, but complementary components. The first and most comprehensive component was a large-scale survey in which respondents completed an in-depth self-report questionnaire that assessed six key issues, including reproductive health, medical history and nutritional status, mental health, lifestyle issues, occupational/environmental risks, and health services issues. This questionnaire study was based on a probability sample of approximately 10,000 active-duty Navy and Marine Corps personnel. The second component consisted of physical measurements taken on a subsample of approximately 1,200 respondents to the main survey. The third component was a telephone interview drawn from volunteers responding to the main survey. This report includes background about the study and discussions of the sampling design, the data collection instruments, data collection methods, sample weighting and estimation procedures, and preliminary results.
