Book excerpt: The position of "Cancer Teaching Coordinator" at The University of Chicago has been a consistently rewarding one because of the enthusiasm and support of the faculty and the students. This volume is the result of the second of two recent intensive teach ing sessions which have been planned and implemented by the group which forms the Cancer Coordinator's Advisory Committee. The first of these teaching sessions was held in early March of 1964 and was entitled "LEUKEMIA, A Current and Forward Look. " It attracted overflow attendance from the students and staff of this medical institution augmented by members of the other medical centers in Chicago. It was a stimulating and instructive colloquium, and the only regret we heard expressed afterward was that we had not arranged for publication of the many excellent presentations. One of the events commemorating the seventy-fifth anniversary of the founding of The University of Chicago was this symposium on malignant transformations. This time the Committee advised us to plan on speedy publication, and, logically, it chose Dr. Werner Kirsten, a member of our faculty and an active and effective investigator in this general field of endeavor, to serve as editor of the volume. Again, two of the same ingredients for effective instruction were blended: an excellent group of scientists presenting their latest work and a fine, attentive audience of students and staff.

A detailed examination of the natural history of these tumors is essential to the design and evaluation of therapeutic trials. The highly lethal nature of many of these tumors, the occurrence of some of them at several different primary sites and the rarity of these tumors have made systematic study of them difficult. Conclusions regarding the efficacy of a particular modification of the therapeutic strategy can be strongly influenced by the assumed natural history of the tumor. I have tried to develop as accurateJy as the literature would allow a picture of the natural history of the common malignant solid tumors, knowing that the image would be imperfect. Book excerpt: Malignant pleural mesothelioma (MPM) is a rare thoracic cancer that derives from the mesothelial cells of the pleura and is causally associated with exposure to asbestos. Because of its aggressiveness and resistance to therapies, MPM prognosis is extremely poor and the 5-year survival rate is approximately 10%. Despite its poor effectiveness, chemotherapy with cisplatin and pemetrexed has long remained the standard-of-care for upfront treatment with no second line therapy available. Recently, the approval of ipilimumab (anti-CTLA-4) plus nivolumab (anti-PD-1) for the first-line treatment of unresectable MPM has marked a significant milestone, in particular for the non-epithelioid MPM subtype, which is more resistant to chemotherapy and associated with a poorer outcome. However, most MPM patients are still resistant to cancer therapies. Beyond histologic subtypes, recent studies have highlighted extensive genetic variation and gene expression deregulation both between and within MPM patients. This molecular heterogeneity highlights the need for a personalized treatment approach. Therefore, both multiple therapeutic options and predictive biomarkers that can guide clinical decision-making are urgently needed. Tumor cell immunogenicity and tumor microenvironment composition (TME) are the major determinants of the success of immunotherapy. Despite the low tumor mutational burden, recent studies indicate that chromoanagenesis and chromotripsis are molecular features of MPM. Since chromosomal instability can lead to the expression of neo-antigens, it could also influence the efficacy of immune checkpoint inhibitors (ICIs). The immunosuppressive TME is an additional important hurdle that limits the efficacy of immunotherapy. The combination of ICIs with strategies to boost anti-tumor immunity by restraining immunosuppressive myeloid cells or by reprograming “cold tumor” into “hot tumor” is an active area of research. This Research Topic aims to discuss recent advances in understanding and targeting MPM immune microenvironment for therapeutic purposes along with potential predictive biomarkers to select patients who most likely will respond to immunotherapies. We are interested in basic and clinical studies that explore the crosstalk between tumor and immune cells in order to highlight new molecular targets, approaches, and combination treatments to enhance anti-tumor immunity in MPM and assess potential predictive biomarkers of response to immunotherapy.

Both parties should cooperate whenever this is useful and possible.
