Book excerpt: The United States faces major challenges in dealing with Iran, the threat of terrorism, and the tide of political instability in the Arabian Peninsula. The presence of some of the world’s largest reserves of oil and natural gas, vital shipping lanes, and Shia populations throughout the region have made the peninsula the focal point of US and Iranian strategic competition. Moreover, large youth populations, high unemployment rates, and political systems with highly centralized power bases have posed other economic, political, and security challenges that the Gulf states must address and that the United States must take into consideration when forming strategy and policy.

Book excerpt: Examining-room computers require doctors to record detailed data about their patients, yet reduce the time clinicians can spend listening attentively to the very people they are trying to help. This book presents original essays by distinguished experts in their fields, addressing this critical problem and making an urgent case for reform, because while electronic technology has revolutionized the practice of medicine, it also poses a unique challenge to health care. Smartphones in the hands of doctors and nurses have become dangerously seductive devices that can endanger their patients. Distracted Doctoring is written for anesthesiologists and surgeons, as well as general practitioners, nurses, and health care administrators and students. Chapters include Electronic Challenges to Patient Safety and Care; Distraction, Disengagement, and the Purpose of Medicine; and Managing Distractions through Advocacy, Education, and Change.

Book excerpt: The story goes, apocryphal perhaps, that one day the Ugandan dictator, Idi Amin, told his foreign minister that the country's name must be changed to Idi, and he should inform the UN and all other international bodies. A week passed. President Amin then summoned the minister and asked, 'Did you carry out my orders?' He replied saying that there was a problem. 'What problem?' the president inquired. 'Your Excellency, there is a country called Cyprus. The people are called Cypriots. If Uganda were to be called Idi, we would be called Idiots.' There are few leaders that K. Natwar Singh, in a diplomatic career spanning more than three decades, has not known - and fewer still about whom he has no story to tell. In Walking with Lions: Tales from a Diplomatic Past, Singh puts together fifty episodes that entertain, inform and illuminate. Featured here is Indira Gandhi as a statesman and friend, alongside other renowned figures such as Fidel Castro, Haile Selassie and Zia-ul-Haq. Singh analyses some personalities with disarming candour, among them Morarji Desai and Lord Mountbatten; at other times, his admiration for leaders like C. Rajagopalalchari and Nelson Mandela shines through. In these pages you will also find a rare, fascinating glimpse of Godman Chandraswami and his cohort Mamaji, and their interaction with a surprisingly submissive Iron Lady Margaret Thatcher. Besides, there are short tributes to artists, writers, cricketers and film stars, like M.F. Husain, Nadine Gordimer, Don Bradman and Dev Anand. Recounted with empathy and humour, this collection of stories from contemporary history is a warm, unaffected and reassuring reminder that the great too can be as fallible as the rest of us.

Book excerpt: Most industries have plunged into data automation, but health care organizations have lagged in moving patients' medical records from paper to computers. In its first edition, this book presented a blueprint for introducing the computer-based patient record (CPR). The revised edition adds new information to the original book. One section describes recent developments, including the creation of a computer-based patient record institute. An international chapter highlights what is new in this still-emerging technology. An expert committee explores the potential of machine-readable CPRs to improve diagnostic and care decisions, provide a database for policymaking, and much more, addressing these key questions: Who uses patient records? What technology is available and what further research is necessary to meet users' needs? What should government, medical organizations, and others do to make the transition to CPRs? The volume also explores such issues as privacy and confidentiality, costs, the need for training, legal barriers to CPRs, and other key topics.

Book excerpt: Effluent Limitations Guidelines and New Source Performance Standards for the Airport Deicing Category (US Environmental Protection Agency Regulation) (EPA) (2018 Edition) The Law Library presents the complete text of the Effluent Limitations Guidelines and New Source Performance Standards for the Airport Deicing Category (US Environmental Protection Agency Regulation) (EPA) (2018 Edition). Updated as of May 29, 2018 EPA is promulgating technology-based effluent limitations guidelines (ELGs) and new source performance standards (NSPS) under the Clean Water Act (CWA) for discharges from airport deicing operations. The requirements generally apply to wastewater associated with the deicing of airfield pavement at primary airports. The rule requires all such airports to comply with requirements based on substitution of less toxic pavement deicers that do not contain urea. The rule also establishes NSPS for wastewater discharges associated with aircraft deicing for a subset of new airports. These airports must also meet requirements based on collection of deicing fluid and treatment of the collected fluid. The ELGs and NSPS will be incorporated into National Pollutant Discharge Elimination System (NPDES) permits issued by the permitting authority. EPA expects compliance with this regulation to reduce the discharge of deicing-related pollutants by 16 million pounds per year. EPA estimates the annual cost of the rule at $3.5 million. This book contains: - The complete text of the Effluent Limitations Guidelines and New Source Performance Standards for the Airport Deicing Category (US Environmental Protection Agency Regulation) (EPA) (2018 Edition) - A table of contents with the page number of each section
