Dans ce contexte se développe fortement la médecine palliative, comme c'est le cas au Centre hospitalier de Saint Nicolas de Port en Meurthe-et-Moselle, qui accueille des lits identifiés de soins palliatifs en soins de suite et de réadaptation. Répondant à une volonté croissante tant de la part du corps médical que des instances sanitaires, son activité de prise en charge palliative est en croissant développement depuis quelques années, mais cela ne va pas sans les répercussions budgétaires importantes que ce type d'activité implique, notamment pour le budget de la pharmacie. La première partie de cette thèse présente ainsi les lits identifiés de soins palliatifs en soins de suite et de réadaptation, leur organisation et leur fonctionnement. Le statut de "lits identifiés" est à présent bien défini et réglementé, et permet une reconnaissance de la pratique des soins palliatifs et justement une valorisation financière pour cette activité. Mais le financement des hôpitaux est, depuis le plan Hôpital 2007, en pleine réforme avec un nouveau système de financement des hôpitaux qu'est la tarification à l'activité. Cette réforme va être progressivement appliquée au service de soins de suite et de réadaptation, et dans ce cadre sont actuellement réalisées des études ministérielles visant à évaluer les besoins globaux de prise en charge palliative en soins de suite et réadaptation.La seconde partie de cette thèse a ainsi voulu participer à l'évaluation plus précise des implications budgétaires, ceci au niveau du coût médicamenteux, au sein du service de soins de suite et de réadaptation du Centre hospitalier de Saint Nicolas de Port. L'étude a été réalisée sur des échantillons représentatifs de plus de 30 patients, sur une période étalée de 2005 à 2007. On en conclut que la prise en charge médicamenteuse pour un patient en soins palliatifs, à profil de dépendance identique, est plus onéreuse que celle d'un patient en soins de suite et réadaptation classique, à hauteur de plus du double. Durant les derniers jours de vie du patient, le coût médicamenteux d'un patient en soins palliatifs diminue légèrement, mais sans se rapprocher de celui d'un patient en soins de suite et réadaptation.

Information ranges from explanations of how specific drugs work, through the precautions to take in the presence of certain disorders, to a list of factors that influence the effectiveness of opioids. Concerning the use of opioids, readers are reminded that psychological dependence does not occur in cancer patients and that the only correct dose of morphine is the one that relieves the pain. Part two provides a guide to opioid availability. A discussion of the reasons why opioids continue to be underprescribed or difficult to obtain is followed by an explanation of the Single Convention on Narcotic Drugs.
