La prise en charge des personnes âgées passe d’abord par leur maintien à domicile qui nécessite des aides à la fois organisationnelles et humaines. La réussite de ce projet va dépendre de nombreuses variables : le souhait de la personne elle-même, son projet, la mise en place des aides, leur coordination assurée généralement par l’aidant familial. Cette étude prospective réalisée chez des patients au moment de leur entrée en maison de retraite a permis d’analyser les facteurs de rupture du maintien à domicile et le rôle de l’aidant familial, considéré comme garant du maintien à domicile. De ce travail, ressortent deux éléments moteurs: l’état de santé du patient avec des troubles fonctionnels ou psycho-comportementaux altérant son autonomie, et, l’épuisement de l’aidant sur le plan physique ou psychologique. Comment éviter l’institutionnalisation ? Que peut on proposer aux aidants ? Aujourd’hui on apporte essentiellement des aides professionnelles et organisationnelles pour faire face au handicap de la personne âgée. L’aide à l’aidant est une alternative vers laquelle il faut se tourner pour optimiser le maintien à domicile, sur sa durée et sa qualité. La reconnaissance du rôle de l’aidant informel, le droit au répit, le soutien psychologique sont autant d’éléments permettant de garantir la pérennité du projet de la personne âgée.
