D'autant plus que le pronostic neurologique de l'enfant dépend directement de la qualité et de la rapidité des gestes entrepris. Pour ces raisons, les étudiants doivent être compétents dans ce domaine à la fin de leur cursus. Méthode : L'objectif de cette étude était d'évaluer le sentiment de compétence des étudiants sages-femmes lors d'urgences néonatales. Cette enquête a été réalisée auprès des étudiants en dernière année, dans 22 écoles françaises. Résultats : Globalement les étudiants se sentaient compétents dans la réanimation néonatale (69,84%). Le temps de formation semblait augmenter ce sentiment de compétence de manière plus significative que le type de formation (haute ou basse fidélité). Conclusion : La simulation est une méthode de formation appréciée des étudiants. Donc la répétition des simulations basses fidélités serait bénéfique pour améliorer le sentiment de compétence. Une forme certificative des formations en réanimation néonatale en formation initiale et en formation continue pourrait être envisagée pour valider les compétences des futurs professionnels et des sages-femmes.

Nous avons donc cherché à connaitre les modalités d'enseignement de la réanimation néonatale dans les écoles françaises, afin de savoir si celles-ci préparaient au mieux les futurs professionnels à ces situations d’urgence. Nous avons interrogé les étudiants sages-femmes de dernière année. La majorité des étudiants sont satisfaits de leur formation mais un tiers ne se sent pas capable d'effectuer une réanimation par manque de pratique. La formation initiale donne les bases, cependant il reste des points à améliorer.

Book excerpt: Notre objectif était de faire un état des lieux de l'utilisation des compétences de la sage-femme récemment diplômée lors de la prise en charge de sa première réanimation néonatale en salle d'accouchement, le pédiatre, l'anesthésiste ou l'obstétricien n'étant pas présents. Notre étude est rétrospective, exhaustive, transversale, descriptive et a été réalisée à l'aide d'un questionnaire. Notre population comprend 214 sages-femmes diplômées en 2006 et en 2007. Ces professionnels ont suivi leur formation de sage-femme en France, exerçaient en salle d'accouchement en France et travaillaient dans les établissements du secteur V du conseil interrégional de l'ordre des sages-femmes pendant la durée de l'étude. 75 % des sages-femmes nouvellement diplômées ont été confrontées, sans la présence d'un médecin, à une réanimation néonatale en moyenne au bout de 5 mois d'exercice. Le plus souvent, la sage-femme travaillait en équipe, le matériel des tables de réanimaation était complet, et le médecin était appelé à bon escient. Les médecins mettaient entre deux et cinq minutes pour arriver, ce qui a permis aux sages-femmes de réaliser les gestes de réanimation " minimum " avant que celui-ci n'arrive. Lors de situations moins fréquentes, celles-ci ont pratiqué le massage cardiaque externe. L'intubation du nouveau-né a été rarement effectuée et la pose de voie veineuse ombilicale n'a jamais été réalisée. 74 % des sages-femmes redoutaient la prise en charge d'une réanimation. 42 % ont trouvé leur formation initiale correcte et 60 % avaient l'intention de suivre une formation complémentaire. Nous pouvons en conclure l'importance pour les sages-femmes de maîtriser parfaitement ces actes qualifiés de " minimum ", et particulièrement la ventilation au masque. Dans les actes de réanimation " complexe ", la capacité de poser des cathéters veineux ombilicaux par la sage-femme semble être intéressante.

Objectif : Identifier les facteurs intrinsèques et extrinsèques qui impactent l'encadrement et le niveau de confiance accordé par la sage-femme à l'étudiant en salle de naissance. Il s'agit d'une étude prospective et quantitative réalisée auprès des sages-femmes exerçant en salle de naissance à la Réunion. Résultats : L'âge de la sage-femme et son niveau de confiance en elle ont une influence significative sur son positionnement en tant qu'encadrant. Très peu de sages-femmes sont formées à la pédagogie. Concernant les facteurs extrinsèques : le stress, a une influence significative sur le positionnement des sages-femmes en tant qu'encadrant et sur leur choix d'encadrer les étudiants. Il n'y a pas de donnée significative concernant les critères propres à l'étudiant. On note que la sage-femme lui fera plus facilement confiance lorsqu'elle le connaît. Conclusion : Mettre en place une formation en pédagogie pour la population étudiée. Favoriser le fait que l'étudiant travaille sur un même terrain pendant une longue période pour être connu des sages-femmes.

Book excerpt: Recent years have seen important developments in the computer and game industry, including the emergence of the concept of serious games. It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured? Are the potential benefits of such products specific, or can these additions be attributed to nonspecific factors? What are the users’ views on such tools? What are the possible links between such tools and social networks? Is there a gap between evidence-based results and market development? Are there any quality challenges? What future developments and studies are needed in the field?
