Tout défect maxillo-facial peut être traité par reconstruction chirurgicale, par reconstruction prothétique ou par l'association des deux procédés. La reconstruction chirurgicale, apportant des tissus autologues, offre le plus souvent les solutions les plus intéressantes sur les plans conceptuels et morphologiques. Autant que possible, la reconstruction chirurgicale sera préférée. Il reste des cas complexes et particuliers où le choix thérapeutique se porte vers la reconstruction par épithèse. Celle-ci peut être maintenue par collage au plan cutané, être fixée sur des montures de lunettes ou mieux être supportée par des implants extra-oraux (IEO) endo-osseux oujuxta-osseux. Le concept matériel des IEO est l'assemblage d'un implant fixé à l'os et d'un pilier percutané. La possibilité d'ancrer définitivement des prothèses externes à un os sous-jacent a transformé les conditions de vie de patients soufflant de mutilations variées, de la simple perte dentaire à celle d'un membre ou de toute une partie du visage. Les IEO ont, ensuite, permis, dans le domaine des prothèses maxillo-faciales, d'améliorer le mode de fixation des épithèses. En chirurgie reconstructive comme en chirurgie maxillo-faciale, le recours aux épithèses "implanto-portée " est une option thérapeutique relativement récente. En France, les premières poses d'IEO datent de 1987 et des applications ORL : pavillon de l'oreille et BAHA (Bone Anchored Hearing Aid). Les premiers cas français de prothèses maxillofaciales implantées oculo-palpébrales, nasales et faciales complexes datent de 1989. Il existedeux sortes d'ïmplants : les implants endo-osseux de type suédois. Le deuxième typed'implants étudiés sont les implants non enfouis: le plus connu est l'implant plaque de FARMAND. Nous conclurons que l'épithèse implanto-portée est désormais une technique de choix dans le schéma thérapeutique de la réhabilitation des pertes de substance de la face. Quelques cas cliniques seront présentés afin de mettre en évidence l'apport des IEO dans la prothèse maxillo-faciale.

Fretting corrosion is a particular degradation mechanism; it highlights the key role of passive film, crevice corrosion, etc. For demonstrating the electrochemical effect of the fretting corrosion of metal, some investigations are presented at free corrosion potential and at applied potential to measure the specific current density. Moreover, the role of proteins is investigated because they constitute the biological environment and thus play a significant role in fretting corrosion processes.
