Book excerpt: Introduction : L'algorithme ABCDE(F) est une approche dynamique de l'évaluation et du traitement des patients en situation critique, développée aux États Unis dans les années 1980 par le Dr Styner, chirurgien orthopédiste. Il s'agit d'évaluer successivement les voies aériennes, la respiration, la circulation, l'état neurologique, les facteurs d'exposition et enfin, les aspects sociaux et logistiques, le contexte, les antécédents et le terrain. Connu de la plupart des services d'urgence et de réanimation et accepté par les comités d'experts, la question peut se poser de connaître sa place en médecine générale. Matériels et méthodes : Une étude multicentrique, d'observation épidémiologique transversale a été réalisée du 18 mars au 4 mai 2018, incluant 59 médecins généralistes du Finistère et 57 médecins généralistes de la Vienne, afin d'évaluer l'intérêt de l'utilisation de l'algorithme ABCDE(F) dans un contexte d'urgence et d'évaluer son impact sur la gestion de ces situations en médecine générale. Résultats : Environ un médecin généraliste installé ayant une activité libérale exclusive sur deux, connaît l'algorithme ABCDE(F). Parmi les médecins généralistes qui le connaissent, trois quarts l'utilisent dans le Finistère versus 67% pour la Vienne. Un taux de 67% des généralistes du Finistère estiment qu'il s'agit d'un bon outil de communication au SAMU versus 94% pour la Vienne. L'algorithme est intégrable à la pratique de tous les professionnels de la santé pour 63% des médecins installés du Finistère versus 68% pour la Vienne. Un médecin généraliste libéral sur 5 le juge davantage adapté aux urgentistes. Les médecins généralistes installés qui connaissent l'algorithme présentent également une plus grande facilité de gestion des urgences et de communication par rapport à ceux qui ne le connaissent pas. Analyse : Cette étude prouve donc l'intérêt de son application en médecine générale et son impact sur la prise en charge des patients en situation d'urgence. Discussion : Les résultats de cette étude sont en parfaite adéquation avec l'article Initial assessment and treatment with the Airway, Breathing, Circulation, Disability, Exposure (ABCDE) approach paru en janvier 2012 qui présage de l'utilisation de l'algorithme par tous les professionnels de la santé pour tous les patients relevant de l'urgence, et également en corrélation avec l'étude observationnelle pilote hollandaise, parue en avril 2017, qui précise que l'algorithme est proportionnellement davantage utilisé selon un degré d'urgence croissant.
