Le Groupe Plaies et Cicatrisation a souhaité que ce support, sous forme poster, soit présenté au 12ème Congrès national des Plaies et Cicatrisations à Paris. Cela a permis de montrer le travail réalisé au Centre Hospitalier de Cholet, dans le but de permettre à d'autres établissements de santé de s'inspirer des outils réalisés et de les appliquer à leur propre fonctionnement.

Les progrés concernant la compréhension de leurs mécanismes physiopathologiques ont permis d'améliorer leur prise en charge et de les inscrire dans une approche multidisciplinaire. Dans cette prise en charge globale, le choix des traitements locaux, notamment des pansements, est un élément essentiel. Il est établi depuis les années 1960 (suite aux travaux de Winter puis Hinman et Maibach) que la cicatrisation en milieu humide, obtenue en utilisant les pansements modernes est plus efficaces que les thérapeutiques sèches des pansements classiques. Les progrès du marché des pansements pour plaies chroniques basés sur ce principe n'ont pas été accompagnés d'une actualisation de la formation des médecins. Les information sotn alors délivrées par les délégués médicaux de chaque laboratoire, qui vantent les mérites de leur propres produits. Les médecins se trouvent alors en difficultés, confrontés à un très grand nombre de pansements aux propriétés et aux galéniques très diverses. L'analyse des pratiques des médecins du centre hospitalier d'Arles par questionnaires d'auto-évaluation retrouve de nombreux points à améliorer : l'usage encore trop fréquent des antiseptiques, bien que leur toxicité sur les acteurs de la cicatrisation a été démontrée ; l'amélioration de la prise en charge de la douleur par un recours plus fréquent aux anesthésiques locaux, dont l'efficacité et la bonne tolérance clinique ont été établies ; l'optimisation de l'utilisation des pansements à chaque stade de la cicatrisation ; la délégation trop fréquente des soins locaux aux IDE dont la formation n'est peut être pas plus développée que celle des médecins. Pour palier à ces difficultés, il a été proposé la mise en place d'une équipe pluridisciplinaire spécialisée en plaies et cicatrisation qui, en plus des conseils pour le choix des soins locaux, pourrait organiser des formations pour l'ensemble des soignants avec des mises à jour sur les recommandations et les thérapeutiques adjuvantes plus modernes (thérapie par pression négative, oxygénothérapie, hyperbare...). Nous avons également proposé un moyen simple, rapide à mettre en place et peu onéreux pour l'aide au choix des pansements, sous la forme d'un guide pratique.

The dating of this manuscript should be accepted with considerable reserve; it bears a clearly legible "1474," although it seems most unlikely that any reference to an Order occurred before the Bull of 1493 or that any Rule appeared in French before the Founder's visit to Louis XI in 1483. 3 The fame of Francis and his reputation as a "guerisseur" had reached the French court where Louis XI was sick and dying; the King summoned him to the chateau of Le Plessis-Ies-Tours, but it required the intervention of the Pope to make the hermit undertake the journey
