It reviews both the conceptual history of progress and the social and political experiences with progress over the past two centuries, and it comes to a surprising conclusion: The idea of progress was misconceived from its beginnings, and the failure of progress in practice was a result of this flawed conception. The experiences of the past half century, in turn, has allowed us to rethink progress in a more adequate way. Rather than the end of progress, they may herald the beginning of a new, reconstructed idea of progress.

Un questionnaire a été diffusé aux représentants locaux des IMG des 35 départements de médecine générale (DMG). Les participants étaient sondés sur : 1) l'existence d'une formation théorique à la psychiatrie, son volume horaire, le contenu et les modalités d'enseignement ; 2) l'existence de stages en psychiatrie, leur nombre et leur type. Un enseignement théorique est proposé dans 29 DMG (83 °/o). Son volume horaire moyen est de 10,6 heures sur les trois années d'internat. Les thématiques les plus abordées sont : addiction et dépendance (77 °/o des 35 DMG), souffrance psychique ( 49 °/o), trouble dépressif caractérisé et troubles anxieux ( 46 °/o). Les médecins généralistes enseignants sont les intervenants principaux (80 °/o). La totalité des DMG propose au moins un stage de psychiatrie, surtout en milieu intra-hospitalier (94 °/o). La formation à la psychiatrie des IMG, hétérogène, gagnerait à être modernisée en proposant de nouveaux outils pédagogiques, des enseignements par des spécialistes psychiatres et davantage 'de stages ambulatoires.

Méthodes - Nous avons réalisé une étude qualitative en interrogeant dix internes de médecine générale. Les données recueillies ont été organisées avec le logiciel Nvivo 12 puis analysées en utilisant la méthode de l'analyse thématique de discours. Résultats - Huit thèmes principaux sont retrouvés : la complexité de la psychiatrie, une vision dichotomique des troubles psychiques, des ressentis variés, un rôle central du médecin généraliste, l'importance de la collaboration entre professionnels, un apprentissage essentiellement pratique, une formation théorique manquante et des suggestions d'amélioration nombreuses. Ils sont répartis à l'intérieur de deux axes thématiques : les expériences vécues et le vécu des acquisitions de compétences en psychiatrie. Conclusion - Le développement d'offres de stages de psychiatrie en ambulatoire et des enseignements spécifiques menés par des professionnels de la santé mentale en collaboration avec ceux de médecine générale pourraient améliorer la prise en charge des patients présentant un trouble psychique.
