Book excerpt: When the external Quinton-Scribner arteriovenous shunt was developed in 1960, and, a little later, the internal Brescia-Cimino arteriovenous fistula was developed as a vascular access for hemodialysis, thereby making possible regular dialysis therapy of chronic uremic patients, many nephrologists became surgeons, having learned the type of vascular surgery related to hemodialysis quite well. The same series of events occurred with regards to peritoneal dialysis with the introduction of the Tenckhoff catheter and the need for gaining a permanent access to the peritoneum for chronic ambulatory peritoneal dialysis (CAPD) therapy. With time, however, problems relating to vascular and peritoneal access have forced many nephrologists to give up their surgery; meanwhile, many surgeons have become quite expert in some sophisticated techniques relating to dial ysis (e. g., vessel grafting, prosthesis implantation, etc.). Today, whether or not involved in this type of surgery, both nephrologists and surgeons remain interested in knowing all available access devices for dialysis as well as the surgical techniques involved. However, all nephrologists involved in dialysis must know how to prevent or treat complications related to dialysis access. Thus, it appeared to me to be quite advisable to have a book in my series, Topics in Renal Medicine, dealing with vascular and peritoneal access for dialysis.

A number of them are interested in problems of renal insufficiency and kidney replacement therapy. Therefore, many vital meetings concerning this vibrant speciality have been included in international pediatric congresses. But the discussion of ten-minutes-papers in a large auditorium was often limited, criticism tended to be muted, lively discussions have only occasionally succeeded for the reason that the topics have tended to be too specialized for the audiences in relation to the large number of other discussions about questions and results in pedeatric science. In Cologne, therefore, we decided to hold a conference concerning problems, advances and controversies in renal insufficiency in children, which would have a limited but general audience in rooms of a modest size, topics that tended to polarize a world-wide faculty of experts who liked their basic views to be opposed. The session would be chaired by persons well-known for inciting discussion and even difficult questions in order to stimulate a vital debate between delegates and audiences. One important part of the meeting was therefore the discussion in an open and familiar way as it is known in other workshops. We called the conference "KaIner Kinderdialyse-Symposium". The first conference was held in November 1975, and evoked an extra ordinary enthusiastic response from both audiences and delegates, so we held a second one in 1978 and the third meeting was in 1981.

Book excerpt: The writing of this book was prompted by the need for a comprehensive of current data on organic acids suitable for both newcomers and collection established researchers in this field. The only previous text of the kind was the excellent review by Nordmann and Nordmann (1961), and at that time the main method of analysis was paper chromatography with liquid chromatography being used in a limited way. Only three diseases in which organic acids accumulate were known (primary hyperoxaluria, phenylketonuria and alcaptonuria). Since then, with the development of gas chromatography and mass spectrometry, and the further development ofliquid chromatography, knowledge concerning the nature of the organic acids in physiological fluids has been greatly extended. At the same time, the number of organic acidurias has increased dramatically, there being now some 40-50 known diseases of this type. During the past 15 years or so, there have been several reviews, dealing with either specific diseases or groups of diseases (Gompertz, 1972, 1974; Tanaka, 1975), or presenting the proceedings of symposia (Stern and Toothill, 1972) or workshops (Marner et al. , 1974). This present text deals comprehensively and in detail with the organic acids in human physiological fluids in health and in disease states, and is particularly concerned with the methods necessary for their separation, determination and indentification.
