[...] Le vieillissement de la population et l'incidence grandissante des maladies chroniques, les courts séjours et l'apparition de nouvelles technologies constituent autant de facteurs qui obligent une grande majorité de la clientèle des services de santé à solliciter des services auprès de deux ou plusieurs organismes de santé à vocation différente. [...] Recours à une typologie de collaboration et à un outil de diagnostic L'analyse des modèles de continuité des soins a permis de dépasser les modèles initiaux pour aller vers une typologie de collaboration qui permet d'analyser, à partir de données empiriques, les processus de collaboration et d'identifier les écarts entre un modèle existant et un modèle souhaitable. [...] Le cadre de référence mis au point aux fins de la présente recherche se fonde sur les travaux de Alter et Hage9, de Provan et Milward10 et de Shortell, Gillies et Anderson11 en ce qui a trait aux variables liées à l'intégration des services, et aux travaux de D'Amour12 en ce qui a trait au processus de collaboration interprofessionnelle et interorganisationnelle (annexe 1). [...] Ce type de collaboration permet de mettre en place des services de santé où le partage des responsabilités entre les établissements fait l'objet d'un consensus et où les services sont basés sur des ententes de continuité des soins et sur des principes d'efficience.
