It argues that Freud's 1911 Schreber text reversed the order of priority in late nineteenth-century conceptions of the disposing causes of psychosis - the objective-biological and subjective-biographical - to privilege subjective disposition to psychosis, but without returning to the paradigms of early nineteenth-century Romantic psychiatry and without obviating the legitimate claims of biological psychiatry in relation to hereditary disposition. While Schreber is the book's reference point, this is not a general treatment of Schreber, or of Freud's reading of the Schreber case. It focuses rather on what was new in Freud's thinking on the disposition to psychosis, what he learned from his psychiatrist contemporaries and what he did not, and whether or not psychoanalysts have fully received his aetiology.

Reilly argues that modernist texts can be understood as critically responding to objective scientific models of the psyche, not simply illustrating their findings. Modernist works written in industrializing Central and Eastern Europe historicize the representation of consciousness as a quantifiable phenomenon within techno-scientific modernity. Looking beyond modernism’s well-studied relationship to psychoanalysis, this book tells the story of the non-Freudian vocabulary for mental illnesses that forms the precursor to today’s Diagnostic and Statistical Manual of Mental Disorders. Developed by the German psychiatrist Emil Kraepelin in the 1890s, this psychiatric taxonomy grew from the claim that invisible mental illnesses were analogous to physical phenomena in the natural world. Reilly explores how figures such as Georg Büchner, Ernst Toller, Daniel Paul Schreber, Nikolai Evreinov, Vsevolod Ivanov, and Santiago Ramón y Cajal understood the legal and political consequences of representing mental life in physical terms. Around 85% of its victims, predominantly children, adolescents and younger adults, survived the acute disorder, but most developed severe neurological syndromes, particularly severe post-encephalitic parkinsonism and other severe motor abnormalities, that incapacitated them for the remainder of their lives. Despite its brief history, encephalitis lethargica played a major role in a variety medical discussions between the two World Wars, as this epitome of neuropsychiatric disease – attacking both motor and mental functions – appeared just as the separation of neurology and psychiatry had reached a critical point. Encephalitis lethargica sufferers presented an unprecedented combination of neurologic and psychiatric symptoms – including previously puzzling phenomena primarily associated with schizophrenia and hysteria, as well as behavioral changes and attention deficit disorders in children – that not only underscored the unity of mind and movement in the CNS, but also illuminated the critical role played by subcortical structures in consciousness and other higher mental functions that had formerly been associated with the soul and more recently presumed to be localized to the human cerebral cortex. Encephalitis lethargica exerted a greater influence on clinical and theoretic neuroscientific thought between the two World Wars than any other single disorder and had an enduring impact upon neurology and psychiatry.
