Book excerpt: Attention-deficit hyperactivity disorder (ADHD) is a common neurodevelopmental disorder, which is characterized by a dyad of symptom dimensions: inattentiveness and hyperactivity-impulsivity. In addition to these core symptoms, children with ADHD often experience co-occurring reading or mathematics disability, and this co-occurrence is primarily attributable to inattentiveness rather than hyperactivity-impulsivity. The present thesis aims to further our understanding of the aetiologies of inattentive and hyperactive-impulsive ADHD symptoms and their associations with reading and mathematics disability, using genetically sensitive methodology. Analyses in this thesis are based on data from a large U.K. population sample of twins from the Twins Early Development Study, who were assessed across middle childhood to early adolescence. -- The first part of this thesis examines associations between the inattentive and hyperactive-impulsive ADHD symptom dimensions. The genetic and environmental aetiologies of concurrent associations in early adolescence were first examined in a bivariate model. The ADHD dimensions were highly heritable and showed large genetic overlap in addition to moderate genetic specificity. There was no evidence for aetiological sex differences. Longitudinal associations across middle childhood to early adolescence were then examined in a cross-lagged model. Results revealed that hyperactivity-impulsivity significantly predicted later inattentiveness, but the converse prediction was non-significant. Moreover, genetic overlap of the ADHD symptom dimensions over time was due to enduring as well as newly-emerging genetic factors. -- The second part of this thesis examines associations between the ADHD symptom dimensions and reading. First, multivariate analyses were conducted on data in early adolescence.

Book excerpt: Background: Attention-deficit/hyperactivity disorder (ADHD) and posttraumatic stress disorder (PTSD) are comorbid disorders that negatively impact multiple areas of functioning. Little research has examined the association between ADHD and PTSD symptoms and the specific impact of these symptoms on impairment in youth. Investigating these comorbid disorders is critical to inform the development of prevention and intervention efforts for youth who are vulnerable to adverse psychosocial outcomes, such as trauma exposed youth with ADHD. Aims The present study aimed to clarify the relationship between ADHD and PTSD symptom severity in adolescents. The study also sought to investigate how ADHD and PTSD symptoms are independently associated with quality of life (QoL), functional impairments, and executive functioning deficits (EFDs). It was hypothesized that (1) PTSD symptom severity would positively correlate with ADHD symptom severity, (2) ADHD and PTSD symptom severity would each independently predict functional impairments and reduced QoL, and (3) ADHD symptom severity, as compared to PTSD symptom severity, would more strongly predict EFDs. Method Adolescents (ages 11-17) were recruited from medical settings, schools, and the community. Caregivers participated in a pre-screen to confirm their youth's study eligibility by completing an ADHD screener and reporting on their child's trauma history. Youth who screened positive for ADHD and had experienced a PTE were invited to participate in the study assessment. Trained graduate-level evaluators comprehensively assessed ADHD, PTSD, and other psychiatric disorders, and youth and caregivers completed measures of the youth's QoL, functional impairments, and EFDs. Results The final sample was comprised of 17 eligible youth. Results were only sufficiently powered to examine the first hypothesis. Clinician-rated ADHD inattentive (IA) symptom count and clinician-rated PTSD symptom severity were strongly correlated, Spearman's [rho] = .53, p = .030, 95% CI [.06, .80]. Clinician-rated ADHD hyperactive-impulsive (H-I) symptom count and clinician-rated PTSD symptom severity were weakly negative correlated, but not at a statistically significant level, Spearman's [rho] = -.11, p = .689, 95% CI [-.56, .40]. There was a weak, but not statistically significant, partial correlation between clinician-rated ADHD IA symptom count and clinician-rated PTSD symptom severity, Spearman's [rho] = .12, p = .740. ADHD IA and PTSD symptoms were negatively moderately to strongly correlated with QoL and weakly to strongly correlated with functional impairment. ADHD H-I symptoms were negligibly associated with QoL and weakly associated with functional impairment. ADHD IA, ADHD H-I, and PTSD symptom severity were associated with EFDs. ADHD IA symptoms tended to be more strongly associated with EFDs than ADHD H-I symptom count and PTSD symptom severity. Discussion The preliminary results suggest that additional monitoring of IA symptoms in PTE-exposed youth would enable clinicians to better identify those at-risk for ADHD and determine if ADHD-focused interventions may also be necessary. Additionally, results suggest that in PTE-adolescents with ADHD symptoms and impairment in multiple domains, initially targeting ADHD IA symptoms as part of treatment may reduce impairment at a faster rate than focusing on ADHD H-I symptoms as well. Lastly, clinicians evaluating PTE-exposed youth for ADHD would benefit from not only comprehensively ruling out PTSD, but also considering other disorders whose symptoms can present as those of ADHD (e.g., anxiety, depression). A study with a larger sample is needed to better understand the relationship between ADHD and PTSD and their specific impact on functioning.

ADHD adolescents, compared to non-ADHD adolescents, have higher rates of SUDs and display more severe substance involvement. However, recent studies have shown these relationships can be explained by full mediation through Conduct Disorder (CD). Yet, delinquent behaviors are common consequences of adolescent alcohol and drug involvement and no studies have examined the relationship between ADHD and SUD by differentiating CD behaviors predating or occurring independent of substance involvement from CD behaviors which occur within the context of substance use. In addition, previous studies examining the ADHD/SUD relationship have been conducted almost exclusively with male samples. A different pattern of associations may emerge among girls. Furthermore, there exists little research examining pathways between ADHD and SUD that are mediated through internalizing disorders. Likewise, other pathways between ADHD and SUD that merit further study include: moderation by CD and moderation by internalizing disorders. This study examined these pathways on a sample of 254 inpatient (54% female, Mean age = 15.8 years) substance-abusing adolescents with comorbid Axis I disorders. Path analyses were also examined separately for boys and girls. Inhibitory control is a cognitive mechanism that influences emotion regulation and is also often impaired in children with ADHD and/or ODD. The present study investigates the role of inhibitory control as a possible mediator in the association between emotion dysregulation and symptoms of ADHD and ODD. Standardized questionnaires and clinician-facilitated structured interviews were used to collect parent and teacher ratings of difficulties in a clinical sample of 297 children (M age = 8.24 (1.7); 79.3% male) with disruptive behaviour. Findings suggest that inhibitory control is a key cognitive mechanism that mediates the relationship between emotional and behavioural difficulties in children with disruptive behaviour. Consequently, emotion dysregulation and inhibitory control may be important targets in psychosocial interventions for children with ADHD and/or ODD symptoms.

An Investigation Into the Relationship Between Trait Emotional Intelligence and Affective Disorders in Adolescents

Book excerpt: The adolescent transition involves a number of changes that for many adolescents result in increased negativity, heightened emotional reactivity, and risk for internalizing symptoms. The purpose of the present set of studies was to examine the role that emotional awareness has in relation to emotion regulation and to subsequent mood symptomatology in adolescence. The first study tested the hypothesized relationship between emotional awareness and emotion regulation, specifically, that emotional awareness is a necessary precursor to adaptive regulation efforts. This study examined the direct and indirect effects of emotional awareness on internalizing symptoms by exploring the extent to which certain emotion regulation strategies influence this relationship. The purpose of the second and third studies was to examine the association between emotional awareness and emotion regulation as measured by changes across three emotional response domains: 1) physiological arousal, 2) self-reported experience, and 3) observed expression during an ecologically valid emotion elicitation task. Participants in these studies were identified as alexithymic or non-alexithymic. Alexithymia involves difficulty identifying and describing emotions and has been used to classify individuals with extremely low emotional awareness. The second study examined the effects of alexithymia on the three emotional response domains in adolescents. Because of the high prevalence rates of alexithymia during adolescence, it could be an important vulnerability factor to help explain increases in internalizing symptoms during the adolescent transition. The purpose of the third study was to examine how patterns of decoupling between physiological arousal, self-reported experience, and observed expression were related to depressive and anxiety symptoms in alexithymic individuals. Overall, findings confirmed that emotional awareness is an important precursor to adaptive emotion regulation efforts. In addition, emotion regulation emerged as a more proximal mechanism in the development of depressive and anxiety symptoms in individuals with low emotional awareness. At a theoretical level, this research has guided our understanding of emotional processing and regulation. At a practical level, results from this program of research will help guide intervention and treatment approaches for adolescents with low emotional awareness.
