Book excerpt: This dissertation investigated attitudes of direct care professionals regarding the sexual expression of adults with intellectual disability. Surveys were distributed to 41 direct care staff (24 male, 17 female) from a residential treatment facility responsible for providing support services to adults with intellectual disability. Two hypotheses were analyzed with both descriptive and qualitative statistics. Results supported the first hypothesis as a group direct care professionals believe in supporting adults with intellectual disability in their behavioral expression of sexuality. Results partially support the second hypothesis; certain personal characteristics of staff impact the level of support they are willing to provide. Length of employment and frequency of religious attendance were found to negatively correlate with the level of stated support while age was not found to be related. Client gender was significant for male participants' level of stated support but not for female participants. Strengths and weaknesses of this research study are discussed and suggestions for further research are provided.

Book excerpt: Why is the sexuality of people with intellectual disabilities often deemed “risky” or “inappropriate” by teachers, parents, support staff, medical professionals, judges, and the media? Should sexual citizenship depend on IQ? Confronting such questions head-on, Already Doing It exposes the “sexual ableism” that denies the reality of individuals who, despite the restrictions they face, actively make decisions about their sexual lives. Tracing the history of efforts in the United States to limit the sexual freedoms of such persons⎯using methods such as forced sterilization, invasive birth control, and gender-segregated living arrangements—Michael Gill demonstrates that these widespread practices stemmed from dominant views of disabled sexuality, not least the notion that intellectually disabled women are excessively sexual and fertile while their male counterparts are sexually predatory. Analyzing legal discourses, sex education materials, and news stories going back to the 1970s, he shows, for example, that the intense focus on “stranger danger” in sex education for intellectually disabled individuals disregards their ability to independently choose activities and sexual partners—including nonheterosexual ones, who are frequently treated with heightened suspicion. He also examines ethical issues surrounding masturbation training that aims to regulate individuals’ sexual lives, challenges the perception that those whose sexuality is controlled (or rejected) should not reproduce, and proposes recognition of the right to become parents for adults with intellectual disabilities. A powerfully argued call for sexual and reproductive justice for people with intellectual disabilities, Already Doing It urges a shift away from the compulsion to manage “deviance” (better known today as harm reduction) because the right to pleasure and intellectual disability are not mutually exclusive. In so doing, it represents a vital new contribution to the ongoing debate over who, in the United States, should be allowed to have sex, reproduce, marry, and raise children.

She argues the importance of informing the work of those responsible at research, practice and policy levels with the voices of people with learning disabilities. In the interviews, women talk openly about what form their sexual activity takes and what it means for them, the circumstances in which it occurs, and the pleasures (or lack thereof) associated with it. These interviews directly shape the policy and practice recommendations the author makes. Michelle McCarthy's findings suggest that women with learning disabilities commonly find themselves engaged in sexual activity which is not to their liking and not of their choosing. A high level of sexual abuse was also reported. The author discusses this in relation to the cultural forces which have influenced Western perceptions of sexuality, feminism and theories and prejudices about learning disabilities. She also studied the impact of institutional and community settings on the sexuality of women with learning disabilities. In Sexuality and Women with Learning Disabilities, McCarthy makes recommendations for policy and practice which will protect this vulnerable group, and advises on education, support and seeking justice for abused women.

Nevertheless, staff attitudes, and the institutional context, profoundly affect how sexuality is expressed. Staff act as gatekeepers to many social and recreational activities; clients are therefore unavoidably affected by staff attitudes and behaviors. This research offers an investigation of these attitudes. Sociological research on cognitive disabilities is very limited, and the study of sexuality and developmental disability is virtually nonexistent. Moreover, there have been no substantial investigations of sociological issues associated with the sexuality of people with more severe developmental disabilities. Social scientists who have investigated sexuality, focus exclusively on individuals with mild mental retardation. This paper bridges that gap by investigating the attitudes of 318 staff at California Developmental Centers towards the sexual behavior of people with developmental disabilities. The proportion of people at these centers who have disabilities that are significant is higher than at other types of care facilities. Many of the individuals also require medical supports and services. Staff provide training and assistance with activities of daily living including feeding, personal hygiene, transportation, recreation, job training, psychosocial support, and medical intervention. Due to the nature of the disabilities, a client's life at the developmental center involves high levels of personal intervention. Privacy is limited. This paper provides an overview of the developmental system and the institutional context; investigates the historical aspect of attitudes towards sexuality; discusses why this population presents a unique challenge to sociology; evaluates previous sexual attitude research; and presents results of a new attitudinal study. Policy implications are also discussed.

Clients are experts in their own recovery from SUDs. Counselors should engage them in collaborative partnerships. Ambivalence about change is normal. Resistance to change is an expression of ambivalence about change, not a client trait or characteristic. Confrontational approaches increase client resistance and discord in the counseling relationship. Motivational approaches explore ambivalence in a nonjudgmental and compassionate way.
