Book excerpt: Intimate Partner Violence (IPV), the most prevalent type of violence against women, has been recognized as a human rights violation by intergovernmental organizations and international law. Due to its physical and psychological sequelae, the World Health Organization has considered IPV a major global public health issue since 1997. Primary care health care professionals (HCPs) are considered especially well-positioned to identify women experiencing IPV, yet screening rates are low. Utilizing the Integrated Behavioral Model (IBM) and Intersectional Feminism as guiding theoretical frames, this mixed method study (qualitative, N = 35; quantitative, N = 176) explored Barbadian HCPs' attitudes, beliefs and knowledge regarding IPV screening, including personal and professional factors that facilitate or inhibit screening. Future screening intention and past screening behaviors were assessed. Findings from the study indicate that the IBM model explains a significant amount of variance in predicting screening intention, with the IBM construct of self-efficacy being the strongest theoretical predictor of screening intention. Overall, practitioners' personal experience of physical abuse was the strongest predictor of screening intention. Implications for social work include future IPV research using a community-based participatory research methodology; in terms of practice and education, increasing the number of medical social workers in primary care settings and public health social workers, capitalizing on social work's person-in-environment perspective to engage systems at multiple levels in addressing IPV. Policy recommendations are to engage in a chronic disease model of care and trauma-informed practice to empower women experiencing violence in primary care settings.

Book excerpt: "Purpose: (1) To identify barriers to screening for intimate partner violence (IPV) within the primary care setting, (2) to determine if formal adoption of an IPV screening policy is warranted and, (3) provide recommendations regarding agency wide IPV screening policy. Background: The COVID-19 pandemic is estimated to have globally increased IPV rates by 25%-33%. A positive history of IPV has been shown to be associated with poorer long-term health outcomes. Health care providers (HCPs) are often the first person to potentially screen a survivor of IPV. Methods: Participants recruitment occurred through the project sites internal communication network (e-mail). Participants were employed within a primary care network as an MA, RN, MD, DO, NP or PA. Participants were emailed access to an anonymous survey consisting of; the consent form, the survey tool (The Domestic Violence Healthcare Provider Survey [DVHCPS]), and a question regarding their job title. Four of the original six subscales of the DVHCPS were included: provider perceived efficacy, system support, role resistance/ fear of offending the patient, and frequency of IPV inquiry. Results: Sixteen people responded; 15 had complete. While all participants perceived they had efficacy in screening (M= 22.38; range 16 to 28), no subgroup strongly agreed. Respondents did not feel significantly supported by current systems. Prescribing providers felt the least supported (M = 5.75; range of 3-10) and MAs felt the most supported (M = 9.66; range 7 to 12). Prescribing providers were found to be the least confident in referral practice for perpetrators (M = 2.75; range 2 to 4) and survivors (M = 3.25; range 2 to 4) of IPV. Most agreed it was within their role to screen for IPV. However, respondents were unlikely to screen for IPV (M= 9.93; range 6 to 15). Conclusion: MAs can conduct routine IPV screen, similar to the clinical data collected as part of standard duties. Improving the training for screening and referrals and developing IPV resources for HCPs may increase screening rates"--Abstract.

The prevalence of intimate partner violence is on the higher end of this spectrum in East Africa, with in-country demographic and health surveys indicating that approximately half of all women between the ages of 15-49 in Uganda, Kenya, and Tanzania having experienced physical or sexual abuse within a partnership. It is now widely accepted that preventing intimate partner violence is possible and can be achieved through a greater understanding of the problem; its risk and protective factors; and effective evidence-informed primary, secondary, and tertiary prevention. The computer survey included questions on IPV and other health risks. Computer-generated risk reports were attached to patients' medical charts for review by their physician. Participants in the control arm received standard medical care. Data were collected by audio taping the medical visits and an exit survey of patients. The results reveal that the overall prevalence of IPV in the family practice was high; 14.6% in the pencil and paper survey and 13.7% in the RCT study. The RCT demonstrated that, compared to standard medical care, the computer-assisted screening resulted in a 1.8-fold increase in the likelihood of physician and patients discussing IPV risk and the detection of women at risk of IPV increased by a factor of 2.5-fold. Further, RCT participant patients showed acceptance of the computer-assisted screening, after its use. The interviewed physicians perceived computer-assisted screening as most feasible for periodic health exams and follow-up visits, given the existing logistical limitations. Overall, physicians expressed general acceptance of the computer screening. The results provide impetus for improvements in clinical practice, medical training, policy and research to address the issue of IPV. The use of the screening tool may be enhanced by continued research into its generalizability by inclusion of multiple family practice clinics and various primary care settings. The overall aim of the dissertation research was to advance knowledge on the phenomenon of computer-assisted screening for intimate partner violence (IPV) within a Canadian family practice context. A series of inter-linked studies (quantitative and qualitative) was conducted to answer three research questions: (1) what is the effectiveness of computer-assisted screening for increasing physician-patient discussions on and detection of IPV risk? (2) what is the acceptance of computer-assisted screening among patients? and (3) what is the feasibility of computer-assisted screening from the perspectives of family physicians? To answer the first two questions, a randomized controlled trial (RCT) was conducted at an academically-affiliated family practice clinic in Toronto with adult female patients in a current or recent intimate relationship (n = 314). The RCT planning was facilitated by studies in the pre-trial phase (a pencil and paper survey of patients, and a psychometric scale validation). To answer the third question, in-depth qualitative interviews were conducted with the trial physicians (n = 10).

Although the clinician's role in identification and intervention is considered a professional and legal responsibility, the effectiveness of these efforts is unclear. OBJECTIVE: To examine evidence on the performance of screening procedures and interventions in the primary care setting in reducing harm from family and intimate partner violence for children, women, and elderly adults. DATA SOURCES: MEDLINE(r), PsycINFO, CINAHL, Health & Psychosocial Instruments, ERIC, AARP Ageline, and the Cochrane Controlled Trials Register, reference lists of systematic reviews, and experts. STUDY SELECTION: Included studies had English-language abstracts, were applicable to U.S. clinical practice, described abuse and neglect in women, children, or elderly adults, were conducted in or linked to primary care, obstetrics/gynecology, or emergency department settings, and included a clinician in the process of assessment or intervention. DATA EXTRACTION: We extracted selected information about study design, patient samples and settings, methods of assessment or intervention, and clinical endpointsand applied a set of criteria to evaluate study quality. DATA SYNTHESIS: No studies directly addressed the effectiveness of screening in a healthcare setting in reducing harm, or described the adverse effects of screening and interventions. All instruments designed to screen for child abuse and neglect were directed to parents, particularly pregnant women. These had fairly high sensitivity but low specificity. Several brief instruments designed to identify women with intimate partner violence compared well to longer previously validated instruments. We found few studies of screening for elder abuse and neglect. A randomized controlled trial with 15-years follow-up indicated that nurse home visits during pregnancy and for 2-years postpartum for low-income women improved abuse and neglect outcomes for children. Studies of interventions for children of other ages, women who are not pregnant, and elderly adults are lacking. CONCLUSIONS: Screening and interventions for child abuse are directed to parents during prenatal and postpartum periods. Several brief screening instruments have been tested for women, but interventions are lacking. Few instruments and no interventions were identified for elderly adults.

Although 84% of women tell friends and family about IPV, only 21% tell doctors, nurses or other healthcare providers about it. A literature review revealed five themes related to barriers to screening for IPV: lack of time, lack of standardized protocols and tools, lack of knowledge, lack of privacy and provider discomfort. An adaptation of the Johari Window, a tool used for self-awareness, was used to examine these themes. This is a descriptive qualitative study where eleven providers in the tri-county area of Michigan were interviewed to determine their perceived barriers to screening for IPV. Demographic and interview questions were adapted from the literature. Data analysis was completed by first analyzing interviews for themes, then analyzing the themes in the context of the Johari Window. Interviews revealed barriers and facilitators to screening which fit into six themes: patient's unwillingness to disclose, lack of privacy, provider discomfort, lack of time, a lack of standardized protocols and tools, lack of knowledge. The patinpatient'singness to disclose was the most prevalent theme in this study and was not identified in previous studies. The themes were analyzed using the Johari Window framework. However, men and even children can also be victims of IPV. Victims of IPV typically experience physical, sexual, and/or emotional abuse that have lasting impacts. EBP Framework: The nursing theory used for this project was Kristen M. Swanson's Theory of Caring, and the evidence-based practice model followed was Steven's Star Model of Knowledge Transformation (SSMKT). Methods: The project aimed to improve providers' confidence and willingness to render IPV screenings to adult patients. Outcomes were measured using the Likert scale questionnaire administered to participating providers before and after project implementation. Data from the questionnaires were evaluated using the Two-Tailed Wilcoxon Signed Rank Test. The HARK screening tool results were assessed for the number of positive vs. negative results. Participation in this project was voluntary for the healthcare clinicians, providers, and patients. No personal identifiers were utilized. Findings/Results: Project results demonstrated insignificance as none of the p-values for all eight questions completed by the participating providers were less than or equal to the designated alpha value. Thus, random variation can explain the testing results. Project findings were managed and disseminated during the final process outcome evaluation phase. Data was gathered, calculated, and discussed with the leadership of Tanner Clinic - Westside. Further discussion regarding process development, implementation, and aggregation of this screening tool to standard provider practice occurred. Conclusion/Recommendations: The upsurge in IPV incidence is an unfortunate and often obscure bi-product of the COVID-19 pandemic. Healthcare providers of all specialities, and more so those in family practice, must regularly screen their patients for IPV and take necessary action. The HARK screening tool is evidence-based, proven accurate, and simple for patients to complete, making it an appropriate screening tool for healthcare professionals to utilize in their daily practice.
