Book excerpt: In the 1980's a primary focus for intense cardiovascular research is in the treatment of patients with acute myocardial infarction. Although the prevalence of this syndrome has been decreasing in the United States, still over 1.5 million patients develop myocardial infarction per year. There is about a 20% chance of a North American male developing myocardial in farction before the age of 65. The in-hospital mortality still remains at ap proximately 10070-15070 and advances in pharmacologic and device therapy have allowed for the intensification of research in the treatment of patients with acute myocardial infarction. The following manuscripts represent the collective efforts of academic in vestigators in the United States and abroad as well as members of the phar maceutical industry, and the Food and Drug Administration to address the issues involved in interventions in the acute phase of myocardial infarction. State-of-the-art papers addressing important topics are followed by discus sion sections which have allowed participants to express their own viewpoints leading to a consensus opinion. The first part of this Symposium addresses the models of experimental myocardial infarction followed by the important issue of how one defines myocardial infarction size. The latter is extremely important to be certain that endpoints of therapeutic or device interventions are objective and reproducible. A detailed description of the pharmacological interventions to reduce myocardial infarction size as well as newer devices to effect mechanical and electrical disorders provide an up-to-date summary of current opinion.

Book excerpt: Although there are many books on ischemic heart disease and myocardial in farction, very few relate to the acute phase of the illness. Pre-hospital coronary care units have been operational for over a decade. In 1975 the first book detailing the operation and results of the out-of-hospital Belfast Mobile Coronary Care Unit was published (The Acute Coronary Attack - Pitman Medical). Nevertheless, deaths due to coronary artery disease still remain a major challenge in contemporary society. Sudden death is largely an out-of-hospital problem. Since ventricular fibril lation is an electrical accident and can be readily corrected when a limited area of the ventricle is depolarized, methods for its containment are essential. The purpose of this book is to project the recent advances in the acute phase of ischemic heart disease. The early chapters document the history and recent develop ments in the understanding of ventricular fibrillation, ventricular defibrillation and cardiopulmonary resuscitation. The etiology of ventricular arrhythmias in the acute phase of myocardial ischemia in experimental animals is discussed with particular reference to the antiarrhythmic action of drugs. Factors observed in the initiation of ventricular fibrillation in patients outside the hospital are reviewed. The practical applications both in the field of biomedical engineering as applied to mobile coro nary care and in the approach and impact of pre-hospital coronary care in the various communities in the United States of America, Canada, United Kingdom, and other countries in Europe are presented.

Book excerpt: Coronary Microvascular Obstruction in Acute Myocardial Infarction: From Mechanisms to Treatment provides a comprehensive understanding of the phenomenon of coronary microvascular obstruction (CMVO) that is the main limitation of reperfusion therapies in ST-elevation myocardial infarction. It provides in-depth coverage of the phenomenon of CMVO which heavily affects prognosis by increasing the risk of death and heart failure at follow-up. A first of its kind reference dedicated solely to this topic, it is appropriate for a wide audience, from researchers, to those who aid in the management, prevention and treatment of CMVO. Provides in-depth coverage of coronary microvascular obstruction (CMVO), spanning research, management, prevention and treatment Includes the most up-to-date information on CMVO as presented from top experts around the word Provides access to a companion website with extra material, including tables, additional references and instructional videos Gives extensive coverage on how to measure CMVO, including in-depth indexes that can be used to detect and quantify the phenomenon

Book excerpt: This book is open access under a CC BY 4.0 license. This quick-reference handbook offers a concise and practical review of key aspects of the treatment of ST-segment elevation myocardial infarction (STEMI) in the era of primary percutaneous coronary intervention (PPCI). In the context of STEMI, PPCI is the preferred mode of emergency revascularization. Access to PPCI is rapidly increasing and is now routinely practiced in both general and specialist hospitals and there has been a recent emphasis on developing STEMI networks to enhance and expedite the referral pathway. This coupled with concurrent developments to enhance the safety and efficacy of the PPCI procedure has heralded an era where STEMI interventions are increasingly considered an important subspecialty within interventional cardiology. Written by leading cardiologists who have been instrumental in the adoption of PPCI in their respective institutions, the book provides junior and senior cardiologists alike with insightful and thought-provoking tips and tricks to enhance the success of PPCI procedures, which may in turn translate into direct improvements in outcomes. The book is also relevant for healthcare providers and emergency department physicians.

Book excerpt: Our understanding of the pathophysiology of acute myocardial infarction has grown enormously in recent years. This has led to an increasingly aggressive approach to management, designed to blunt the extent of infarction by salvaging acutely ischemic myocardium. Alternatives now include throm bolysis, PTCA with and without prior thrombolysis, and emergency bypass surgery, as well as the more aggressive use of a variety of drugs. This book consists of a series of chapters by experienced cardiologists and cardiovascular surgeons that present today's state of the art in managing acute myocardial infarction. It is written with the purpose of presenting practical approaches of value to the clinician related to the more complex problems faced in dealing with patients undergoing myocardial infarction. xi EARLY INTERVENTIONS IN ACUTE MYOCARDIAL INFARCTION 1. MANAGEMENT OF ARRHYTHMIAS IN THE CORONARY CARE UNIT NORA GOLDSCHLAGER, RODERICK WOODS, AND NEAL BENOWITZ With the advent of coronary care units, mortality from cardiac arrhythmias occurring during acute myocardial infarction has been unquestionably and dramatically reduced. In addition, continuous electrocardiographic monitoring has resulted in the recognition of specific arrhythmias related to acute ischemic heart disease and thus to the development of appropriate strategies for their management. This chapter will review certain specific aspects of those bradyarrhythmias and atrial and ventricular tachyarrhythmias that are seen most commonly in acute myocardial infarction. Pharmacologic therapy, including the use of some of the newer antiarrhythmic agents, will be discussed, as will pacemaker ther apy of both bradyarrhythmias and tachyarrhythmias.

Book excerpt: Cardiovascular, respiratory, and related conditions cause more than 40 percent of all deaths globally, and their substantial burden is rising, particularly in low- and middle-income countries (LMICs). Their burden extends well beyond health effects to include significant economic and societal consequences. Most of these conditions are related, share risk factors, and have common control measures at the clinical, population, and policy levels. Lives can be extended and improved when these diseases are prevented, detected, and managed. This volume summarizes current knowledge and presents evidence-based interventions that are effective, cost-effective, and scalable in LMICs.

Book excerpt: The management of patients who present with a myocardial i nfarcti on has altered radi ca lly over the past two decades. The expansion of knowledge relating to the epidemiology of the condition together with a greater understanding of the causes of the early mortality from it have resulted in major changes in the way these pati ents are treated duri ng the acute phase of the illness. The deve 1 opment of 'dedi cated' Coronary Care Units in hospitals and the recogni ti on of the need for ' Mobile Coronary Care Units' in the cOl1Ul1unity have made a major impact upon the early mortality from myocardial infarction. Over the past decade, a great deal of attention has been paid to strategies designed to limit the size of a myocardial infarct. As an extensi on of thi s approach, the 1980' s have seen the evo 1 uti on of techniques for revascularization of ischemic tissue. All these procedures while appearing to hold the promise of reducing the acute mortality from myocardial infarction, create the need for a planned approach to the long term management of these patients. While there are some modest i ndi cati ons that coronary atherosc 1 erosi s coul d be arrested or even reversed, for all practical purposes it remains a chroni c progressi ve di sease.
