Book excerpt: Les facteurs biologiques sont les principales limitations de la durée des traitements orthodontiques. La durée d'un traitement classique est d'environ deux ans. Elle peut être un obstacle chez les adultes, de plus en plus demandeurs, sans compter l'incidence des problèmes carieux et parodontaux qui augmentent avec cette durée. D'après la littérature, la corticotomie alvéolaire semble être une technique fiable accélérant les déplacements dentaires en orthodontie. Cette technique consiste à effectuer des résections superficielles de l'os alvéolaire pénétrant à peine l'os médullaire dans la zone où un déplacement dentaire est souhaité. Elle met en jeu les mécanismes de la cicatrisation osseuse naturelle permettant un phénomène d'accélération régionale du mouvement. Le but de ce travail est, d'une part, de décrire les différentes techniques opératoires de réalisation d'une corticomie en développant plus particulièrement la technique de piézocision. D'autres parts, nous étudierons les effets biologiques de la corticomie sur le tissu osseux permettant cette accélération des mouvements dentaires. Enfin, nous illustrerons ce travail par la présentation de deux cas cliniques.
