Accumulated big data are now estimated to be 4.4 zettabytes in the digital universe; and trends predict an exponential increase in the future. Health data are gathered from professional routine care and other expanded sources including the social determinants of health, such as Internet of Things. Biomedical research has recently moved through three stages in digital healthcare: (1) data collection; (2) data sharing; and (3) data analytics. With the explosion of stored health data, dental medicine is edging into its fourth stage of digitization using new technologies including augmented and virtual reality, artificial intelligence, and blockchain. Big data collaborations involve interactions between a diverse range of stakeholders with analytical, technical and political focus. In oral healthcare, data technology has many areas of application: prognostic analysis and predictive modeling, the identification of unknown correlations of diseases, clinical decision support for novel treatment concepts, public health surveys and population-based clinical research, as well as the evaluation of healthcare systems. The objective of this Special Issue is to provide an update on the current knowledge with state-of-the-art theory and practical information on human and social perspectives that determine the uptake of technological innovations in big data science in the field of dental medicine. Moreover, it will focus on the identification of future research needs to manage the continuous increase in health data and to accomplish its clinical translation for patient-centered research and personalized dentistry. Poor and minority children are less likely to have access to oral health care than are their nonpoor and nonminority peers. Older adults, people who live in rural areas, and disabled individuals, uniformly confront access barriers, regardless of their financial resources. The consequences of these disparities in access to oral health care can lead to a number of conditions including malnutrition, childhood speech problems, infections, diabetes, heart disease, and premature births. Improving Access to Oral Health Care for Vulnerable and Underserved Populations examines the scope and consequences of inadequate access to oral health services in the United States and recommends ways to combat the economic, structural, geographic, and cultural factors that prevent access to regular, quality care. The report suggests changing funding and reimbursement for dental care; expanding the oral health work force by training doctors, nurses, and other nondental professionals to recognize risk for oral diseases; and revamping regulatory, educational, and administrative practices. It also recommends changes to incorporate oral health care into overall health care. These recommendations support the creation of a diverse workforce that is competent, compensated, and authorized to serve vulnerable and underserved populations across the life cycle. For each service or condition examined, the committee commissioned a review of the scientific literature that was presented and discussed at a public workshop. As requested by Congress, this report includes explicit estimates only of costs to Medicare, not costs to beneficiaries, their families, or others. It also does not include cost-effectiveness analyses. That is, the extent of the benefits relative to the costs to Medicareâ€"or to society generallyâ€"is not evaluated for the services examined. The method for estimating Medicare costs follows the generic estimation practices of the Congressional Budget Office (CBO). The objective was to provide Congress with estimates that were based on familiar procedures and could be compared readily with earlier and later CBO estimates. For each condition or service, the estimates are intended to suggest the order of magnitude of the costs to Medicare of extending coverage, but the estimates could be considerably higher or lower than what Medicare might actually spend were coverage policies changed. The estimates cover the five-year period 2000-2004. In addition to the conclusions about specific coverage issues, the report examines some broader concerns about the processes for making coverage decisions and about the research and organizational infrastructure for these decisions. It also briefly examines the limits of coverage as a means of improving health services and outcomes and the limits of evidence as a means of resolving policy and ethical questions.

In February 1840, the Baltimore College of Dental Surgery, the Dental School of the University of Maryland, was chartered by the Maryland General Assembly as the world's first dental school. In the same year the American Society of Dental Surgeons, the antecedent of the present day American Dental Association, was founded, also in Baltimore. In the previous year, 1839, the American Journal of Dental Science was initiated as the first periodic scientific and professional publication in dentistry, later evolving to the Journal of the American Dental Association. With the congruence of three fundamental elements which are essential to any profession--a unique program of education, a formal means to communicate and freely share new information, and an organization devoted to maintenance of professional ethics and standards through self-regulation--dentistry began evolving to its current status as a valued and respected health profession. From its birth and through the intervening century and a half, dentistry has been a profession heavily reliant on technology as well as science. Dentists variously are credited with the discovery and development of general anesthesia and the precision casting technique; and they make significant use of rotary cutting instruments, ultrasonics, laser technology, unique biomaterials, and intraosseous implants, to mention only a few techniques.
