Book excerpt: Dans les pays développés, on assiste à une augmentation de l'incidence de l'insuffisance rénale chronique terminale liée au vieillissement, au diabète et à l'hypertension artérielle. Le coût considérable de la prise en charge de l'insuffisance rénale chronique est donc un enjeu majeur de santé publique en France. Toutes les études s'accordent à dire qu'il existe un réel défaut de diagnostic et de prise en charge de l'insuffisance rénale chronique en médecine générale. Les dernières recommandations françaises sur le sujet datent d'ailleurs de 2012. L'objectif de ce travail est d'établir un catalogue actualisé de néphrologie pratique à l'attention du médecin généraliste, pour lui permettre une prise en charge optimale des patients à « risque néphrologique ». Un total de cent quarante-sept publications et documents issus de la littérature sur le dépistage, le diagnostic, le suivi et la prise en charge de l'insuffisance rénale chronique en médecine ambulatoire ont été étudiés au cours de ce travail. Nous avons ainsi élaboré 14 fiches synthétiques, traitant des sujets les plus fréquemment rencontrés en médecine générale lors de la prise en charge des patients à « risque néphrologique ». Ce travail souligne les spécificités propres aux patients à « risque néphrologique », qui doivent être prises en compte par le généraliste. Il devrait maintenant être diffusé aux praticiens de ville dans un soucis d'amélioration des pratiques et pour une meilleure collaboration entre généralistes et néphrologues. Dans un second temps, une étude d'impact pourrait être conduite afin d'évaluer les modifications de pratique liées à ces travaux.

Les personnes âgées de plus de 75 ans représentent 9% de la population française sur la pyramide des âges. Ce pourcentage va s’accentuer au fil des années. Il nous paraissait donc intéressant de savoir comment était prise en charge, en soins primaires, l’insuffisance rénale chronique chez les personnes âgées.Nous avons donc réalisé une étude descriptive et rétrospective des pratiques du dépistage de l’insuffisance rénale chronique par les médecins généralistes en médecine ambulatoire.Nous avons retrouvé que ce dépistage, même si il n’est pas clairement préconisé par les dernières recommandations de la HAS, est réalisé par 82% de nos médecins interrogés, mais que seulement 45% de ceux-ci connaissent ces recommandations qui ont été publiées en 2002. La situation va vraisemblablement s’améliorer dans les années à venir grâce à de nombreux plans de prévention lancés à l’échelon national, suite aux recommandations de la HAS, comme le Plan de Prévention IRC et la mise en place de réseaux régionaux de prise en charge des insuffisants rénaux chroniques, permettant ainsi une prise en charge coordonnée entre le néphrologue et le médecin généraliste.Nous avons ensuite fait une mise au point sur la spécificité du dépistage chez la personne âgée et sur la formule la plus adaptée à la personne âgée qui, selon les études les plus récentes, resterait la formule de Cockcroft et Gault.

Ses facteurs de risque principaux étant identifiés et sa prise en charge précoce reconnue efficace pour ralentir sa progression, des recommandations ont été émises en 2002 puis 2009 afin de dépister les patients à risque. Mais les médecins généralistes, en 1ère ligne dans le suivi de ces patients dépistent-ils la MRC ? Nous avons réalisé un audit de pratique qui a consisté pour 21 médecins d’Ile de France à inclure 20 patients chacun, connus pour être diabétiques, hypertendus et/ou de plus de 65 ans et à remplir pour chaque patient un questionnaire. 406 patients ont été inclus de janvier à mai 2011. 83 % d’entre eux avaient bénéficié d’un dosage de la créatininémie dans l’année. La clairance n’était calculée que dans 77% des cas, et l’usage de l’équation de Cockroft et Gault était largement prédominante malgré les recommandations de 2009 d’utiliser le MDRD.Seulement 55% des patients diabétiques avaient eu une protéinurie dans l’année et 49 % des patients hypertendus non diabétiques dans les 5 ans. 64% des protéinuries étaient des concentrations sur échantillon, 26.6% un dosage sur urines des 24h, et 4.6 % des rapports albuminurie/créatininurie sur échantillon (technique recommandée). La prévalence de l’insuffisance rénale chronique avérée était de 11,8%, et de 23.1 % pour la Maladie Rénale Chronique tous stades confondus. A la lecture de leurs résultats individuels et des nouvelles recommandations émises en 2011 et 2012, les 21 médecins audités ont des indications pour améliorer leurs pratiques ; améliorations qui pourront être mesurées par un deuxième tour d'audit.

In this classic of French cultural studies, Georges Didi-Huberman traces the intimate and reciprocal relationship between the disciplines of psychiatry and photography in the late nineteenth century. Focusing on the immense photographic output of the Salpetriere hospital, the notorious Parisian asylum for insane and incurable women, Didi-Huberman shows the crucial role played by photography in the invention of the category of hysteria. Under the direction of the medical teacher and clinician Jean-Martin Charcot, the inmates of Salpetriere identified as hysterics were methodically photographed, providing skeptical colleagues with visual proof of hysteria's specific form. These images, many of which appear in this book, provided the materials for the multivolume album Iconographie photographique de la Salpetriere. As Didi-Huberman shows, these photographs were far from simply objective documentation. The subjects were required to portray their hysterical "type"—they performed their own hysteria. Bribed by the special status they enjoyed in the purgatory of experimentation and threatened with transfer back to the inferno of the incurables, the women patiently posed for the photographs and submitted to presentations of hysterical attacks before the crowds that gathered for Charcot's "Tuesday Lectures." Charcot did not stop at voyeuristic observation. Through techniques such as hypnosis, electroshock therapy, and genital manipulation, he instigated the hysterical symptoms in his patients, eventually giving rise to hatred and resistance on their part. Didi-Huberman follows this path from complicity to antipathy in one of Charcot's favorite "cases," that of Augustine, whose image crops up again and again in the Iconographie. Augustine's virtuosic performance of hysteria ultimately became one of self-sacrifice, seen in pictures of ecstasy, crucifixion, and silent cries.

Oxygen therapy remains an inaccessible luxury for a large proportion of severely ill children admitted to hospitals in developing countries. This is particularly true for patients in small district hospitals, where, even if some facility for delivering oxygen is available, supplies are often unreliable and the benefits of treatment may be diminished by poorly maintained, inappropriate equipment or poorly trained staff with inadequate guidelines. Increasing awareness of these problems is likely to have considerable clinical and public health benefits in the care of severely ill children. Health workers should be able to know the clinical signs that suggest the presence of hypoxaemia and have more reliable means of detection of hypoxeamia. This can be achieved through more widespread use of pulse oximetry, which is a non-invasive measure of arterial oxygen saturation. At the same time oxygen therapy must be more widely available; in many remote settings, this can be achieved by use of oxygen concentrators, which can run on regular or alternative sources of power. Having effective systems for the detection and management of hypoxaemia are vital in reducing mortality from pneumonia and other severe acute illnesses. Oxygen therapy is essential to counter hypoxaemia and many times is the difference between life and death. This manual focuses on the availability and clinical use of oxygen therapy in children in health facilities by providing the practical aspects for health workers, biomedical engineers, and administrators. It addresses the need for appropriate detection of hypoxaemia, use of pulse oximetry, clinical use of oxygen and delivery systems and monitoring of patients on oxygen therapy.
