It's a major transition in anyone's life and change of this magnitude often arouses anxiety. This is much more so for people with disability, particularly intellectual disability. But, as this manual shows, it doesn't have to be like that. The Transition to Retirement (TTR) program has been developed in response to a genuine problem: the need for an effective approach to supporting older employees to build an active, socially inclusive lifestyle after retirement. The approach mapped out in this manual may not be the solution for all workers with disability, but it will certainly assist quite a few. The TTR program emphasises social inclusion. It is consistent with the focus of the National Disability Insurance Scheme (NDIS) on building community participation and independence. It is also consistent with the National Disability Strategy's emphasis on promoting social inclusion in mainstream community settings and service systems. The TTR program supports ageing people with disability to develop new interests, skills and social networks, and facilitates their participation in mainstream community groups. The problem has elicited a variety of societal and public policy responses over the years, concomitant with fluctuations in the economy and changes in the demographics of and attitudes toward poor and disenfranchised citizens. In recent decades, federal agencies, nonprofit organizations, and the philanthropic community have worked hard to develop and implement programs to solve the challenges of homelessness, and progress has been made. However, much more remains to be done. Importantly, the results of various efforts, and especially the efforts to reduce homelessness among veterans in recent years, have shown that the problem of homelessness can be successfully addressed. Although a number of programs have been developed to meet the needs of persons experiencing homelessness, this report focuses on one particular type of intervention: permanent supportive housing (PSH). Permanent Supportive Housing focuses on the impact of PSH on health care outcomes and its cost-effectiveness. The report also addresses policy and program barriers that affect the ability to bring the PSH and other housing models to scale to address housing and health care needs.

Book excerpt: The U.S. population of older adults is predicted to grow rapidly as "baby boomers" (those born between 1946 and 1964) begin to reach 65 years of age. Simultaneously, advancements in medical care and improved awareness of healthy lifestyles have led to longer life expectancies. The Census Bureau projects that the population of Americans 65 years of age and older will rise from approximately 40 million in 2010 to 55 million in 2020, a 36 percent increase. Furthermore, older adults are choosing to live independently in the community setting rather than residing in an institutional environment. Furthermore, the types of services needed by this population are shifting due to changes in their health issues. Older adults have historically been viewed as underweight and frail; however, over the past decade there has been an increase in the number of obese older persons. Obesity in older adults is not only associated with medical comorbidities such as diabetes; it is also a major risk factor for functional decline and homebound status. The baby boomers have a greater prevalence of obesity than any of their historic counterparts, and projections forecast an aging population with even greater chronic disease burden and disability. In light of the increasing numbers of older adults choosing to live independently rather than in nursing homes, and the important role nutrition can play in healthy aging, the Institute of Medicine (IOM) convened a public workshop to illuminate issues related to community-based delivery of nutrition services for older adults and to identify nutrition interventions and model programs. Nutrition and Healthy Aging in the Community summarizes the presentations and discussions prepared from the workshop transcript and slides. This report examines nutrition-related issues of concern experienced by older adults in the community including nutrition screening, food insecurity, sarcopenic obesity, dietary patterns for older adults, and economic issues. This report explores transitional care as individuals move from acute, subacute, or chronic care settings to the community, and provides models of transitional care in the community. This report also provides examples of successful intervention models in the community setting, and covers the discussion of research gaps in knowledge about nutrition interventions and services for older adults in the community.

In December 2016, the National Academies of Sciences, Engineering, and Medicine convened a public workshop to better understand the importance of affordable and accessible housing for older adults and people with disabilities, the barriers to providing this housing, the design principles for making housing accessible for these individuals, and the features of programs and policies that successfully provide affordable and accessible housing that supports community living for older adults and people with disabilities. This publication summarizes the presentations and discussions from the workshop.

Youth who age out of care experience poor outcomes, including high rates of mental health conditions, housing instability and homelessness, unemployment and poverty, and justice involvement. At particular risk are youth who exit semi-institutional care settings, including transitional living programs (TLPs) that support youth with achieving "independence" by the time they emancipate. This project explores: (1) how the child welfare TLPs instill independence in youth aging out of care and (2) the intersection of race and racism in the independence construction process that occurs during the transition out of care. Methods. This project used a qualitative case study approach. A nationally recognized community mental health provider in Illinois served as the study site. The program provided housing, psychiatric and transitional supports to youth diagnosed with mental health conditions who had histories of complex trauma and institutionalization. Data collected and thematically analyzed included: (1) child welfare system contract with the program and program's manual; (2) interviews with ten youth during their transition out of care; (3) memos reflecting my own experiences and observations in the field. Results. First, the child welfare system and TLP valued independence as the goal of emancipation and definition of success for this vulnerable population. Second, a deep tension emerged in transition out of care as young people adopted the rhetoric of independence while also simultaneously pursuing benefits based on their disability status. Third, lack of acknowledgment of race, racism, and institutional hardship and oppression that impacts these vulnerable youth Discussion. This project contributes to understanding "why" focusing on interdependence matters for preparing youth for aging out of care, especially from semi-institutional settings.
