Book excerpt: Improving Nurse-patient Communication about New Medicines Abstract The project aimed at improving nursing communication with patients and families about medication in order to increase patient satisfaction in terms of safety, quality of care, and patient-centeredness in an acute care setting. The selected patient cohort was at a 42-bed medical-surgical unit in a trauma center in Oakland, California. The gap of nurse-patent communication about medicines was identified based on the HCAHPs scores that had been found to be lower than average (44.1% in March 2015; 40.9% in April 2015). The visibility board was provided to show the staff the HCAHPS scores, the pre-survey scores, the information about the teach-back method, and how the "ask3/teach3" approach would be implemented during medication administration with the easy access to the information of the medicines through the EHR during the medication administration. The compared results of 40 surveys from the pre-intervention and post-intervention revealed the increased number of the patients who knew the names of new medicines they had been taking from 25% to 75%, knew the purposes from 50% to 90%, and knew the side effects from 15% to 50%. The teaching materials and strategies included the flyers about the ask3/teach3 methods, the sheets of common side effects of medications list, the communication board on the medicine section, and the role-play on ask3/teach3 intervention. Based on the project, to ensure the effectiveness of the engagement in medication communication process, it is recommended that sustained efforts in collecting data and evaluating the effectiveness of the intervention be encouraged as an ongoing process with staff collaboration. The Cardiac Procedure Unit (CPU) of a large medical center in California recently received a low HCAHPS (CMS, 2019) score of one star (out of five) in the area of medication communication. Context: The CPU is a microsystem that provides pre and post cardiac procedure care for patients of diverse backgrounds. It has a highly competent staff administering patient care. Current patient medication education faces challenges that hinder patient receptivity and thus understanding. Intervention: The proposed project employs a multi-pronged approach to improve the communication of medication education nurses provide to their patients in the CPU. This includes the creation of visual aids/flyers, changes in the patient discharge workflow, and incorporating medication discussion during the Nurse Knowledge Exchange (NKE). Measures: HCAHPS score in the area of medication communication is the primary measure for the project efficacy. Additionally, patient and staff engagement are assessed through patient rounding and NKE. Result: Since the project inception last September, with a score of one star, the score peaked at five stars two months later. The significant increase in HCAHPS score reflects the effectiveness of the project. Conclusion: COVID-19 has significantly disrupted CPU's workflow and caused a significant decrease in the Q1-2020 scores. As we strive to return to a new normal post-COVID19, additional training and adjustment must be made in the CPU to maintain quality medication communication. This will help to sustain the initial gains achieved by the project.

Health professionals often overestimate the health literacy of patients, thereby affecting patient outcomes. In a hospital environment, patients rate the ability of the nurse to explain things in a way patients understand by completing the Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) after discharge. The HCAHPS results are converted to a score which is used in a formula to determine hospital reimbursement; the lower the score, the lower the reimbursement. The purpose of this project was to improve nurse communication skills and facilitate patient understanding of care. A literature review revealed the teach-back method as a best practice strategy. Teach-back is a communication technique designed to improve patient understanding about what was said by healthcare providers. Teach-back is a way of presenting information, then asking patients to repeat what was said in their own words. An education module designed to improve nurse communication skills was implemented on a 14-bed hospitalist medical unit for the registered nurse staff (N=13). The education included content on health literacy and the teach-back method of communication. Nursing staff was surveyed before (61.5% response rate) and six weeks following (50% response rate) the educational offering, using the Health Professionals Communication Skills Scale (HP CSS), an 18-item tool measuring empathy, informative communication, respect, and social skills. In addition, patient responses to the HCAHPS question, the nurse explained things in a way you could understand were compared before and after the educational offering. -- Overall, the post-survey responses were lower than the pre-survey responses in each domain, indicating a decreased perception of communication by the nursing staff. The aggregate mean score for informative communication decreased from 5.01 to 4.83 (p = 0.37) and social skills decreased from 3.85 to 3.75 (p = 0.87) on the post-survey, with significant aggregate mean score decreases for respect 5.46 to 4.76 (p

Patients who have limited literacy skills, who have multiple comorbidities, and who are elderly face the greatest risk, and limited literacy skills are significantly associated with inadequate understanding and use of prescription instructions and precautions. The Agency for Healthcare Research and Quality notes that only 12 percent of U.S. adults have proficient health literacy that allows them to interpret a prescription label correctly. Given the importance of health literacy to the proper use of medications, and the apparent lack of progress in improving medication adherence, the Roundtable on Health Literacy formed an ad hoc committee to plan and conduct a 1-day public workshop that featured invited presentations and discussion of the role and challenges regarding clarity of communication on medication. Participants focused on using health literacy principles to address clarity of materials, decision aids, and other supportive tools and technologies regarding risks, benefits, alternatives, and health plan coverage. It explores the relationships between coordinated care, expert leadership, provider-patient communications, and the patient experience. When clinical and nonclinical staff collaborate effectively, healthcare teams can improve patient outcomes, prevent medical errors, improve efficiency, and increase patient satisfaction. Surprisingly, however, healthcare leaders tend to prioritize specific metrics to improve hospital performance and patient satisfaction even though patient experience and provider-patient communications are intertwined. Determining the most effective strategy for achieving higher levels of service quality and patient satisfaction can prove elusive for providers. Consider the evidence: a survey in 2012 of more than 17,000 healthcare leaders in North America, for example, found that leaders’ perceptions did not always match the data, and many hospital leaders overestimated the performance of their hospitals. Over 75% of the hospital leaders reported "quality of care" was something their hospital did well, while their patients, on average, rated them lower on perceived service quality. Ten years later, in 2022, only a few providers integrated best practices to achieve high patient satisfaction which severely impacted CMS Hospital Star Rating. This has significant effects on profit margins since patients consider the star rating differentials in their choices of hospitals and are willing to pay upward of 17% extra for treatments in 5-star hospitals, a revenue generating source of income at times when hospitals have seen falling revenues (down 4.8%) and rising labor (up 37%) from pre-COVID-19 pandemic levels. To reduce the gap between perception and reality, hospital leaders can consider the link between communication goals (e.g., responsiveness of hospital staff, pain management, communication about medicines) and outcomes (e.g., increased adherence and compliance, readmission, healthcare delivery costs, hospital overall ratings) as well as improve the patient experience. When intentions and outcomes are aligned, they create a powerful medium by which healthcare leaders can evaluate the gaps that exist between patient care measures and best practices and mitigate organizational or technological factors relevant to improving the patient experience. When the alignment is optimal, care teams develop a better sense of shared purpose, become more committed and accountable, and work together to improve the patient experience. When accomplished, patients participate more fully and actively in the exchange and are discharged with an enhanced commitment to carry out care management requirements. Key topics in this practical guide include provider-patient communications; demonstrating the value of patient-focused care; how physician and nurse executives use synergy as a strategy; engaging board members in promoting quality and safety goals and in developing hospital community partnerships; building bridges between physicians, administrators, trustees, and hospital staff; and developing a leadership pipeline.
