The problem was that the Channahon Fire Protection District did not have an early automatic external defibrillation (AED) program. The purpose of this applied research project was to determine whether or not it is feasible for the Channahon Fire Protection District to implement an early defibrillation program for its jurisdiction. A descriptive research methodology was used to answer the following research questions, while action research methodology was used to develop the framework for implementing an early defibrillation program: 1. Can the Channahon Fire Protection District impact the survival rate of heart attack victims through an early automatic external defibrillation program? 2. How can the Channahon Fire Protection District financially support an early automatic external defibrillation program? 3. How can the Channahon Fire Protection District make the public aware of the need for an early automatic external defibrillation program? 4. What can the Channahon Fire Protection District do to educate the public first responders on the proper use of automatic external defibrillators? The principal procedures utilized to complete the research project were a review of the fire service literature contained in magazine articles, fire service journals, and resources of the United States Fire Administration. Also referenced was literature available through the American Medical Association. The major results of this research were that successful early defibrillation programs involved delivery strategies that focused on rapid intervention as time was the major component to quality in an automatic external defibrillator program (Browett, 1998). Experts believed that more than three hundred lives a day could be saved if the maximum time to defibrillation were in the five to seven minute range (Newman,1997). Survival rates for out-of-hospital cardiac arrest are as low as 1.4% (Olson, Fontanarosa,1999) with current intervention techniques. Other factors that might influence the placement of AED's included call volume, response-area demography and the distribution of advanced cardiac life support (ACLS) and basic life support (BLS) response times (Becker,Mandell,1991). The recommendations resulting from this research included the Channahon Fire Protection District developing a comprehensive plan and framework for implementing an early defibrillation program, specific to its resouces and particular needs. These recommendations are based upon results of the research. [STAR#: 124828]

Strategies to Improve Cardiac Arrest Survival examines the complete system of response to cardiac arrest in the United States and identifies opportunities within existing and new treatments, strategies, and research that promise to improve the survival and recovery of patients. The recommendations of Strategies to Improve Cardiac Arrest Survival provide high-priority actions to advance the field as a whole. This report will help citizens, government agencies, and private industry to improve health outcomes from sudden cardiac arrest across the United States.

The purpose of the Applied Research Project (APR) is to investigate the effect of having Automated External Defibrillators on fire department apparatus and vehicles as well as the development of an operational plan for AEDs within the Village of Westmont. The research process was facilitated using action and evaluation research to answer the following questions: 1. Would an AED program improve emergency services to the community? 2. Would the development and implementation of an AED program be cost effective? 3. How effective are AEDs in actual field use? 4. What are the advantages of having an AED program? The process used to answer the above questions included a ten-question survey mailed to a total of forty fire departments, twenty in the State of Illinois and twenty to the Executive Fire Officer participants across the United States (Appendix A). Thirty-three of the forty questionnaires (82.5%) were completed and returned (Appendix B). The Illinois Fire Chiefs Association and Executive Fire Officer participant departments were chosen because they represent a sample selection of the fire departments found in Illinois and across the nation. The results of this project demonstrated positive effects for the implementation of an AED program within the Village of Westmont Fire Department. The results also indicated the need to strategically implement a plan to place AEDs in businesses, organizational groups, and schools, all of which have high probability for sudden cardiac arrests. It is the author's recommendation that an Automated External Defibrillator program to improve Emergency Medical Services and increase the survival rate of sudden cardiac arrest (SCA) victims be implemented for the Village of Westmont. Coronary artery disease accounts for approximately 500,000 of these deaths; the majority of them are sudden deaths (AHA, 1997). These pre-hospital cardiac arrest victims deaths are caused by the deadly cardiac rhythm ventricular fibrillation (VF). Treatment for VF is rapid defibrillation. The Semi Automatic External Defibrillator (SAED) is proposed as a way for Emergency Medical Technicians (EMT's) to deliver rapid defibrillation in the field. The purpose of this study was to determine the outcomes of the SAED when used by EMT's compared to manual defibrillators used by paramedics. Outcomes were compared in terms of response time and success determined by admission to the Emergency Department with an adequate cardiac rhythm. Using an Expost Facto research design data were collected from medical records from 1992-1997. The paramedics field report and the SAED utilization report used by EMT's provided data regarding age of the patient, their sex, and the response time of the EMT's and/or paramedics. The response time and success rate of EMT's using the SAED at converting VF to an adequate cardiac rhythm were compared to the response time and success rate of paramedics using manual defibrillators. One hundred and fifty pre-hospital arrest patients in each group was studied. In the SAED group there was a 15% survival rate of reaching the Emergency Department with a pulse post cardiac arrest. The manual defibrillation group had a 17% survival rate. However, the prehospital cardiac arrest patients treated with the SAED who survived to reach the Emergency Department had a 39% chance of being discharged from the hospital alive compared to an 11% if treated by manual defibrillators. Based on a chi square test (x 2 =3.125, df=298) there was no significant difference in the two groups.
