De nouveaux enjeux se dessinent pour les professionnels de santé avec pour objectif, le maintien de la plus grande autonomie possible chez les personnes âgées. Nous avons conduit une étude pilote au sein du CHU de Caen, randomisée, prospective, au premier trimestre 2012 pour évaluer l’impact de la mise en place d’une consultation avancée de gériatrie pour les patients de plus de 75 ans consultant aux urgences et présentant un score ISAR (score d’évaluation de la fragilité) supérieur ou égal à deux. Trois objectifs étaient recherchés : étudier les seconds recours aux urgences à un mois, rechercher des indicateurs de fragilité dans la population cible, évaluer les bénéfices en médecine générale. Au terme de notre travail, il apparait que la consultation avancée de gériatrie semble diminuer significativement le taux de second recours à trente jours (20,4% vs 6,3% ; p=0,02). Par ailleurs, les patients consultant pour des pathologies ostéoarticulaires présentent le plus fort taux de retour (27,8%). La consultation avancée de gériatrie constitue en outre un outil intéressant pour les médecins généralistes afin d’optimiser leur prise en charge de ces patients. Il ressort également que Le délai entre la consultation aux urgences et l’évaluation gériatrique devrait être modulé en fonction du motif de recours initial. La pérennisation et l’élargissement de ce dispositif pourrait améliorer la gestion des patients âgés. Une étude à plus grande échelle serait alors nécessaire pour confirmer ces perspectives et en évaluer l’impact économique et sanitaire.
