Book excerpt: Durant la pandémie « Covid-19 », la France a fait face à une période de 2 mois de confinement en 2020, marquée par de nombreuses restrictions. Cette étude était la première qui s'intéresse à l'impact du confinement sur le vécu et la prise en charge des enfants âgés de 6 à 17 ans atteints de troubles des apprentissages et/ou de l'attention. Il s'agissait d'une étude observationnelle, rétrospective, avec recueil de données par questionnaire qualitatif numérique anonyme à destination de leurs parents. Le recueil de données s'est effectué du 1er février au 31 mai 2021. Les données ont été comparées selon 2 groupes : vécu négatif et vécu positif ou neutre du confinement de l'enfant. Au total, 251 questionnaires ont été collectés et 217 analysés. 47 enfants ont déclaré avoir mal vécu la période de confinement, soit 21,7%. 75,6% ont bénéficié d'un suivi de leur scolarité durant toute la période, mais 50,5% des parents rapportaient un format de cours inadaptés aux troubles de leur enfant, 53% rapportaient un manque de motivation et 46,5% un refus de leur enfant d'étudier. Dans le groupe traité, la progression en compréhension de texte est significativement plus importante (significativité persistante en sous-groupe chez les femmes, les enfants sans hyperactivité, sans trouble du langage oral) ; il n'est pas trouvé de différence en leximétrie, transcription, mathématiques, par rapport au groupe non traité. Cette étude présente plusieurs limites, dont un petit effectif ayant pu impacter nos résultats, mais il apparait nécessaire de dépister systématiquement un TDA/H devant des troubles spécifiques des apprentissages, afin de proposer rapidement un traitement médicamenteux pour favoriser la progression, sans banaliser les difficultés attentionnelles présentes.

L’objectif principal de cette étude vise à analyser les particularités du parcours de soins de ces enfants. Méthode. Une étude de cohorte, rétrospective, monocentrique et descriptive a été menée chez 125 enfants âgés de 6 à 15 ans, présentant un TDA/H, confirmé sur la base d’un bilan neuropsychologique, et adressés au centre de référence des troubles des apprentissages (CRTA) du Centre hospitalier universitaire (CHU) de Saint-Etienne, sur une période de 6 ans, de janvier 2008 à décembre 2013. Résultats. Cette étude met en évidence un parcours de soins difficile chez ces enfants avec un délai moyen de 37 mois entre la plainte attentionnelle et le moment du diagnostic de TDA/H. Conclusion. Ce travail souligne l’importance d’une prise en charge précoce de ces enfants afin de leur permettre une bonne acquisition des apprentissages scolaires. Cela nécessite une bonne formation des professionnels de première intention.

L'hyperactivité infantile suscite un intérêt croissant bien qu'elle reste un sujet controversé en France. Historiquement, deux concepts idéologiques se sont affrontés: l'une européenne, qui décrit l'hyperactivité comme un symptôme de troubles psychologiques internes et l'autre anglo-saxonne qui défend la thèse d'un trouble d'origine neurologique. Aujourd'hui, l'hyperactivité est reconnue par la plupart des auteurs comme une pathologie à part entière. - L'hyperactivité de l'enfant, encore appelée instabilité psychomotrice, THADA, est un trouble de découverte vers l'âge de 7 ans, associant trois symptômes : un déficit persistant d'attention et/ou une hyperactivité, une impulsivité plus fréquents et plus sévères que ce qui est constaté habituellement chez les sujets d'âge similaire, aussi bien en milieu scolaire que familial, entravant les processus d'apprentissages. - La prévalence du trouble est de l'ordre de 5 %. Les étiologies évoquées, d'une entité complexe, révèlent des facteurs multiples plus ou moins associés : génétiques, neurobiologiques, neurodéveloppementaux, pré ou pernataux, environnementaux et psychosociaux. - La clinique se traduit chez l'enfant par des difficultés à s'atteler et à se maintenir à une tâche, s'organiser, anticiper un travail, maintenir son attention ou encore à un déficit de la mémoire à court terme. - Il existe fréquemment, dans 70 % des cas, des troubles associés, qualifiés de comorbides : troubles des apprentissages, anxiodépressifs, de l'humeur, oppositionnels et/ou de provocation, tics... - Le diagnostic du THADA, évoqué après un parcours long, repose avant tout sur différents entretiens de l'enfant, complétés éventuellement par des questionnaires (échelles de Conners) aux parents, aux enseignants. L'ensemble sera corrélé à l'évaluation des capacités intellectuelles, du bagage langagier ainsi que des performances psychomotrices. - Le cheminement de ces enfants est global, associant un traitement psychothérapique comportemental, médicamenteux avec l'utilisation d'un psychostimulant apparenté aux amphétamines, le méthylphénidate, commercialisé en France sous le nom de Ritaline® et Concerta® LP et mesures psychopédagogiques (stratégies d'apprentissage). - Notre travail, dans un premier temps, présente et discute les différentes descriptions de l'hyperactivité observées dans la littérature. Ce faisant, celui-ci met en lumière l'hétérogénéité, la complexité et l'aspect plurifactoriel du trouble, nécessitant une évaluation pluridimensionnelle. - Dans une deuxième partie, l'analyse rétrospective des dossiers de 20 enfants reçus en consultation neuropsychopédiatrique à AIR à Besançon confirme l'intérêt d'une approche conjointe et précise sa place dans le parcours de soins d'enfants en grandes difficultés... - La dernière partie de notre tâche montre la place du médecin généraliste dans le dépistage le plus précoce possible de l'hyperactivité infantile. - L'omnipraticien, premier intervenant à l'écoute des problèmes des différents membres d'une famille, peut être frappé par différents éléments cliniques observés chez l'enfant, faisant suspecter le trouble. - Par ses explications et conseils, il va dédramatiser la situation en déculpabilisant les parents. Le médecin traitant averti et motivé peut servir de lien entre les différents intervenants dans le cadre d'une équipe pluridisciplinaire en coordonnant la prise en charge globale de l'enfant. L'aboutissement final du dépistage est d'éviter les troubles des apprentissages graves avec comme conséquence un échec scolaire irrémédiable et un handicap social. L'objectif principal était d'évaluer la perception des médecins généralistes concernant leur rôle dans la prise en charge du TDAH chez l'enfant et l'adolescent, et secondairement faire un état des lieux des connaissances et des pratiques des médecins généralistes dans ce contexte. - Matériel et méthodes : Une étude descriptive transversale a été réalisée à l’aide d'un auto-questionnaire, envoyé par mail via l'Ordre des médecins de Saône-et-Loire, l'Union régionale des professionnels de santé (URPS) et à titre personnel aux médecins généralistes de Bourgogne Franche - Comté. - Résultats : Parmi les 2186 médecins contactés, 157 réponses ont été analysées. Un tiers des médecins sous-estimait la prévalence du TDAH. Plus de la moitié des médecins interrogés (57,3%) déclaraient suivre au moins un enfant ayant un TDAH. Cependant, la très grande majorité (81,5%) considérait ne pas être l'interlocuteur de référence dans la prise en charge. 93,6% jugeaient leur niveau de formation sur le TDAH insuffisant et 87,9% avaient exprimé un intérêt pour une formation sur le sujet. - Discussion : Ces résultats encouragent l'approfondissement d'outils et de formations adaptés aux médecins généralistes, ainsi qu'une meilleure communication sur le développement récent des plateformes de coordination et d'orientation des troubles neurodéveloppementaux dans le cadre de la stratégie nationale de Santé 2018 - 2022.

It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured? Are the potential benefits of such products specific, or can these additions be attributed to nonspecific factors? What are the users’ views on such tools? What are the possible links between such tools and social networks? Is there a gap between evidence-based results and market development? Are there any quality challenges? Comment intégrer de façon spécifique dans un projet thérapeutique ces troubles associés qui entravent la rééducation orthophonique ? Nous avons abordé cette question sous l'angle de la métacognition, à travers un outil de renforcement positif appelé Cartes de fierté. Afin d'étudier l'impact de la métacognition et de la motivation sur les compétences exécutives, nous avons analysé les progrès des patients, rapportés grâce à un questionnaire rempli par les orthophonistes qui ont utilisé cet outil. Parmi 15 sujets avec TDAH, on observe des tendances d'amélioration de l'inhibition, de l'impulsivité, du contrôle exécutif, de l'attention et de la motivation. Des résultats similaires sont observés à l'échelle de la population des 50 sujets. Ces tendances illustrent l'intérêt d'une remédiation métacognitive dans la prise en charge de troubles d'apprentissage associés à des déficits exécutifs, et l'effet bénéfique de la motivation sur l'apprentissage.

Book excerpt: Introduction : La complication la plus fréquente de la Neurofibromatose de type 1 (NF1) est représentée par les troubles d'apprentissage parmi lesquels les troubles visuo-spatiaux et de l'attention sont prépondérants et les plus communément admis. Pour autant, l'expérience clinique acquise durant le suivi de ces patients indique que ces troubles attentionnels diffèrent de ceux observés dans le Trouble Déficitaire de l'Attention avec ou sans Hyperactivité (TDAH). L'objectif était donc de comparer le profil attentionnel d'enfants atteints d'une NF1 ayant des symptômes suggérant un phénotype TDAH à celui d'enfants non atteints de NF1 mais ayant un TDAH idiopathique typique dit syndromique. Méthodes : 15 enfants atteints de NF1 âgés de 6 à 15 ans et 47 enfants ayant un TDAH syndromique, diagnostiqués et suivis en Neuropédiatrie au CHRU de Tours entre 2012-2017, ont été inclus. Les profils attentionnels ont été déterminés au travers d'une évaluation subjective (Conners) et objective (TEACh). D'autres fonctions (exécutives et visuo-spatiales) ont été analysées via des subtests des échelles de Wechsler et du TEACh. Les patients avec une déficience intellectuelle ont été exclus. Résultats : Alors qu'il n'y avait pas de différence significative aux questionnaires subjectifs de Conners, seuls 40% enfants atteints d'une NF1 présentaient un phénotype TDAH lors de l'évaluation objective au travers du TEACh. De plus, les profils attentionnels différaient significativement entre les deux groupes avec de meilleurs scores pour l'attention sélective et l'attention divisée dans le groupe NF1. Il n'y avait pas de différence significative pour les fonctions exécutives mais un trouble visuo-perceptif a été identifié dans le groupe NF1. Conclusions : Nos résultats suggèrent que les troubles attentionnels observés dans la NF1 sont différents de ceux observés dans le TDAH syndromique et ne seraient pas liés non plus à un trouble des fonctions exécutives. Nous formulons l'hypothèse que les troubles d'attention dans la NF1 seraient consécutifs à un trouble visuo-perceptif propre à la NF1. Ceci pourrait expliquer la nature différente des difficultés scolaires et des plaintes parentales rencontrées dans la NF1 par comparaison au TDAH, avec un impact décisif sur la prise en charge de ces patients.

Brown offers a contemporary perspective on how the mind transforms, and gives meaning to, emotional experience that arises unconsciously in the here-and-now of the clinical hour. Brown surveys the developments in theory and practice that follow from Freud’s original observations and traces this evolution from its conception to contemporary analytic field theory. Brown emphasizes that these unconscious transformational processes occur spontaneously, in the blink of an eye, through the "unconscious work" in which the analyst and patient are engaged. Bion’s theories of alpha function, transformations, dreaming and his clinical emphasis on the present moment are foundational to this book. Brown’s writing is clear and aims to describe the various theoretical ideas as plainly as possible. Detailed clinical material is given in most chapters to illustrate the theoretical perspectives. Brown applies this theory of transformational processes to a variety of topics, including the analyst’s receptivity, countertransference as transformation, the analytic setting, the paintings of J.M.W. Turner, "autistic transformations" and other clinical situations in the analysis of children and adults. The Analyst's Reveries finds increasing veneration for the analyst’s use of their reveries, while revealing important differences amongst post-Bionians in how reverie is defined and used clinically. Fred Busch ponders if it has been fully recognized that some post-Bionions suggest a new, radical paradigm for what is curative in psychoanalysis. After searching for the roots of the analyst’s use of reverie in Bion’s work and questioning whether in this regard Bion was a Bionian, Busch carefully examines the work of some post-Bionians and finds both convincing ways to think about the usefulness and limitations of the analyst’s use of reverie. He explores questions including: From what part of the mind does a reverie emerge? How does its provenance inform its transformative possibilities? Do we over-generalize in conceptualizing what is unrepresented, with the corresponding problem of false positives? Do dreams equal understanding and what about the generalizability of the co-created reverie? Busch concludes that it is primarily through the analyst’s own associations that the reverie’s potential is revealed, which further helps the analyst distinguish it from many other possibilities, including the analyst’s countertransference. He believes in the importance of converting reveries into verbal interpretations, a controversial point amongst post-Bionians. Busch ends with the difficult task of classifying the analyst’s reveries based on their degree of representation.
