Book excerpt: L'imagerie par résonance magnétique (IRM) est une technique d'imagerie relativement lente. Les mouvements du patient, en particulier en imagerie cardiaque et abdominale, constituent un obstacle important, venant perturber le processus d'encodage spatial nécessaire à la réalisation d'une image. Ces mouvements induisent des dégradations de l'image, appelées artéfacts, qui prennent des formes complexes du fait que cet encodage est réalisé dans un espace inhabituel (espace hybride Fourier/sensibilités d'antennes). Les méthodes généralement utilisées (apnée du patient, synchronisation de l'acquisition avec l'électrocardiogramme) sont imparfaites et ne sont pas toujours applicables. A l'aide d'une plateforme spécialement développée pour l'acquisition et l'analyse des données physiologiques pendant l'examen IRM, nous proposons d'intégrer des informations de mouvement a priori, à différents niveaux des processus d'acquisition et de reconstruction, avec un intérêt particulier pour la correction des erreurs d'encodage spatial. Nous construisons un modèle prédictif permettant d'estimer les champs de déplacements élastiques dans le plan ou le volume imagé, à partir de combinaisons linéaires des signaux fournis par la plateforme. Puis nous définissons un cadre de reconstruction généralisé permettant d'inclure les données de déplacements prédites par le modèle, afin de reconstruire une image corrigée, minimisant ainsi les artéfacts de mouvement. Les hypothèses et conditions de validité sont analysées, et le modèle prédictif ainsi que les différentes méthodes de reconstruction proposées sont validés sur des données réelles, cardiaques et abdominales, de sujets sains.

Bien que la résolution temporelle se soit améliorée avec les techniques d'imagerie rapide et parallèle, les mouvements physiologiques du type cardio-respiratoire restent gênants et dégradent la qualité des images. L'apnée et la synchronisation sont les techniques les plus couramment utilisées. Celles-ci ont des limites et restent difficiles pour certains patients. Pour mettre en oeuvre l'imagerie adaptive qui consiste à intégrer les mouvements des patients dans les processus d'acquisition et de reconstruction, nous devons disposer de capteurs de mouvements compatibles IRM donnant la meilleure représentation du mouvement des organes. Différents types de capteurs et méthodes de validations ont été développés pour intégrer ces informations externes aux algorithmes de reconstruction. Chaque capteur conçu devait être non-invasif et compatible à l'environnement IRM (champ magnétique, gradients de champ magnétique, et ondes de radiofréquences). La conception des capteurs impliquait la mise en oeuvre d'outils d'acquisition, et des méthodes de tests et de validations. Différent concepts de capteurs ont été explorés et testés. La validation finale étant faite par l'intégration des signaux à l'imagerie adaptative qui conduit à un apport au niveau de la qualité de l'image et par conséquent une amélioration du diagnostic.

Pour s'affranchir de ses mouvements, des méthodes telles que l'apnée ou la synchronisation sont habituellement utilisées en routines cliniques. Elles présentent des limites et restent contraignantes. Les techniques adaptatives intègrent les informations liées aux mouvements dans les processus d'acquisition et/ou de reconstruction des images afin de les compenser ou de les corriger. Elles nécessitent la connaissance de ces mouvements qui peuvent être estimés à l'aide de capteurs internes ou externes. Nous avons développé des capteurs à base d'accéléromètres, souvent utilisés pour estimer les mouvements, en respectant les contraintes imposées par l'environnement hostile de l'IRM et par l'utilisation en conditions cliniques. La compatibilité IRM ainsi que le fonctionnement en IRM, sur fantômes et volontaires sains, ont été évalués à l'aide de méthodes spécifiques mises en place. Deux autres capteurs basés sur des principes optiques ont également été testés, l'un sur fantômes uniquement et l'autre sur fantômes ainsi que sur volontaires sains. Enfin, les signaux obtenus par les différents capteurs ont été intégrés dans une méthode de correction rétrospective de mouvements développée au sein du laboratoire. L'objectif est de réduire les artéfacts de mouvements en intégrant les informations, issues des capteurs, lors de la reconstruction des images.

The fourth chapter studies the brain stem vascular network in detail. Thus, this chapter sums up the results of research on brainstem superficial blood vessels and their intra nervous territories that were already presented in two previous works [79, 80]. By contrast, presentation of the cerebellar vascularization follows the previous literature.
