Book excerpt: The functional heterogeneity of cancer phenotypes and responses to therapeutics is a huge obstacle to clinical cures and an intense area of study. A relatively new hypothesis posits the existence of a subpopulation of tumor cells, termed cancer stem cells, which are primarily responsible for tumor propagation and resistance to therapy. Identification of such cells may be important to develop targeted therapies for more effective cancer treatment. This dissertation focuses on the validation and functional study of cancer stem cells associated with human prostate and pancreatic cancers. By leveraging the fact that surgical castration leads to enrichment of stem/progenitor cells in the prostate, we idenitifed CD166 as a novel marker for prostate stem cells. We show that CD166 can further enrich and refine the established prostate stem/progenitor cell and cancer stem cell population in both murine and human systems. While genetic deletion of CD166 would not inhibit the development of the prostate gland or the formation of prostate cancer in our genetically engineered model systems, this protein may serve as an attractive marker for identifying and targeting prostate cancer stem cells. Further attempts were made at identifying stem/progenitor populations for prostate and pancreatic cancer through various discovery and targeted approaches. We found that Lgr5, a marker of stem cells in the gut and skin, is not expressed in prostate and pancreatic epithelial tissue, but only in prostatic stroma. Novel surface markers such as CD138 were found to be highly upregulated in pancreatic tumors, but specificity for cancer stem cells was lacking. Lastly, we studied the functional role of cancer stem cells in gemcitabine therapy resistance in a human pancreatic cancer model. We found that the PI3K and ubiquitin-mediated proteosomal degradation pathways can be used to stratify gemcitabine treatment response in patient tumors. Inhibition of Skp2 expression and therapeutic treatment with PI3K inhibitors proved to be effective sensitiziers to gemcitabine treatment. Importantly, cancer stem cell content and function did not mediate gemcitabine relapse. As such, our studies reveal the cancer stem cell paradigm may be more complicated and cancer context-dependent, and must be rigorously functionally proven prior to clinical adoption.

Despite these advances in individual tumors, a number of patients are treated for their primary tumors and ultimately relapse. Relapsing tumors can be due to resistance to chemotherapy or to antioncogene drugs. Another hypothesis is that heterogeneity in the tumor leads to an inability of classical chemotherapy to completely eradicate all cells of the tumor. This concept of heterogeneity has led some investigators to propose a cancer stem cell model. As patients are treated with chemotherapy, most of the dividing cells are killed, but this leaves a small subset of cells that have the ability to remake the entire tumor. These are cancer stem cells. They possess the signals of self-renewal and yet can also differentiate. If one could understand more about the cells that remain after classical chemotherapy or the cells that can remake the tumor among the heterogeneous population, this information would have a huge impact on our treatment of cancer.

Book excerpt: Despite recent advances in cancer treatment, resistance to therapy and metastatic progression remain critical drivers of mortality. One central mechanism underlying therapy resistance and metastasis is tumor heterogeneity. Within the tumor bulk, genetic and epigenetic diversity fuel variable responses to therapy and a spectrum of invasive potential. In particular, rare subpopulations of tumor cells that reactivate developmental signals are uniquely primed for therapy resistance and metastatic success. These cells, often referred to as cancer stem cells, are enriched for the ability to self-renew in the face of therapy, driving eventual relapse. Deepening our understanding of the molecular dependencies of these aggressive cells may provide new opportunities for therapeutic intervention. In collaboration with a fellow graduate student (Nikki Lytle) who led this project, we used transcriptional and epigenetic profiling paralleled by a genome-wide CRISPR analysis to map the molecular dependencies of pancreatic cancer stem cells. This integrated approach revealed an unexpected utilization of immuno-regulatory signals by pancreatic cancer cells, and identified the nuclear hormone receptor (ROR[gamma]) as a targetable dependency in pancreatic cancer stem cells. We expanded preclinical work to test ROR[gamma] inhibitors, providing new evidence that clinical grade ROR[gamma] inhibitors can block pancreatic cancer growth and deplete cancer stem cells in vivo. These studies also revealed a unique epigenetic landscape in cancer stem cells, suggesting upstream epigenetic regulation of stem cell fate. Thus, to follow this work, I used a curated functional screen for stem cell-enriched epigenetic factors, ultimately identifying the SWI/SNF subunit SMARCD3 as an epigenetic dependency in pancreatic cancer stem cells. Using diverse genetic mouse models, I showed that Smarcd3 dependency is bimodal, with a preferential impact in established tumors, improving survival and chemosensitivity in vivo. Finally, I leveraged genetically engineered mouse models to identify and test clinical inhibitors that target cancer stem cells. Using a genetic reporter for the stem cell signal Msi2, I helped conduct an image-based screen and found that clinical inhibitors of MEK signaling inhibited Msi2 and blocked CSC growth in vivo. Together, these studies generate a comprehensive molecular profile of the landscape and functional requirements of pancreatic cancer stem cells that may be used to identify new therapeutic targets in the future.
