Book excerpt: As Dr. Cullen’s chapter on information technology points out, what is required is not just a new electronic system that follows the patients, but a new language that creates and defines a system that can appropriately care for the patient. What we design for the complexities of caring for the medically underserved can serve as model for caring for everyone in this country. Many innovative, bold, and wonderful solutions have been developed as local/ regional models. As communities and states we can learn from, and support, each other. But the local models are not, by and large, self-sustaining. Ultimately, so- tions to the lack of medical insurance in this country will require a national persp- tive, and federal funding. That is part of the work we all must do, and Dr. Dalen’s chapter points out some of the possibilities and pitfalls other countries have experienced. When I wonder how the system we have hasn’t already collapsed from its own weight, I just need to look at the people working within it. Healthcare is a service industry, and we have been blessed with professionals who understand and live the concept of service in their daily lives, who go the extra mile for the patient despite the vagaries, the barriers, and the sometimes mean spiritedness of the organi- tional infrastructure.

Available in PDF, EPUB and Kindle. Book excerpt: Health Insurance is a Family Matter is the third of a series of six reports on the problems of uninsurance in the United Sates and addresses the impact on the family of not having health insurance. The book demonstrates that having one or more uninsured members in a family can have adverse consequences for everyone in the household and that the financial, physical, and emotional well-being of all members of a family may be adversely affected if any family member lacks coverage. It concludes with the finding that uninsured children have worse access to and use fewer health care services than children with insurance, including important preventive services that can have beneficial long-term effects.

Book excerpt: The Affordable Care Act’s impact on coverage, access to care, and systematic exclusion in our health care system The Affordable Care Act set off an unprecedented wave of health insurance enrollment as the most sweeping overhaul of the U.S. health insurance system since 1965. In the years since its enactment, some 20 million uninsured Americans gained access to coverage. And yet, the law remained unpopular and politically vulnerable. While the ACA extended social protections to some groups, its implementation was troubled and the act itself created new forms of exclusion. Access to affordable coverage options were highly segmented by state of residence, income, and citizenship status. Unequal Coverage documents the everyday experiences of individuals and families across the U.S. as they attempted to access coverage and care in the five years following the passage of the ACA.It argues that while the Affordable Care Act succeeded in expanding access to care, it did so unevenly, ultimately also generating inequality and stratification. The volume investigates the outcomes of the ACA in communities throughout the country and provides up-close, intimate portraits of individuals and groups trying to access and provide health care for both the newly insured and those who remain uncovered. The contributors use the ACA as a lens to examine more broadly how social welfare policies in a multiracial and multiethnic democracy purport to be inclusive while simultaneously embracing certain kinds of exclusions. Unequal Coverage concludes with an examination of the Affordable Care Act’s uncertain legacy under the new Presidential administration and considers what the future may hold for the American health care system. The book illustrates lessons learned and reveals how the law became a flashpoint for battles over inequality, fairness, and the role of government. More books on the health care debate

Book excerpt: The “wildly undersold story” (Lawrence Lessig) of the next American revolution, and the inspiring citizen activists fighting to save America’s fragile democracy. Our country is dominated by a political party that has no interest in governing, and that seeks to entrench its power by limiting democracy—going so far as to force people to the polls in the middle of a pandemic. Yet there is hope, as best-selling author David Daley argues in Unrigged, though it doesn’t lie in Congress, gerrymandered statehouses, or even the courts. We must, instead, look to the grassroots. Introducing us to groups that have pioneered innovative organizing methods—often combining old-school activism with new digital tools—Daley uncovers the story behind voting-rights victories nationwide and the new organizations reinventing our politics. The result is a vivid portrait of a new civic awakening, and an essential toolkit for reviving our democracy in the Trump era and beyond.

Available in PDF, EPUB and Kindle. Book excerpt: The surprise election of Donald J. Trump to the presidency of the United States marks a singular turning point in the American republic – not only because of his idiosyncratic approach to the office, but also because the Republican Party now holds the presidency and both houses of Congress, presenting a historic opportunity for change. The role of older Americans has been critical in both shaping and reacting to this political moment. Their political orientations and behaviors have shaped it through their electoral support for Republican candidates. But, older Americans stand as highly invested stakeholders in the policy decisions made by the very officials they elected and as beneficiaries of the programs that Republicans have targeted for cuts or elimination. This comprehensive volume explores the ways in which Trump administration policies are likely to significantly undermine the social safety net for near-elderly and older Americans, including long-term care, housing, health care, and retirement. The authors also explore how the Trump administration might shape politics and political behavior through the policy changes made. The response of older voters, in upcoming elections, to efforts by the Trump administration and its Republican allies in Congress to draw back on the federal government’s commitment to programs and policies affecting them will shape the direction of aging policy and politics for years to come. This book was originally published as a special issue of the Journal of Aging & Social Policy.
