Book excerpt: By the mid-twentieth century, two things appeared destined for extinction in the United States: the practice of home birth and the profession of midwifery. In 1940, close to half of all U.S. births took place in the hospital, and the trend was increasing. By 1970, the percentage of hospital births reached an all-time high of 99.4%, and the obstetrician, rather than the midwife, assumed nearly complete control over what had become an entirely medicalized procedure. Then, seemingly out of nowhere, an explosion of new alternative organizations, publications, and conferences cropped up, documenting a very different demographic trend; by 1977, the percentage of out-of-hospital births had more than doubled. Home birth was making a comeback, but why? The executive director of the American College of Obstetricians and Gynecologists publicly noted in 1977 the "rising tide of demand for home delivery," describing it as an "anti-intellectual-anti-science revolt." A quiet revolution spread across cities and suburbs, towns and farms, as individuals challenged legal, institutional and medical protocols by choosing unlicensed midwives to catch their babies at home. Coming Home analyzes the ideas, values, and experiences that led to this quiet revolution and its long-term consequences for our understanding of birth, medicine, and culture. Who were these self-proclaimed midwives and how did they learn their trade? Because the United States had virtually eliminated midwifery in most areas by the mid-twentieth century, most of them had little knowledge of or exposure to the historic practice, drawing primarily on obstetrical texts, trial and error, and sometimes instruction from aging home birth physicians to learn their craft. An Update on Research Issues in the Assessment of Birth Settings is the summary of a workshop convened in March, 2013, to review updates to the 1982 report. Health care providers, researchers, government officials, and other experts from midwifery, nursing, obstetric medicine, neonatal medicine, public health, social science, and related fields presented and discussed research findings that advance our understanding of the effects of maternal care services in different birth settings on labor, clinical and other birth procedures, and birth outcomes. These settings include conventional hospital labor and delivery wards, birth centers, and home births. This report identifies datasets and relevant research literature that may inform a future ad hoc consensus study to address these concerns.

Pamela Klassen introduces a surprisingly diverse group of women, in their own words, while also setting their birth stories within wider social, political, and economic contexts. In doing so, she emerges with a study that disrupts conventional views of both childbirth and religion by blurring assumed divisions between conservative and feminist women and by taking childbirth seriously as a religious act. Most American women who have a choice give birth in a hospital and request pain medication. Yet enough women choose and advocate unmedicated home birth--and do so for carefully articulated reasons, social resistance among them--to constitute a movement. Klassen investigates why women whose religious affiliations range from Old Order Amish to Reform Judaism to goddess-centered spirituality defy majority opinion, the medical establishment, and sometimes the law to have their babies at home. In considering their interpretations--including their critiques of the dominant medical model of childbirth and their views on labor pain--she examines the kinds of agency afforded to or denied women as they derive religious meanings from childbirth. Throughout, she identifies tensions and affinities between feminist and traditionalist appraisals of the symbolic meaning of birth and the power of women. What does home birth--a woman-centered movement working to return birth to women's control--mean in practice for women's gender and religious identities? Is this supreme valuing of procreation and motherhood constraining, or does it open up new realms of cultural and social power for women? By asking these questions while remaining cognizant of religion's significance, Blessed Events challenges both feminist and traditionalist accounts of childbearing while broadening our understanding of how religion is ''lived'' in contemporary America.
