Book excerpt: Monitoring and analysis of public expenditure on HIV/AIDS is vital given the severity of the HIV/AIDS epidemic, the substantial inflows of donor funds, and the critical need for investment from the national fiscus. This book aims to examine how governments in 4 African (Kenya, Mozambique, Namibia and South Africa) and 5 Latin American countries (Argentina, Chile, Ecuador, Mexico and Nicaragua) are funding the fight against HIV/AIDS, and simultaneously builds capacity for HIV/AIDS budget analysis within civil society. This study is unprecedented and invaluable for its unique research into government budget allocations for HIV/AIDS in these countries. No other such recent, targeted research exists in these countries, let alone is collected for its comparative value from a regional and cross-continental perspective. The research was generated by local, independent institutions within the countries concerned. The report therefore represents a collective effort by local non-governmental organizations to monitor their own government spending on HIV/AIDS, and thus to act as a watchdog to ensure that government resource allocation decisions reflect vital public priorities. This report needs to be read by anyone who is concerned with AIDS and with government financing - and that includes parliamentarians and governments themselves. It especially needs to be read by those who are looking at AIDS and government expenditure, trying to understand where the money is coming from, where it is going, and why it is being spent that way.

Book excerpt: HIV/AIDS continues to take a tremendous toll on the populations of many countries, especially in sub-Saharan Africa. In some countries with high HIV prevalence rates, life expectancy has declined by more than a decade and in a few cases by more than two decades. Even in countries with HIV prevalence of around 5 percent (close to the average for sub-Saharan Africa), the epidemic can reverse gains in life expectancy and other health outcomes achieved over one or two decades. This volume highlights work conducted under the umbrella of a World Bank work program on “The Fiscal Dimension of HIV/AIDS,” including country studies on Botswana, South Africa, Swaziland, and Uganda. It covers four aspects of the fiscal dimensions of HIV/AIDS: First, it aims for a comprehensive analysis of the fiscal costs of HIV/AIDS, with a wider scope than a costing analysis focusing on only the policy response to HIV/AIDS. Second, it embeds the analysis of HIV/AIDS costs in a discussion of the fiscal context, and interprets these costs as a quasi-liability, not a debt de jure, but a political and fiscal commitment that binds fiscal resources in the future and cannot easily be changed, and very similar to a pension obligation or certain social grants or services. Third, it develops tools to assess the (fiscal dimensions of) trade-offs between HIV/AIDS policies and measures that take into account the persistence of these spending commitments. Fourth, most of the fiscal costs of HIV/AIDS are ultimately caused by new infections, and this study estimates the fiscal resources committed (or saved) by an additional (or prevented) HIV infection. Building on these estimates, the analysis here is able to assess the evolving fiscal burden of HIV/AIDS over time.

Book excerpt: Across Africa, people in whom significant resources have been invested are dying prematurely from AIDS that continues to pose serious challenges to the ability of African States to maintain efficient public institutions and deliver sound policies and promote the rule of law. HIV infection patterns are continually changing; economic realities are making treatment adherence difficult and resistance to current drugs are beginning to emerge. These challenges can be faced and surmounted through better appreciation of the country-specific epidemic, use of better data for national response good governance and stronger political commitments at all levels of society. Therefore, comprehensive and enhanced response to AIDS must be at the centre of development strategies. This retrospective report presents key messages and policy recommendations formulated with a crucial role of the views of people affected and infected by AIDS.

Book excerpt: The failure of many African universities to speak out and confront the HIV/AIDS crisis head-on leads to the unraveling of hard-won development gains and crippling prospects for future economic growth. The report, "Challenging the Challenger: Understanding and Expanding the Response of Universities in Africa to HIV/AIDS," based on seven case studies (Benin, Ghana, Kenya, Namibia, South Africa, and Zambia), seeks to understand how the disease is affecting African universities and to identify responses and coping mechanisms that might profitably be shared with sister institutions in similar circumstances. The studies show that HIV/AIDS is having a serious impact on the fiscal situation of the universities in much the same ways as it does on other institutions. The disease increases operating costs, reduces productivity, diverts resources, and threatens sources of income. The case studies insist that committed leadership among the university's top management is the foremost requirement to combat this crisis.

Book excerpt: While much progress has been made on achieving the Millenium Development Goals over the last decade, the number and complexity of global health challenges has persisted. Growing forces for globalization have increased the interconnectedness of the world and our interdependency on other countries, economies, and cultures. Monumental growth in international travel and trade have brought improved access to goods and services for many, but also carry ongoing and ever-present threats of zoonotic spillover and infectious disease outbreaks that threaten all. Global Health and the Future Role of the United States identifies global health priorities in light of current and emerging world threats. This report assesses the current global health landscape and how challenges, actions, and players have evolved over the last decade across a wide range of issues, and provides recommendations on how to increase responsiveness, coordination, and efficiency â€" both within the U.S. government and across the global health field.

Book excerpt: This report presents three hypothetical case studies for how the AIDS epidemic in Africa could evolve over the next 20 years based on policy decisions taken today by African leaders and the rest of the world; and considers the factors likely to drive the future responses of African countries and the international community. The scenarios draw on the age-old tradition of story-telling, rather than using data projections, to explore the wider context of the AIDS epidemic, reflecting the complexity of the subject matter.

[...] The researchers warn that treatment alone may not be enough to curb the epidemic and that the governments of Latin America need to increase their allocations and commitment to prevention. [...] 3 While it is difficult to isolate specific HIV/AIDS allocations from the overall health budget, the report finds HIV/AIDS earmarked funds consume less then 4% of health budgets in the Latin American countries included in the study, ranging in 2002, from under 1% in Chile to 3.5% in Argentina. [...] In Africa, the priority accorded to HIV/AIDS programmes in health budgets varied significantly-from 1.6% in Mozambique, to 11.4% in Kenya in 2002. [...] In South Africa the HIV/AIDS allocations have climbed steadily from 0.67% of the consolidated national and provincial health budget in 2000/1, to 3.86% in 2004/5.

Available in PDF, EPUB and Kindle. Book excerpt: Sustainable infrastructure development is vital for Africa s prosperity. And now is the time to begin the transformation. This volume is the culmination of an unprecedented effort to document, analyze, and interpret the full extent of the challenge in developing Sub-Saharan Africa s infrastructure sectors. As a result, it represents the most comprehensive reference currently available on infrastructure in the region. The book covers the five main economic infrastructure sectors information and communication technology, irrigation, power, transport, and water and sanitation. 'Africa s Infrastructure: A Time for Transformation' reflects the collaboration of a wide array of African regional institutions and development partners under the auspices of the Infrastructure Consortium for Africa. It presents the findings of the Africa Infrastructure Country Diagnostic (AICD), a project launched following a commitment in 2005 by the international community (after the G8 summit at Gleneagles, Scotland) to scale up financial support for infrastructure development in Africa. The lack of reliable information in this area made it difficult to evaluate the success of past interventions, prioritize current allocations, and provide benchmarks for measuring future progress, hence the need for the AICD. Africa s infrastructure sectors lag well behind those of the rest of the world, and the gap is widening. Some of the main policy-relevant findings highlighted in the book include the following: infrastructure in the region is exceptionally expensive, with tariffs being many times higher than those found elsewhere. Inadequate and expensive infrastructure is retarding growth by 2 percentage points each year. Solving the problem will cost over US$90 billion per year, which is more than twice what is being spent in Africa today. However, money alone is not the answer. Prudent policies, wise management, and sound maintenance can improve efficiency, thereby stretching the infrastructure dollar. There is the potential to recover an additional US$17 billion a year from within the existing infrastructure resource envelope simply by improving efficiency. For example, improved revenue collection and utility management could generate US$3.3 billion per year. Regional power trade could reduce annual costs by US$2 billion. And deregulating the trucking industry could reduce freight costs by one-half. So, raising more funds without also tackling inefficiencies would be like pouring water into a leaking bucket. Finally, the power sector and fragile states represent particular challenges. Even if every efficiency in every infrastructure sector could be captured, a substantial funding gap of $31 billion a year would remain. Nevertheless, the African people and economies cannot wait any longer. Now is the time to begin the transformation to sustainable development.

This book was released on 2002 with total page 212 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book examines the impact of HIV/AIDS on education in sub-Saharan African countries. It looks at the situation at both macro and micro levels and emphasizes the need to react quickly and to institutionalize the response of education systems to the negative consequences of the pandemic. Drawing on studies of a few countries in sub-Saharan Africa, the first part of the book discusses the practicability of implementing a range of indicators for monitoring the impact of HIV/AIDS, specifically on the demand for supply, management, and quality of education at all levels. It underlines the difficulties of assessing and monitoring the impact on demand, supply, and quality in many of the worst affected countries in Africa. The second part focuses on the essential role that the education system has to play in preventing the expansion and mitigating the impact of the epidemic. A range of responses is developed, drawing on the experience of various national and international organizations. This part also presents an overview of the education system in several countries that have attempted programs to impart life skills to children and young people. It considers the problems of evaluating such programs in light of cost effectiveness. (Author/WFA).
