Book excerpt: Associated with both acute kidney injury (AKI) and cardio-renal syndromes (CRS), new biomarkers represent both a popular area of investigation and a new opportunity for advancement of therapy. This book contains the resolutions of the most recent ADQI conferences on biomarkers in AKI (Dublin) and on cardio-renal syndromes (Venice). The first part answers specific questions about new biomarkers and their use and utility in AKI: What are the most suitable candidate molecules and physiologic measures, how solid and evidence based is the discovery phase? How can we incorporate the new biomarkers in the AKI conceptual model describing the evolution from susceptibility to insult, decreased GFR and organ death? Even if we have a positive biomarker pattern and we can identify patients at risk or patients with early or even subclinical AKI, how is this information affecting our clinical behavior and practice? The second part is dedicated to the appraisal of the current knowledge about the pathophysiological mechanisms involved in different forms of CRS: it contains contributions on the state-of-the-art knowledge and practice of CRS, particularly focusing on the pathophysiology of the five subtypes. Acute and chronic mechanisms of damage are explored in depth, with particular attention to the primacy of organ involvement and the subsequent pathways of organ crosstalk.Presenting the most recent research in the field of biomarkers, AKI and CRS, this publication is an important educational tool for advanced investigators and clinical experts, but also for students and fellows.

Book excerpt: Excerpt from Functional Diagnosis of Kidney Disease: With Especial Reference to Renal Surgery Clinical Experimental Investigations Owing to the advances in physical chemistry and through the introduction into medical practice of ureteral catherization, Kidney Diagnosis has lately made the most gratifying progress. We have had opportunity on several occasions to refer briefly to our own participation in the work on the questions pending, and now publish the material which we have collected and investigated on this subject in the course of the last years. We hope that the results of our examinations will lead to a general utilization of the new method in Kidney Diagnosis. We have been aided in our work by Herren Geh. Rath Koenig, Dr. Kummel (Hamburg) and Professor Rotter, who placed appropriate material at our disposal. To them, as well as to Herr Geh. Rath Senator, in whose laboratory the physico-chemical investigations were carried out, we express our most sincere thanks. Book excerpt: This book provides a comprehensive and systematic review of the latest findings in a wide spectrum of clinically important aspects of chronic kidney disease (CKD), focusing on clinical diagnosis and therapeutics. CKD is a global health problem with a rising morbidity and mortality. The last decade has seen significant improvements in determining the incidence, prevalence, and complications of CKD, mainly thanks to the definitions of CKD developed by the National Kidney Foundation’s Kidney Disease Outcomes Quality Initiative (K/DOQI). However, increased recognition of CKD has led to awareness of the limitations of its clinical diagnosis and treatment, which are essential to patients’ wellbeing. This book is of particular value not only to nephrologists, but also to general practitioners and residents with an interest in CKD. It offers a well-organized exposition of the current knowledge base. Compared with previously published books on kidney disease and CKD, it has a smaller number of more concise chapters. As a result, readers can easily obtain an overview of the most important topics in CKD. We hope that practitioners will gain as much from reading this practical guide to clinical management of CKD as we have from editing it.

Available in PDF, EPUB and Kindle. Book excerpt: Nutrition in Kidney Disease, Second Edition addresses the relationships between nutrition and (1) normal kidney function and disease, (2) the progressiveness of chronic kidney disease (CKD) and strategies to prevent further compromise, and (3) the treatment and management of kidney failure especially during medical crises, such as acute kidney injury and its consequent nutritional therapies (e.g., enteral and parenteral nutrition). Demographic patterns, trends and outcomes in the current health care systems are explored in the United States and abroad. Disease prevention and management are presented over the entire lifespan, beginning with pregnancy, followed by infancy, childhood, adolescence, and adulthood, concluding with the elder years. Foundations for clinical practice are established by devoting a complete section towards conducting a comprehensive nutritional assessment, comprising of anthropometric, biochemical, clinical, physical parameters and psychosocial concerns unique to the kidney disease population. Nutritional therapy is also discussed across the spectrum of kidney disease, and pertinent aspects critical to successful management of disorders and conditions, such as bone disease, obesity, and nephrotic syndrome are explored. Nutrition in Kidney Disease, Second edition highlights cutting edge research in regards to exercise and functional outcomes, malnutrition and the inflammatory response, experimental therapies, and the use of complementary and alternative medicine, with a special emphasis on relevant preventative strategies.

Available in PDF, EPUB and Kindle. Book excerpt: This text aids medical students and trainee doctors in developing their knowledge of the conditions covered and improving their ability to clinically assess patients and formulate a management plan. Once doctors are comfortable in interpreting these graphs, they can use them in consultations with patients about their kidney condition and so support self-management by patients, an increasingly important aspect of healthcare. Kidney diseases and the speciality of nephrology have traditionally been regarded as difficult to understand. This is compounded by the use of complex definitions and terminology. These complex definitions result from a need to categorise kidney diseases according to the way kidney function changes over time, and then to express this in numbers and words. Central to the assessment of patients is the measurement of kidney function. In our teaching of medical students and trainee doctors, as well as in our clinical practice and interaction with GPs and consultant colleagues, we have found that viewing graphs of patients’ kidney function (estimated glomerular filtration rate, or eGFR) against time is a much easier way of understanding kidney diseases. It provides the patient’s ‘kidney history’ and leads into a description of the natural history and management of their condition. We were surprised to find that this graphical approach is not used routinely in teaching or in daily clinical practice. We therefore believe there is a need for a book that uses this approach and we thus sought to compile a collection of illustrative case studies covering an array of disease categories, together with patients’ eGFR readings.
