Book excerpt: Introduction : Les fractures ouvertes de jambe avec perte de substance cutanée sont un challenge thérapeutique du fait de la complexité du traitement initial et de la fréquence des complications. Nous proposons donc dans notre étude de nous intéresser aux fractures ouvertes de jambe associées à une perte de substance cutanée ayant nécessité la mise en place d’un lambeau de couverture. L’objectif de cette étude était d’analyser, d’identifier cette population spécifique des fractures ouvertes de jambes et de proposer des recommandations sur la prise en charge thérapeutique. Patients et Méthodes L’ensemble des patients présentant une fracture ouverte de jambe avec une perte de substance cutanée pris en charge au CHRU de Lille entre 2005 et 2010 ont été inclus rétrospectivement de manière consécutive. Il a été recueilli le type, le délai et l’évolution de la couverture ainsi que le mode de reconstruction osseuse primaire et secondaire. Nous avons ensuite précisé l’évolution mécanique infectieuse et fonctionnelle. Résultats : 20 patients ont été inclus dans cette étude. Les fractures et les pertes de substance du tiers inferieur de la jambe étaient les plus retrouvées avec une perte de substance osseuse (PSO) associée à 40%. Le taux de consolidation est de 80 % avec un délai moyen de 16,25 mois. On retrouve un taux de complications à 65% (pseudarthrose mécanique 15%, désaxation 5% infection 25% échec du lambeau 20%) et un taux d’amputation de 20% sur un délai de 76 jours en moyenne (2,5 mois). Le MESS était en moyenne de 5. La qualité du lambeau a été jugée bonne dans 80% des cas. 50% des patients présentaient un périmètre de marche limité et 25% présentaient une raideur articulaire sévère. Conclusion : Même si ces situations restent relativement rares, elles sont de mauvais pronostic. On ne peut raisonner sur ces fractures sans penser à l’évolution naturelle de la pathologie devant l’importance des complications et la morbidité de ces lésions. Il est primordial d’anticiper les complications à venir de façon la plus large possible. Il faut pouvoir garder des ressources thérapeutiques à chaque nouvel évènement qu’engendre la maladie. Il nous parait essentiel d’avoir recours à des recommandations (conformément à celles énoncées par le symposium de la SOFCOT) avant d’engager le patient dans un protocole de soin lourd où le préjudice fonctionnel et la perte de vie vécue seront majeurs.

Book excerpt: La reconstruction des pertes de substance tégumentaire de jambe traumatique s'inscrit le plus souvent dans un contexte de fracture ouverte Gustilo 3, avec un pronostic fonctionnel pour le membre lésé assez médiocre. La prise en charge de ces patients fait donc appel à une équipe multidisciplinaire, alliant des orthopédistes et des plasticiens, dans des séquences temporelles variables. Cette étude rétrospective a cherché à évaluer l'impact du délai de couverture cutanée sur le résultat de la reconstruction dans 56 transferts libres effectués sur une durée de 32 ans. Les résultats obtenus corroborent une supériorité de la reconstruction en urgence de ces pertes de substance tégumentaire de jambe dans le cadre de fracture ouverte, avec un taux d'échec statistiquement inférieur. Cette prise en charge permet également d'éviter les complications à type d'obésité (0 %) de ces fractures ouvertes et autorise la réalisation des ostéosynthèses internes plus stables afin d'améliorer la réduction fracturaire et, par conséquent, le pronostic fonctionnel en autorisant une rééducation plus précoce. Elle diminue aussi le temps d'hospitalisation et le nombre d'interventions subies par les patients. La reconstruction des pertes de substance de jambe est à réaliser en urgence pour en diminuer les complications, à partir du moment où le patient est assez stable hémodynamiquement et ne présente pas de lésions vitales.
